Inspection Summary:
The regional office continues to have concerns about the Permittee's continuing lack of O&M on the collection system &
WWTP and oversight of connections onto the collection system.
The Main PS still lacks having its second submersible pump installed and a bypass pump connected; this was observed in
May 2017. This pump station receives all the flow from within the subdivision and two additional subdivisions are planned to
pump to this station. Two other stations had inoperable generators with logs showing them down as far back as 8-2018 and
luckily Hurricane Dorian earlier this month wasn't too bad and didn't cause loss of power.
ONS reported that there were 153 active residential customers on the system as of 8-1-2018 without considering the newer
Majestic Oaks West, however, the Division only shows 126 potentially active connections approved. In response to this
information the current system map was requested, but was never provided and was not available during this inspection.
That concern is growing given observations during recent inspections of both the collection and treatment/disposal systems
and in that ONS has agreed to accept flow from two proposed subdivision projects (Southside Commons approved in 6-2008
for 106 units and Salters Haven in 2-2019 for 310 units) with Salters Haven under construction.
Currently, neither the Main PS nor the WWTP appears capable of accepting additional flow in the near future.
A Notice of Violation with Intent to Assess is being issued.

Comment: The Main PS #1 is still missina its 2nd pump with a bypass DumD still in place as first noted b
RO staff in May 2017. PS #2 is not posted and the generator has been out of service since
8-2018. PS #4 generator been out of service since 10-2018. ONS claims no responsibility for
the 13 simplex units in the Grant Dr system (W00036244) and there are at least 7 active homes
in the eastern side of MO West that ONS is required to modify per WQ0040002 issued in
2-2019.
