Failure to have required records available at the time of inspection has been an issue going back several inspections and needs to be addressed. The inspection report and DWR example logs do a very good job outlining what records are needed and to some degree the order in which they may be reviewed by an inspector. Additional records were provided 6-8 weeks after the inspection, but inspector cannot afford to wait to review these that long. Records that were lacking most importantly were: 1) Info on commercial locations that require grease controls. 2) Annual Wastewater Performance Reports are required to be submitted to DWR for both the treatment facility and the collection system permits; a combined report can be done but a copy must be sent to the applicable DWR permitting group in Raleigh for each permit. 3) Gravity sewer cleaning records by year and keep current year up-to-date within a month. 4) Pump station pump or power failure contingency plan. Operational Issues: 1) Need to ensure required minimum sewer line cleaning is being done. 2) Couple of stations had issues with the alarms that needed to be addressed; see report. Page 2 of 7 Permit: WQCS00178 Owner - Facility: Town of Chadbourn Inspection Date: 07/17/2019 Inspection Type : Collection System Inspect Non Sampling Reason for Visit: Routine General Yes No NA NE Is there a properly designated primary ORC and at least one back-up of proper grade? 0 ❑ ❑ ❑ Are logs being reviewed by the system management or owner on a regular basis? 0 ❑ ❑ ❑ # What is that frequency? weekly # Are there any issues being addressed currently or any in the planning stages? 0 ❑ ❑ ❑ Is there a specific pump failure plan available for all pump stations? ❑ ❑ ❑ Does plan indicate if pump parts/new pumps are in spare parts/equipment inventory? ❑ ❑ ❑ Are new/significantly upgraded pump stations equipped with anti -corrosion materials? ❑ ❑ ❑ Does the permittee have a copy of their permit? ❑ ❑ ❑ # Is permit expiring within the next 6 months? ❑ ❑ ❑ If Yes, has the Permittee applied for renewal? ❑ ❑ ❑ Comment: There is a very aeneric pump failure plan. but each station or arouD of stations if identical pumps should indicate what back-up pumps or parts are in inventory, where they can be acquired if not, plan on how to maintain functionality in the event that both fail, etc. Should consider installation of a bypass on the force main to allow use of a mobile pump. Sewer and FOG Ordinances Yes No NA NE Is Sewer Use Ordinance (SUO) or other Legal Authority available? ❑ ❑ ❑ Does it appear that the Sewer Use Ordinance is enforced? ❑ ❑ ❑ Is there a Grease Control Program that legally requires grease control devices? ❑ ❑ ❑ What is the standard grease trap cleaning requirement in the FOG ordinance? 30 days Is Grease Control Program enforced via periodic inspections/records review? ❑ ❑ ❑ Is action taken against violators? ❑ ❑ ❑ # Have satellite systems adopted an equivalent or more stringent ordinance? ❑ ❑ ❑ Is grease/sewer education program documented with req'd customer distribution? ❑ ❑ ❑ # Are other types of education tools used like websites, booths, special meetings, etc? ❑ ❑ ❑ If Yes, what are they? (This can reduce mailing to annual.) Comment: Records very poorly maintained in regards to what commercial customers have grease controls, what type, required cleaning minimum, and if any actions taken on inspections which a few were indicated. Town needs to use the DWR logs provided online as a basis for minimum information needed & activities documented. FOG/sewer education materials should be placed on the Town's website. Capital Improvement Plan Yes No NA NE Has a Capital (CIP) or System Improvement Plan been developed and adopted? ❑ ❑ ❑ Is it designated for wastewater only or does it have a dedicated section? ❑ ❑ ❑ Does CIP cover three to five year period of earmarked improvements? ❑ ❑ ❑ Does CIP include description of project area? ❑ ❑ ❑ Does CIP include description of existing facilities? ❑ ❑ ❑ Page 3 of 7 Permit: WQCS00178 Owner - Facility: Town of Chadbourn Inspection Date: 07/17/2019 Inspection Type : Collection System Inspect Non Sampling Reason for Visit: Routine Does CIP include known deficiencies? 0 ❑ ❑ ❑ Does CIP include forecasted future needs? 0 ❑ ❑ ❑ Comment: A new sewer study and asset inventory is supposedly underway; this may generate new projects or changes to existing proposed projects to incorporate into the CIP. Map Is there a overall sewer system map? Does the map include: Pipe Type (GS/FM) Pipe sizes Pipe materials (PVC, DIP, etc) Pipe location Flow direction Approximate pipe age Pump station ID, location and capacity # Force main air release valve location & type # Location of satellite connections Is the map being updated for changes/additions within 1 year of activation? Comment: Some additional detail is needed on their map or mapping system, but the inventory study underway should help with it. Reporting Requirements # Have there been any sewer spills in the past 3 years? If Yes, were they reported to the Division if meeting the reportable criteria? If applicable, is there documentation of press releases and public notices issued? Is an Annual Wastewater Performance Report being filed with the Division, if required? Is the report being made available to all its sewer customers? Yes No NA NE ■❑❑❑ Yes No NA NE ❑■❑❑ ❑❑❑■ # How is it being made available? Comment: 2018 Annual Report for the WWTP found on the Town's website, but not collection system. No evidence of submittals to DWR central office for either report over the last several vears. Inspections Are adequate maintenance records maintained? Are pump stations being inspected at the required frequency? # Is at least one complete functionality test conducted weekly per pump station? Is there a system or plan in place to observe the entire system annually? Is the annual inspection documented? # Does the system have any high -priority lines/locations? Are inspections of HPL documented at least every 6 months? Yes No NA NE ■❑❑❑ ❑■❑❑ ❑■❑❑ ■❑❑❑ ■❑❑❑ Page 4 of 7 Permit: WQCS00178 Owner - Facility: Town of Chadbourn Inspection Date: 07/17/2019 Inspection Type : Collection System Inspect Non Sampling Reason for Visit: Routine Are new lines being added to the HPL list when found or created? Comment: Need to develoD an annual insDection Droaram for the entire sewer system. Svstem alona & ❑❑❑■ within roadways are easier versus cross-country and many owners will spray paint manholes/ARVs annually after checked to track movement. Could use better documentation of HPL inspections and again recommend the DWR logs as a basis for their records. Spill Response Action Plan Yes No NA NE Is a Spill Response Action Plan available? 0 ❑ ❑ ❑ Is a Spill Response Action Plan available for all personnel? ❑ ❑ ❑ Does the plan include: 24-hour contact numbers 0 ❑ ❑ ❑ Response time ❑ ❑ ❑ Equipment list and spare parts inventory ❑ ❑ ❑ Access to cleaning equipment ❑ ❑ ❑ Access to construction crews, contractors, and/or engineers ❑ ❑ ❑ Source of emergency funds ❑ ❑ ❑ Site sanitation and cleanup materials ❑ ❑ ❑ Post-overflow/spill assessment ❑ ❑ ❑ Does the Permittee appear to respond within 2 hours of first knowledge of a spill? ❑ ❑ 0 ❑ Comment: Need to have the Plan available and ensure it is reviewed/updated each year. alll[L Yes No NA NE Is system free of known points of bypass? ❑ ❑ ❑ If No, describe type of bypass and location Are all spills or sewer related issues/complaints documented? ❑ ❑ ❑ # Are there repeated overflows/problems (2 or more in 12 months) at same location? ❑ ❑ ❑ # If Yes, is there a corrective action plan? ❑ ❑ 0 ❑ Comment: Lines/Right-of-Ways/Aerial Lines Yes No NA NE Please list the Lines/Right of Ways/Aerial Lines Inspected: Are right-of-ways/easements maintained for full width for access by staff/equipment? ❑ ❑ ❑ If No, give details on temporary access: Is maintenance documented? ❑ ❑ ❑ Are gravity sewer cleaning records available? ❑ ❑ ❑ Has at least 10% of lines older than 5 yrs been cleaned annually? ❑ ❑ ❑ Were all areas/lines inspected free of issues? ❑ ❑ ❑ Page 5 of 7 Permit: WQCS00178 Owner - Facility: Town of Chadbourn Inspection Date: 07/17/2019 Inspection Type : Collection System Inspect Non Sampling Reason for Visit: Routine Comment: Need to document cross country ROW/easement inspections and maintenance at least annually. Once again use DWR lops as a basis for their records is recommended. Must average at least 10% over the last 3 years and recommend use of a spreadsheet for tracking progress toward annual goal and is one of MR'S logs. Cleaning & video work for inventory study counts and signifcant cleaning in excess of 10% can be used in subsequent years. Required minimum is 10,700 ft per year.
