Reason for Inspection: Routine Inspection Type: Collection System Inspection Sampling Permit Inspection Type: Deemed permitted collection system management and operation Facility Status: Compliant Not Compliant Question Areas: Miscellaneous Questions General Grease Control Spills/Response Plan Inspections Lines Manholes Pump Stations (See attachment summary) Page 1 of 6 Permit: WQCSDO526 Owner - Facility:904 Georgetown Treatment Plant LLC Inspection Date: 08/16/2019 Inspection Type : Collection System Inspection Sampling Reason for Visit: Routine Inspection Summary: This inspection represented the 3rd inspection of the collection system and was originally plannned for September 2018 until Hurricane Florence hit. The inspection was conducted in conjunction with the treatment facility. The area received significant rainfall overnight and during the inspections which seemed to be pushing the WWTF even though its permitted for 150K and only sees an average of about 60-70K per day. Consequently, the collection system does have some vulnerability showing itself as inflow which should be investigated to determine where the excess flow is originating from and if it can be eliminated/reduced via repair even of broken/missing cleanouts or installation of manhole rain stopper inserts that prevent rainfall from entering thru or around the covers. Gravel was observed on the WWTF's bar screen, thus there must be a leak on a gravity line leading into the plant's influent pump station or passing through solids handling pumps at a system pump station which isn't good for pumps. The Utility has yet to create an overall map of the system which was mentioned as a requirement back in 2014 & 2015, but has at least some of the sewer construction record drawings. In addition, the Utility needs to locate its pump station designs or have an as -built design generated by a PE based on what is currently installed and its connectivity. The first sewer systems were approved for construction in the development back in the late 1990s thus some of the system is approaching nearly 20 years old. The Utility should be planning to start cleaning some of the older gravity lines soon as the goal should be clean lines every 10 years and likely the lines have never been cleaned except in the areas of blockages. Cleaning followed by video inspection would be good to help locate line issues. Two of the permits issued for the construction of portions of the sewer system within the community having pump stations required on -site portable generators, thus the Utility should have two on hand. At the very least one portable generator or bypass pump (if bypasses installed) must be purchased. A Notice of Violation with Intent to Assess is being issued for the reoccuring issues. This may require a survey to be conducted if the electronic files cannot be used or obtained to make one. Cleaning & camera inspections may be needed to assist with gravity sewer mapping. All the items above are ideal to include if applicable, but at least the first 3 items are required. These should be documented to see if its a reoccurring issue that may need to be investigated in case a partial or temporary blockage of the mains are contributing or if the material causing the service line blockages is making its way into the main system. Plan is very simplistic, but should be updated to include some or all the above. Inspections Are adequate maintenance records maintained? Are pump stations being inspected at the required frequency? The maintenance station had issues with both the visual and audible alarms, the float was triggering the alert to the panel, but nothing was happening. Logs are good -- Utility needs to have a copy of these available for 3 years thus suggest using a monthly inspection log like DWR's example forms that operator hands in monthly. Page 6 of 6
