Dear Mr. Bolich,
Thank you for your patience in waiting for the laboratory results you requested on related
to sanitary sewer incident # 201901533. This letter is intended to identify the steps taken
during the collection.
The sample collected were in response to a sanitary sewer spill located 5005 Oak Park Road,
Raleigh, NC, which occurred on September 15t^, 2019. This incident spilled approximately
13,400 gallons of untreated sewer into an unnamed tributary of the Crabtree basin.
In your request, you specified that the following test were conducted: COD, Ammonia,
Nitrate, Nitrite, and Fecal Coliform. These tests were collected and delivered to the Public
Utilities laboratory, located and the Neuse River Resource Recovery Facility, on 09/18/2019.

The samples were collected on four separate locations downstream of the spill, which
stretched to approximately 2,400 ft. from the site of the spill to the spill collection site.
