Wastewater Irrigation System, Permit Number WQ0001284

This letter is in response to your letter dated August 21, 2019. Listed below are our answers to the violations noted:
Ill. Operation and Maintenance Requirements:
Condition I. The Town of Conway has contacted a contractor to replace the door on the shop at the lagoon. Once
the new door is installed the room will be secured against unauthorized entry.
IV. Monitoring and Reporting Requirements:
Condition 4. 1 will begin immediately to report all parameters on NDMR's and also report Chlorides on the required
forms. I will verify, before submitting, all forms have been signed by the ORC and the Permittee. Once I receive
the up-to-date forms I will begin using those,
Condition 11. 1 will ensure monitoring well sampling documentation submitted to DWR includes form GW-59,
laboratory analyses, GW-59a Certification Form, and well samples are analyzed for Total Phosphorous (TP). l have
attached the GW-59 and GW-59a documents you requested.
