Causeway Dr Winston Salem, NC 27106 Subject: REQUEST FOR ADDITIONAL INFORMATION High Knoll Subdivision Dear Mr. Ramey: On September 13, 2019, the Division of Water Resources — Water Quality Programs (Division) received your application dated August 15, 2019, requesting a 401 Water Quality Certification from the Division for your project. The Division has determined that your application is incomplete and cannot be processed. The application is on -hold until all of the following information is received: 1. A complete review of the project cannot be conducted until the U.S. Army Corps of Engineers (USACE) has verified all stream and wetland locations for the proposed project. [15A NCAC 02H .0502(a)(7)]. 2. The Division requires confirmation from the USACE that the Nationwide Permit #14 is appropriate for this project prior to issuance of a 401 Certification. 3. The Division recommends that you verify that the project does not require application to, and approval from the USACE. 4. Please provide a 1:24,000 USGS Topographic Map with the site boundaries clearly drawn or delineated. DWR# 20041150 v2 Request for Additional Information Page 2 of 3 Please submit site plans with proposed lot layout on full plan sheets at a scale of no smaller than 1"= 50' with topographic contours shown. Please locate all wetlands, streams, and other waters of the State as overlays on the site plan. [15A NCAC 02H .0502(b)] 6. Please enumerate all wetland and stream impacts including fill slopes and dissipater pads, etc. on the site plan and clearly label impacts (Wetland/ Stream Impact 1, etc.). The Division recommends highlighting by hatching or shading all impacts areas for temporary impact areas and permanent impact areas. [15A NCAC 02H .0502(g)] 7. The impact drawing provided with the application does not match the wetland impact table provided in the PCN. Temporary impacts for the roadway are shown on the impact drawing but not identified in the impact table. Please revise the impact drawing and/or the impact table to ensure consistency and please ensure that all impacts are clearly labeled and identified in both locations. [15A NCAC 02H .0502(b)] 8. Please detailed a construction sequence and a restoration plan for the temporary wetland impact area noted as "utility". [15A NCAC 02H .0502(b)] 9. Based on the impact drawing provided, it appears that the proposed activities would leave a remnant wetland on the downslope side of the road above the riprap pad which will be disconnected from the hydrology of the remaining wetland above the culvert. It is likely that this remaining wetland area will not retain sufficient hydrology to remain jurisdictional/functional and should be included in the impacts table as indirect impacts. [15A NCAC 02H .0502(b)] 10. Please provide a detailed engineering plan, profile view, and cross-section of the proposed culvert. These drawings must include details regarding adjacent stream alignment in relation to pipe alignment, pipe slope, pipe burial, and dissipater pad burial. [15A NCAC 02H .0502(b)] 11. It appears that some lots adjacent to the proposed road may include wetlands and/or streams within the lot limits. Please provide building envelopes for all lots with wetlands or streams on the site plans to demonstrate these lots are able to be developed without additional jurisdictional impacts. [15A NCAC 02H .0502(g)] Pursuant to Title 15A NCAC 02H .0502(e), the applicant shall furnish all of the above requested information for the proper consideration of the application. Please respond in writing within 30 calendar days of receipt of this letter by sending one (1) copy of all of the above requested information to the 401 & Buffer Permitting Branch, 1617 Mail Service Center, Raleigh, NC 27699-1617 OR by submitting all of the above requested information through this link: https://edocs.deg.nc.gov/Forms/Supplemental-Information-Form (note the DWR# requested on the link is referenced above). The denial of this project will necessitate reapplication to the Division for approval, including a complete application package and the appropriate fee. Please be aware that you have no authorization under the Water Quality Certification Rules for this activity and any work done within waters of the state may be a violation of North Carolina General Statutes and Administrative Code.
