On February 25, 2016 Glen White and Sue White of the North Carolina Department of
Environmental Quality — Minerals & Land Resources met with Keith York at the subject facility
located at 701 Snyder Street; Wilkesboro to conduct a compliance inspection for the facilities
Industrial Stormwater Permit.
Permit:
This facility holds General Stormwater Permit NCG110031 to discharge stormwater from
industrial activity classified as Treatment Works treating domestic sewage or any other
sewage sludge or Wastewater Treatment Device or System under the National Pollutant
Discharge Elimination System (NPDES). The permit became effective .Tune 1, 2013 and expires
on May 31, 2018. A copy of the current permit is required to be maintained with the Stormwater
Pollution Prevention Plan (SPPP) documents. A copy of the current permit was available for
review at the time of inspection.
Records/Reports-
This facility was required to develop and maintain a Stormwater Pollution Prevention Plan
(SPPP) in accordance with Part 1I, Section A of the permit. Inspectors reviewed the SPPP and
each of its components. Documents appeared to be complete and up to date.
Facility Site Review:
Mr. York took inspectors to review the facilities and the location of each outfall. There are (3)
three stormwater outfalls. All outfalls exit into the adjoining Cub Creek.
Effluent Receivin p, Waters:
Stormwater Effluent from this facility drains into Cub Creek, Class "C" graters of the Yadkin
Pee -Dee River Basin.

Self-MonitoringProgram:
This facility is not required to provide analytical monitoring as a condition of its stormwater
permit requirements. Testing is performed as part of the wastewater treatment requirements.
Qualitative monitoring is conducted as required.

Analytical monitoring for on -site vehicle maintenance is not required, since an average 55 gallons per month of new
motor oil is not used.
Facility Site Review
The site plan identified three (3) stormwater outfalls. The Town has secondary containment measures to control chemicals
in the event of a spill.
Self -Monitoring Program
Stormwater Discharge Outfalls (SDO) were inspected semi-annually and qualitatively monitored as required in the permit.

15 2012..Failure to request renewal within this time period will result in the inactivation of your expired
permit in our system. Discharge of Stormwater without coverage under a valid stormwater NPDES permit
constitutes a violation of N.C. General Statute 143-215.1 and may result in the assessment of civil
penalties of up to $25,000 per day.

Records & Reports
The basic elements of Stormwater Pollution Prevention Plan SPPP were developed.
Analytical monitoring for on -site vehicle maintenance is not required, since an average 55 gallons per month of new
motor oil is not used.
Facility Site Review
The site plan identified three (3) stormwater outfalls.
The Tovm has secondary containment measures to control chemicals in the event of a spill.
Self -Monitoring Program
The facility had all monitoring records on -site.
Stormwater Discharge Outfalls (SDO) were inspected semi-annually and qualitatively monitored as required in the permit.

In November 2007 the town did return our Rescission Request form instead of our
renewal form for the Cub Creek WWTP. That means they did indeed request that we
rescind their permit. We have logged the request into BIMS, but we haven't processed
it, yet.
They note on the form, "Within the footprint of the Wastewater Plant there is no
stormwater discharged to any receiving streams by means of pipes, ditchlines or
culvert." However, based on your report, we will inform them that their request has
been denied. Can"you fill in the blanks for us by sending me the basis for your
recommendation? I think we need to have that recorded in our file on the facility.
