Please find enclosed for the referenced project the original Fast -Track Application (FTA
04/16), the Flow Tracking/Acceptance for Sewer Extension Permit Applications (FTSE-
10/18), the USGS Topographic Map, the original Stream Classification — Watershed
Classification Attachment (WSCAS-0 I - 15), CMSD flow acceptance letter, existing permit
number WQ0038222, and one copy of each for your review and approval. Also attached is the
application fee of $480.00 (check # 14333) and the following brief narrative.
This is a modification to the existing permit number WQ0038222 (copy attached) that was
issued for the Phase One portion of the project. In addition to adding additional property and
lots, this modification request also includes a partial reconfiguration of what was covered
under the existing permit. Tracts "A" and "B" as shown on the attached map were included
under the existing permit number. This modification request includes adding new Tract "C"
and a reconfiguration of the yet to be constructed Tract `B".
Note that the total flow request is now 55,320 gpd, of which 40,800 gpd had already been
allocated.
This project is now a 122-lot single family subdivision which will be served entirely by
gravity -sewer. Wastewater flow generated by this development will discharge into an existing
sewer pump station owned and operated by the Town of Winterville. Ultimate treatment will
be the Contentnea Metropolitan Sewerage District.
Therefore, we are requesting the following items within the existing permit be modified:
• Change Project Name to "Villa Grande Phase One and Phase Two'

• Increase permitted by 14,520 gpd, for a total permitted flow of 55,320 gpd (36 single
family 3-bedroom residential dwellings @ 360 GPD/ dwelling; 72 single family 4-
bedroom residential dwellings @ 480 GPD/ dwelling; 13 single family 5-bedroom
residential dwellings @ 600 GPD/ dwelling)
• Change line construction to approximately 6,126 linear feet of 8-inch gravity sewer;
and approximately 560 linear feet of 10-inch gravity sewer

III. Certification Statement:
I Charles M. Smithwick, Jr. certify to the best of my knowledge that the addition of
the volume of wastewater to be permitted in this project has been evaluated along the route to the
receiving wastewater treatment facility and that the flow from this project is not anticipated to
cause any capacity related sanitary sewer overflows or overburden any downstream pump station
en route to the receiving treatment plant under normal circumstances, given the implementation
of the planned improvements identified in the planning assessment where applicable. This
analysis has been performed in accordance with local established policies and procedures using
the best available data. This certification applies to those items listed above in Sections I and II
plus all attached planning assessment addendmns for which I am the responsible party.
Signature of this form indicates acceptance of this wastewater flow.

111. Certification Statement:
I Travis Welborn certify to the best of my knowledge that the addition of
the volume of wastewater to be permitted in this project has been evaluated along the route to the receiving
wastewater treatment facility and that the flow from this project is not anticipated to cause any capacity
related sanitary sewer overflows or overburden any downstream pump station en route to the receiving
treatment plant under normal circumstances, given the implementation of the planned improvements
identified in the planning assessment where applicable. This analysis has been performed in accordance
with local established policies and procedures using the best available data. This certification applies to
those items listed above in Sections I and II plus all attached planning assessment addendums for which I
am the responsible party. Signature of this form certifies that the receiving collection system or treatment
works has adequate capacity to transport and treat the proposed new wastewater.

creation of nuisance conditions, the Permittee shall take immediate corrective action, including those
as may be required by this Division, such as the construction of additional or replacement facilities.
[15A NCAC 02T .0110; 15A NCAC 02T .0108(b)]
