I am looking at the renewal for our NCG080677 permit and notice that the address needs to be
updated. The correct address for our Public Works Shop is 730 South Smith Street, Nashville NC
27856. Once I see this is updated I will sign and submit.

It was good speaking with you earlier this morning. The permits I found attached to the Town of
Nashville are the following:

NCG080677 – Public Works Shop (industrial general stormwater permit for vehicle maintenance
activities), and
NCS000485 – Stormwater MS4 permit

The NCG08 renewal Certificate of Coverage was never obtained for this site. The attached letter was
sent out last year about it. You need to go on-line here and request that COC record:

You also need to familiarize yourself with the NCG080000 General Permit requirements. You can
find that information on this section of our website.

For NCS000485, you can access all the MS4 permit records we have in our Laserfiche Repository for
this permit here. I have copied Jeanette Powell on our staff who is our MS4 Program Coordinator if
you have questions. (Incidentally, once you have requested the COC for NCG080677, you can
navigate to the General Permits folder in the repository and see a copy there, too).

For both of these permits, it’s very important that you review the permit contacts with this report.
You will type in the permit number and click “ok” for each. Please notify Suzanne McCoy of any
contact updates that are necessary. If Legally Responsible Owner affiliation needs to be changed,
you’ll have to use this on-line form below. Other contact updates can just be emailed to Suzanne.
