o You must submit a copy of your application to the NC Wildlife Resources Commission (WRC) since your
proposed work is in a designated trout water county (see remarks section below for the address of your
WRC representative)

1) Please provide a compensatory mitigation proposal or a statement describing how the mitigation
requirement will be satisfied. If you plan to utilize the North Carolina Ecosystem Enhancement
Program (NCEEP) to satisfy the compensatory mitigation requirement, please provide a letter from
the NCEEP that states that they are willing to accept your payment for the proposed impacts.

2) Please provide a Routine Wetland Data Form for the wetland and the upland areas.

Please provide the requested information to our office within 30 days of receipt of this
Notification or we will consider your application withdrawn and close the file.
