Dear Mr. Weaver:
This letter presents a request for termination of the National Pollutant Discharge Elimination System
(NPDES)permit referenced above for the Don Lee Gas and Grocery site(the"site"). The site groundwater
recovery and treatment system was shut down on February 1,2019 as requested by the North Carolina
Department of Environmental Quality(NCDEQ)-Division of Waste Management—Underground Storage
Tank(UST)Section,and has remained offline since.
The UST Section approved a request for permanent shut-down of the site groundwater recovery and
treatment system in an Acknowledgement of Report Receipt letter dated August 6,2019(attached). In
conjunction with this approval, Piedmont Geologic is requesting the termination of NPDES Certificate of
Coverage NCG510527,effective immediately.

The UST Section,Division of Waste Management,is in receipt of the Groundwater Monitoring
Report dated August 2,2019,and the Remedial Action Progress Report date August 5,2019. After
reviewing the reports UST staff recommends the following:
• Permanent shut down of remedial system
• Updated well receptor survey
• Fate transport groundwater model to show impact to potential receptors
