Facility was compliant during calendar year 2014 with all conditions of the land application permit
(including but not limited to items 1-12 below) issued by the Division of Water Quality. ® Yes ❑ No
If no please, provide a written description why the facility was not compliant, the dates, and explain
corrective action taken.
1. Only residuals approved for this permit were applied to the permitted sites.
2. Soil pH was adjusted as specified in the permit and lime was applied (if needed) to achieve a soil pH of at
least 6.0 or the limit specified in the permit.
3. Annual soils analysis were performed on each site receiving residuals during the past calendar year and three
(3) copies of laboratory results are attached.
4. Annual TCLP analysis was performed and three (3) copies of certified laboratory results are attached.
5. All other monitoring was performed in accordance with the permit and reported during the year as required
and three (3) copies of certified laboratory results are attached.
6. The facility did not exceed any of the Pollutant Concentration Limits in Table 1 of 40 CFR Part 503.13 or the
Pollutant Loading Rates- in Table 2 of 40 CFR part 503.13 (applicable to 40 CFR Part 503 regulated facilities).
7. All general requirements in 40 CFR Part 503.12 and management practices in 40 CFR Part 503.14 were
complied with (applicable to 40 CFR Part 503 regulated facilities).
8. All operations and maintenance requirements in the permit were complied with or, in the case of a deviation,
prior authorization was received from the Division of Water Quality.
9. No contravention of Ground Water Quality Standards occurred at a monitoring well or explanations of
violations are attached to include appropriate actions and remediations.
10. Vegetative cover as specified in the permit was maintained on this site and the crops grown were removed
in accordance with the crop management plan.
11. No runoff of residuals from the application sites onto adjacent property or nearby surface waters has occurred.
12. All buffer requirements as specified on the permit were maintained during each application of residuals.

CERTIFICATION: This biosolid product has been produced by the Town of Cheraw's Wastewater
Treatment Plant by a treatment process that pasteurized plant residual solids using high temperature
and lime (calcium oxide). The Town of Cheraw certifies that this biosolid complies with the
Environmental Protection Agency regulations 40 CFR Part 503 for a "Class A" product and has a metal
and nutrient content as presented above.
DIRECTIONS FOR USE: This biosolid is intneded to be used as a soil amendment. The biosolid
land application rate shall not exceed the most restrictive Maximum Application Rate as indicated in
the table above. The application rate for nitrogen, phosphorus, potassium, and calcium is dependent
upon the crop grown and site soil characteristics. The user should contact the Clemson Extension
Service (843-623-2134) and/or the Soil Conservation Service (843-623-2187) for assistance in
determining the proper application rate for each crop and site. In no case shall the land application
rate exceed the agronomic rate for the crop grown. User should also refer to the Clemson Extension
Service Circular #476 titled "Lime and Fertilizer Recommendations."
THIS PRODUCT IS HIGHLY ALKALINE AND SHOULD BE HANDLED ACCORDINGLY.
AVOID PROLONGED CONTACT WITH THE SKIN. AVOID CONTACT WITH THE EYES.
THE TOWN OF CHERAW IS NOT RESPONSIBLE FOR IMPROPER USE OR HANDLING OF THIS
BIOSOLID PRODUCT.

Qualifier Meaning
J5 The sample matrix interfered with the ability to make any accurate
determination; spike value is high

T4 (ESC) - Additional method/sample information: QNS - Quantity Not Sufficient
Qualifier Report Information
ESC utilizes sample and result qualifiers as set forth by the EPA Contract Laboratory Program and
as required by most certifying bodies including NELAC. In addition to the EPA qualifiers adopted
by ESC, we have implemented ESC qualifiers to provide more information pertaining to our analytical
results. Each qualifier is designated in the qualifier explanation as either EPA or ESC.
Data qualifiers are intended to provide the ESC client with more detailed information concerning
the potential bias of reported data. Because of the wide range of constituents and variety of
matrices incorporated by most EPA methods,it is common for some compounds to fall outside of
established ranges. 'These exceptions are evaluated and all reported data is valid and useable
"unless qualified as 'R' (Rejected)."
Definitions
Accuracy - The relationship of the observed value of a known sample to the
true value of a known sample. Represented by percent recovery and
relevant to samples such as: control samples, matrix spike recoveries,
surrogate recoveries, etc.
Precision - The agreement between a set of samples or between duplicate samples.
Relates to how close together the results are and is represented by
Relative Percent Differrence.
Surrogate - Organic compounds that are similar in chemical composition, extraction,
and chromotography to analytes of interest. The surrogates are used to
determine the probable response of the group of analytes that are chem-
ically related to the surrogate compound. Surrogates are added to the
sample and carried through all stages of preparation and analyses.
TIC - Tentatively Identified Compound: Compounds detected in samples that are
not target compounds, internal standards, system monitoring compounds,
or surrogates.

INCORPORATED PLANT AVAILABLE NITROGEN ESTIMATE - 80% of ammonium-N, 60% of
,organic-N, and 100% of nitrate-N (if determined). Assumes the manure will be
incorporated into the soil within hours of application. Assumes some loss of
ammonium-N during application and prior to incorporation.
SURFACE PLANT AVAILABLE NITROGEN ESTIMATE - 50% of ammonium-N, 60% of
organic-N, and 100% of nitrate-N (if determined). Assumes the manure will be
left on the surface of the soil with no incorporation by plowing or irrigation.
Available nitrogen calculations are estimates and if nitrate-N was not re-
quested the amount of available nitrogen may be slightly more than reported.
Also, the actual amount may be more or less than the estimate depending on the
composition of the manure, soil type, and environmental conditions.
All of the potash in the animal waste should be plant available in the first
year of application. Although not all of the phosphorous is available in the
first year, its availability should be comparable to that in commercial
fertilizers.
The rate of animal waste to apply for crop production is dependent on the
nutrient content of the waste, method of application and incorporation, soil
test, crop to be grown, and previous manure applications. In most cases, the

CERTIFICATION: This biosolid product has been produced by the Town of Cheraw's Wastewater
Treatment Plant by a treatment process that pasteurized plant residual solids using high temperature
and lime (calcium oxide). The Town of Cheraw certifies that this biosolid complies with the
Environmental Protection Agency regulations 40 CFR Part 503 for a "Class A" product and has a metal
and nutrient content as presented above.
DIRECTIONS FOR USE: This biosolid is intneded to be used as a soil amendment. The biosolid
land application rate shall not exceed the most restrictive Maximum Application Rate as indicated in
the table above. The application rate for nitrogen, phosphorus, potassium, and calcium is dependent
upon the crop grown and site soil characteristics. The user should contact the Clemson Extension
Service (843-623-2134) and/or the Soil Conservation Service (843-623-2187) for assistance in
determining the proper application rate for each crop and site. In no case shall the land application
rate exceed the agronomic rate for the crop grown. User should also refer to the Clemson Extension
Service Circular #476 titled "Lime and Fertilizer Recommendations."
THIS PRODUCT IS HIGHLY ALKALINE AND SHOULD BE HANDLED ACCORDINGLY.
AVOID PROLONGED CONTACT WITH THE SKIN. AVOID CONTACT WITH THE EYES.

INCORPORATED PLANT AVAILABLE NITROGEN ESTIMATE - 80% of ammonium-N, 60% of
organic-N, and 100% of nitrate-N (if determined). Assumes the manure will be
incorporated into the soil within hours of application. Assumes some loss of
ammonium-N during application and prior to incorporation.
SURFACE PLANT AVAILABLE NITROGEN ESTIMATE - 50% of ammonium-N, 60% of
organic-N, and 100% of nitrate-N (if determined). Assumes the manure will be
left on the surface of the soil with no incorporation by plowing or irrigation.
Available nitrogen calculations are estimates and if nitrate-N was not re-
quested the amount of available nitrogen may be slightly more than reported.
Also, the actual amount may be more or less than the estimate depending on the
composition of the manure, soil type, and environmental conditions.
All of the potash in the animal waste should be plant available in the first
year of application. Although not all of the phosphorous is available in the
first year, its availability should be comparable to that in commercial
fertilizers.
The rate of animal waste to apply for crop production is dependent on the
nutrient content of the waste, method of application and incorporation, soil
test, crop to be grown, and previous manure applications. In most cases, the

These are typical values and must be adjusted based upon specific site conditions
DT is Dry Tons of Biosolids. WT is Wet Tons of Biosolids.
CERTIFICATION: This biosolid product has been produced by the Town of Cheraw's Wastewater
Treatment Plant by a treatment process that pasteurized plant residual solids using high temperature
and lime (calcium oxide). The Town of Cheraw certifies that this biosolid complies with the
Environmental Protection Agency regulations 40 CFR Part 503 for a "Class A" product and has a metal
and nutrient content as presented above.
DIRECTIONS FOR USE: This biosolid is intneded to be used as a soil amendment. The biosolid
land application rate shall not exceed the most restrictive Maximum Application Rate as indicated in
the table above. The application rate for nitrogen, phosphorus, potassium, and calcium is dependent
upon the crop grown and site soil characteristics. The user should contact the Clemson Extension
Service (843-623-2134) and/or the Soil Conservation Service (843-623-2187) for assistance in
determining the proper application rate for each crop and site. In no case shall the land application
rate exceed the agronomic rate for the crop grown. User should also refer to the Clemson Extension
Service Circular #476 titled "Lime and Fertilizer Recommendations."
THIS PRODUCT IS HIGHLY ALKALINE AND SHOULD BE HANDLED ACCORDINGLY.
AVOID PROLONGED CONTACT WITH THE SKIN. AVOID CONTACT WITH THE EYES.

INCORPORATED PLANT AVAILABLE NITROGEN ESTIMATE - 80% of ammonium-N, 60% of
organic-N, and 100% of nitrate-N (if determined). Assumes the manure will be
incorporated into the soil within hours of application. Assumes some loss of
ammonium-N during application and prior to incorporation.
SURFACE PLANT AVAILABLE NITROGEN ESTIMATE - 50% of ammonium-N, 60% of
organic-N, and 100% of nitrate-N (if determined). Assumes the manure will be
left on the surface of the soil with no incorporation by plowing or irrigation.
Available nitrogen calculations are estimates and if nitrate-N was not re-
quested the amount of available nitrogen may be slightly more than reported.
Also, the actual amount may be more or less than the estimate depending on the
composition of the manure, soil type, and environmental conditions.
All of the potash in the animal waste should be plant available in the first
year of application. Although not all of the phosphorous is available in the
first year, its availability should be comparable to that in commercial
fertilizers.
The rate of animal waste to apply for crop production is dependent on the
nutrient content of the waste, method of application and incorporation, soil
test, crop to be grown, and previous manure applications. In most cases, the

These are typical values and must be adjusted based upon specific site conditions
DT is Dry Tons of Biosolids. WT is Wet Tons of Biosolids.
CERTIFICATION: This biosolid product has been produced by the Town of Cheraw's Wastewater
Treatment Plant by a treatment process that pasteurized plant residual solids using high temperature
and lime (calcium oxide). The Town of Cheraw certifies that this biosolid complies with the
Environmental Protection Agency regulations 40 CFR Part 503 for a "Class A" product and has a metal
and nutrient content as presented above.
DIRECTIONS FOR USE: This biosolid is intneded to be used as a soil amendment. The biosolid
land application rate shall not exceed the most restrictive Maximum Application Rate as indicated in
the table above. The application rate for nitrogen, phosphorus, potassium, and calcium is dependent
upon the crop grown and site soil characteristics. The user should contact the Clemson Extension
Service (843-623-2134) and/or the Soil Conservation Service (B43-623-2187) for assistance in
determining the proper application rate for each crop and site. In no case shall the land application
rate exceed the agronomic rate for the crop grown. User should also refer to the Clemson Extension
Service Circular #476 titled "Lime and Fertilizer Recommendations."
THIS PRODUCT IS HIGHLY ALKALINE AND SHOULD BE HANDLED ACCORDINGLY.
AVOID PROLONGED CONTACT WITH THE SKIN. AVOID CONTACT WITH THE EYES.

INCORPORATED PLANT AVAILABLE NITROGEN ESTIMATE - 80% of ammonium-N, 60% of
organic-N, and 100% of nitrate-N (if determined). Assumes the manure will be
incorporated into the soil within hours of application. Assumes some loss of
ammonium-N during application and prior to incorporation.
SURFACE PLANT AVAILABLE NITROGEN ESTIMATE - 50% of ammonium-N, 60% of
organic-N, and 100% of nitrate-N (if determined). Assumes the manure will be
left on the surface of the soil with no incorporation by plowing or irrigation.
Available nitrogen calculations are estimates and if nitrate-N was not re-
quested the amount of available nitrogen may be slightly more than reported.
Also, the actual amount may be more or less than the estimate depending on the
composition of the manure, soil type, and environmental conditions.
All of the potash in the animal waste should be plant available in the first
year of application. Although not all of the phosphorous is available in the
first year, its availability should be comparable to that in commercial
fertilizers.
The rate of animal waste to apply for crop production is dependent on the
nutrient content of the waste, method of application and incorporation, soil
test, crop to be grown, and previous manure applications. In most cases, the

CERTIFICATION: This biosolid product has been produced by the Town of Cheraw's Wastewater
Treatment Plant by a treatment process that pasteurized plant residual solids using high temperature
and lime (calcium oxide). The Town of Cheraw certifies that this biosolid complies with the
Environmental Protection Agency regulations 40 CFR Part 503 for a "Class A" product and has a metal
and nutrient content as presented above.
DIRECTIONS FOR USE: This biosolid is intneded to be used as a soil amendment. The biosolid
land application rate shall not exceed the most restrictive Maximum Application Rate as indicated in
the table above. The application rate for nitrogen, phosphorus, potassium, and calcium is dependent
upon the crop grown and site soil characteristics. The user should contact the Clemson Extension
Service (843-623-2134) and/or the Soil Conservation Service (843-623-2187) for assistance in
determining the proper application rate for each crop and site. In no case shall the land application
rate exceed the agronomic rate for the crop grown. User should also refer to the Clemson Extension
Service Circular #476 titled "Lime and Fertilizer Recommendations."
THIS PRODUCT IS HIGHLY ALKALINE AND SHOULD BE HANDLED ACCORDINGLY.
AVOID PROLONGED CONTACT WITH THE SKIN. AVOID CONTACT WITH THE EYES.

DT is Dry Tons of Biosolids. WT is Wet Tons of Biosolids.
CERTIFICATION: This biosolid product has been produced by the Town of Cheraw's Wastewater
Treatment Plant by a treatment process that pasteurized plant residual solids using high temperature
and lime (calcium oxide). The Town of Cheraw certifies that this biosolid complies with the
Environmental Protection Agency regulations 40 CFR Part 503 for a "Class A" product and has a metal
and nutrient content as presented above.
DIRECTIONS FOR USE: This biosolid is intneded to be used as a soil amendment. The biosolid
land application rate shall not exceed the most restrictive Maximum Application Rate as indicated in
the table above. The application rate for nitrogen, phosphorus, potassium, and calcium is dependent
upon the crop grown and site soil characteristics. The user should contact the Clemson Extension
Service (B43-623.2134) and/or the Soil Conservation Service (B43-623-2187) for assistance in
determining the proper application rate for each crop and site. In no case shall the land application
rate exceed the agronomic rate for the crop grown. User should also refer to the Clemson Extension
Service Circular #476 titled "Lime and Fertilizer Recommendations."
THIS PRODUCT IS HIGHLY ALKALINE AND SHOULD BE HANDLED ACCORDINGLY.
AVOID PROLONGED CONTACT WITH THE SKIN. AVOID CONTACT WITH THE EYES.
