(Mulkey) has been retained by Novartis Vaccines and Diagnostics (Novartis) to prepare an annual report summarizing the inspection and maintenance events associated with the south reinforced earthen slope structure from May 2008 to May 2009. The annual inspection reports are required by the North Carolina Division of Water Quality (NCDWQ) Section 401 Water Quality Certification (DWQ Project # EXP 06-1376) issued for the Novartis project on September 29, 2006. This report is meant to summarize observations, issues, and maintenance conducted on a weekly basis since construction. Photos of the reinforced slope are attached to this letter report. A. Project Description The south reinforced earthen slope is located on the southern side of the Novartis campus in Holly Springs, North Carolina (Figure 1 and Drawing C JSK-8). It was built to minimize jurisdictional stream impacts on the subject property. According to Qore Property Services (Qore), the slope was constructed utilizing approved fill material placed in 9-inch lifts, with two rows of lifts comprising each row of metal baskets (Drawing 00-C-60-99-2). At the completion of each row, the top was dressed with topsoil to ensure acceptable root growth from the sowed grass seed. This reinforced earthen slope was completed and approved by Qore on April 3, 2007 (attached). Per Special Condition # 8 of the NCDWQ water quality certificate issued on September 29, 2006, inspection reports of the reinforced earthen slope are required on an annual basis. These assessments are to be performed monthly for the first six (6) months and quarterly for three (3) years thereafter. The reports are to include documentation of any structural failings, significant mortality of cover vegetation, and the description of any repairs based on periodic visual assessments. Weekly visual assessments of the reinforced earthen slope were conducted by Jacobs Engineering Qacobs) beginning the week of April 16, 2007 and have continued to the present. This report represents the second inspection report submittal and even though only quarterly site inspections are required, Jacobs inspections consisted of weekly visual assessments from May 26, 2008 to May 29, 2009. Also included are photos of the reinforced earthen slope. Inspections Below is an outline of minor issues that were observed and noted during the inspection events for the past year according to inspection documentation provided by Jacobs. No issues were observed and no significant rain events occurred during the weeks that are not referenced below. • June 15, 2008 - no issues following 1 3/4 inches of rain • July 6, 2008 - no issues following 1 '/4 inches of rain • September 6, 2008 - no issues following 5 inches of rain (Storm Hanna) • September 16, 2008 - no issues following 1 inch of rain • September 25, 2008 - no issues following 1 1/2 inches of rain • November 15 and November 16, 2008 - no issues following 1 '/4 inches of rain • November 30, 2008 - no issues following 1 1/2 inches of rain 0 December 11, 2008 - no issues following 3 1/2 inches of rain; cleaned all silt fences December 12, 2008 • January 20, 2009 - no issues following 4 inches of snow 0 February 28, 2009 - no issues following 2 inches of rain • March 1 and March 2, 2009 - no issues following 2 '/2 inches of snow • March 14 and March 15, 2009 - no issues following 1 '/4 inches of rain • March 28 and March 29, 2009 - no issues following 1 '/2 inches of rain • May 17 and May 18, 2009 - no issues following 1 '/2 inches of rain C. Conclusions The inspection data from May 2008 through May 2009 demonstrates that the south reinforced earthen slope structure was tested numerous times during this monitoring period yet there have been no major issues or failings. Our scope of services entailed an evaluation of the soil subgrades prior to fill placement, observation of the wire-basket placements, observation of the geogrid placement, and in-place density testing of the fill materials. The structural fill materials were generally placed in 9-inch lifts (two lifts per row of baskets) and the steps formed by the baskets were topped with topsoil. Field reports were prepared on a daily basis as the work progressed and these reports are submitted under separate cover. In summary, it is our opinion that the work described above complies with the requirements of the project Drawings and Specifications based on our professional judgment from field observations, field tests, and limited laboratory tests conducted by QORE.
