Box 610 Four Oaks, NC 27524 Subject; Compliance Evaluation Inspection Town of Four Oaks Wastewater Collection System Permit No. WQCSDO013 Johnston County Dear Mr. Parker: On September 19, 2019, Jason Robinson of the NC Division of Water Resource's Raleigh Regional Office (RRO) conducted a routine compliance evaluation inspection of the Town of Four Oaks' Wastewater Collection System. Mr. Barry Stanley (Public Works Director), Mr, Chris Bardour, and Ms. DeAnna Haley was appreciated. The inspection report is attached (Attachment A). The following observations were made: I. The Town's Collection System is considered a "deemed permitted" system, or "Permitted by Regulation" (no individual permit), which is typical for systems with flows less than 200,000 GPD. The system is covered by a deemed permit number (WQCD0013) and is classified as a CS-2 system. Collection systems that are "Deemed permitted/Permitted by Regulation" must meet the requirements of 15A NCAC 02T .0403 (Attachment B). If the system exceeds an average of 200,000 GPD, the Town should notify the Division. 2. Mr. Barry Stanley is the Public Works Director and serves as the Primary Operator in Charge (ORC) and is a CS-2 operator. Mr. Chris Barbour serves as the backup Operator (bORC) and is a CS-1. DWR's database needs to be updated to reflect the correct operators for the system. Please complete the attached Operator's Designation Form Attachment C to designate both the primary operator ORC and backup operator bORC . Please submit it to the address on the form. 3. A complete map of the sewer system was available for review (15A NCAC 0@T .0403 (a)(2)). 4. A current "Operation and Maintenance Plan" that includes an "Overflow (SSO) Response Plan" was not available during the inspection. These are required per 15A NCAC 02T .0403 (a)(3). Pleasedevelop an Cleration and Maintenance Plan that specifies the pump station inspection frequency, spare varts inventory, schedule to test emerizency equipment, Preventative maintenance schedule and an Overflow (SSO) Res once Plan". Information is also available online. Please make sure to keep a record of when these Pamphiets a rea re sent out. Also dease determine the locations of the rease tra s tra s that flow to the system and develop a program for inspections/enforcement of these. 15A NCAC 02T .0403 (8). 6. Records were available that demonstrated the entire collation system is observed at least once a year. (15A NCAC 02T .0403 (5)) 7. DWR records show that a spill has not been reported by the Town since 2011. You are reminded that spills greater than 1,000 gallons, and any discharges that reach surface waters (regardless of size) shall be reported to the Department as soon as practicable, but no later than 24 hours after the discharge has reached surface waters, as required by 15A NCAC 02B .0506(a) and G.S. 1430215.2C. (15A NCAC 02T.0406 (7)) 8. The town has two high -priority lines that are inspected weekly. Documentation was available for review. Both high priority lines were visited during the inspection. No issues were found. 9. The city has five pump stations. Two have SCADA and are inspected at least once a week. Three do not have SCADA and are inspected daily. All stations have permanent generators and automatic transfer switches. Generators test automatically every Monday. The Town also has a service contract for the generators. Log books are available at each pump station where pump run times are recorded. Records should also be kept of any testing or maintenance that is done at each station (testin2 alarms and generators exercising valves vacuuming grease from wet well etc.. These records can be kept at each DUMV station. 10. The following pump stations were inspected. High water alarm floats were pulled to test audible and visual alarms. Generators were tested. Upstream manholes were opened and observed. No issues were found. a. Northside Pump Station —No telemetry. b. Austin Acres Pump Station —No telemetry c. Forest Hills Pump Station —SCADA d.
