Inspection Summary:
Susan Wilson met with Mr. Wendall Owen at the SCM Mills River site. The site had been off line from August 2010 until
August 2012.
The site is very well maintained and good records are being kept. The entire site is graded so that any stormwater (or
potential spills) are drained down to the concrete basins. There are 3 basins in series for settling (with truck wash going
to the first basin). Recycle water is pumped from the last basin as needed (but usually SCM has to supplementally pump
from Boylston Creek).
Rarely, if ever, can there be a discharge from the site. Behind the concrete basins is a small earthen trap and ditchline.
There was no evidence of discharge from this are to the creek.
The SWPPP should be updated (along with the site plan).

I have a consultant in Charlotte working on SWPPP updates for several sites including Mills River and I told him to move
it to the front of the line. I spoke to Wendell about the samples and I think that he and/or Eddie Teague have been
taking samples before it got offsite. I have told them that from now on if it does not discharge then we will just fill out
the form as no flow. Have a great weekend.

The only comments I had were that the SWPPP needs to be updated (and signed off on) and I was a little puzzled as to
how the analytical samples were taken. Seems like there is rarely, if ever, a discharge event (and when there is — it
probably comes from that earthen ditch).

The storm water/wash water management system serving Southern Concrete Materials' (SCM) Mills River
Plant very effectively accomplishes the goal of operating a ready -mix plant with minimal, if any, discharge of
wash or storm water to the State's surface waters. Documents and records were in order. For the record I
will say that the system is a three cell concrete basin with wash water going to the first cell and storm water
flowing to all cells. Mix water is pumped from the last cell. Exterior truck wash water comes from a well.
Water for keeping the pit level up is pumped from the nearby stream when necessary.

up that they may be inspected in the near future. Since I have listed
each of the regional stormwater contacts in the letters you may be
receiving calls from some of the facilities. FYI -there were 256 letters
for the entire state.

permit regulates the discharge of stormwater and certain wastewaters
from your project site. Due to limited staff resources you may not have
had a Division inspection in the past. With adjustments in other areas
of our program we are beginning to shift additional effort to stormwater
permit inspections. We are prioritizing our efforts in areas where
water quality issues and complaints have arisen in the past.
Over the last few years the Division of Water Quality has responded to
several complaints related to ready mix concrete facilities. Because of
these concerns we are focusing some of our efforts in stormwater
inspections on this industry. Investigations at ready mix sites have
shown that the following problem areas have been the causes of water
quality and stream bank degradation;
- Poorly designed or maintained sediment basins (inadequate size, flow
short-circuiting, etc.)
- Bypassing of sediment basins
- Poorly or inadequately designed waste concrete pits
- pH stream standard violations
- Wetland impacts or violations
Settling basins located in regulatory buffer zones
Routing of unnecessary water through sediment basins (treating
stormwater from non -process areas)
This letter is to inform you that representatives of the Division may
visit your site to inspect your facility in the near future. If your
site is inspected the inspector will look at all aspects of your
operation but will be paying special attention to the items detailed
above which have been problem areas in the past.

permit reissued by the summer of 1999. Once the permit is reissued your facility would be eligible for continued
coverage under the reissued permit.
In order to assure your continued coverage under the general permit, you must apply to the Division of Water
Quality (DWQ) for renewal of your permit coverage. To make this renewal process easier, we are informing you
in advapce that your permit will be expiring. Enclosed you will find a general permit renewal application form.
This form serves two purposes. First, it is an application for renewal of your permit. Second, it serves as an
invoice for the $80.00 permit fee. The application must be completed and returned with the required fee by
February 1, 1999 in order to assure continued coverage under the general permit.
Recent legislation modified the fee structure for DWQ permits. Your general permit fee has been changed to an
annual fee of $80.00. A copy of the new fee schedule is enclosed in this package.
Failure to request renewal within this time period may result in a civil assessment of at least $250.00. Larger
penalties may be assessed depending on the delinquency of the request. Discharge of stormwater from your
facility without coverage under a valid stormwater NPDES permit would constitute a violation of NCGS
143-215.1 and could result in assessments of civil penalties of up to $10,000 per day.

subject state — NPDES general permit- This permit is issued pursuant to the requirements of
North Carolina General Statute 143-215 .1 and the Memorandum of Agreement between North
Carolina and the US Environmental Protection agency dated December 6, 1983.
If any parts, measurement frequencies or sampling requirements contained in this permit
are unacceptable to you, you have the right to request an individual permit by submitting an
individual permit application. Unless such demand is made, this certificate of coverage shall be
final and binding.
Please take notice that this certificate of coverage is not transferable except after notice to
the Division of Water Quality. The Division of Water Quality may require modification or
revocation and reissuance of the certificate of coverage.
This permit does not affect the legal requirements to obtain other permits which may be
required by the Division of Water Quality or permits required by the Division of Land
Resources, Coastal Area Management Act or any other Federal or Local governmental permit
