On December 16, 2014 the Division of Energy, Mineral and Land Resources received your request to
rescind your coverage under Certificate of Coverage Number NCG 140431. In accordance with your request,
Certificate of Coverage Number NCG]40431 is rescinded effective immediately.
Operating a treatment facility, discharging wastewater or discharging specific types of stormwater to waters
of the'State without valid coverage under an NPDES permit is against federal and state laws and could result
in fines. If something changes and your facility would again require stormwater or wastewater discharge
permit coverage, you should notify this office immediately. We will be happy to assist you in assuring the
proper permit coverage.
If the facility is in the process of being sold, you will be performing a public service if you would inform the
new or prospective owners of their potential need for NPDES permit coverage.

In accordance with your application fora discharge permit received on October 3, 2013, we are
forwarding herewith the subject certificate of coverage (COC) to discharge under the subject state -
NPDES general permit. This permit is issued pursuant to the requirements of North Carolina General
Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the US
Environmental Protection Agency dated October 15, 2007 (or as subsequently amended).
Please note that this COC does not cover discharges from the irrigation of raw material
stockpiles, a wastewater, from this site. This condition is a result of the information presented in your
application.
Please note that permit requirements in Part 11, Section A, numbers 1-5, and Part 11, Section B,
number 1 are no longer valid per NC Session Law 2011-394. Howevcr, note that Part 11, Section A,
number 6, and Part 11, Section 13, numbers 2-10 are unaffected by this Session Law. Those sections will
remain in effect for the duration of this permit term. Please be reminded that your facility must notify tlic
appropriate DWQ. Regional Office at least 72 hours in advance of operation of your wastewater treatment
facility.
This certificate of coverage is not transferable except upon approval of the Division of Energy,
Mineral, and Land Resources (DEMLR). If the facility changes ownership or is closed, DEMLR may
require modification or revocation and reissuance of the certificate of coverage. ']'his permit does not
affect the legal requirements to obtain other permits which may be required by DEMLR, or permits
required by other federal, state, or local authorities.

All applications, reports, or information submitted to the Director shall be signed and certified.
a. All notices of intent to be covered under this General Permit shall be signed as follows:
(1) For a corporation: by a responsible corporate officer. For the purpose of this permit, a responsible corporate officer means: (a) a president,
secretary, treasurer or vice president of the corporation in charge of a principal business function, or any other person who performs similar
policy or decision making functions for the corporation, or (b) the manager of one or more manufacturing production or operating facilities
employing more than 250 persons or having gross annual'sales or expenditures exceeding 25 million (in second quarter 1980 dollars), if
authority to sign documents has been assigned or delegated to the manager in accordance with corporate procedures.
(2) For a partnership or sole proprietorship: by a general partner or the proprietor, respectively; or
(3) For a municipality, state, federal, or other public agency: by either a principal executive officer or ranking elected official.

Roxboro Pugmill Site
S) Description of Industrial Activities
The facility will be a temporary concrete pugmill to be located at a site on Boston road/Highway 501
approximately 3 miles south of the Virginia state line. The Pugmill will provide the concrete for the construction
of an access road at the Duke Energy power plant located on Highway 501 approximately 4 miles north of the
pugmill site.
12) Type of Wastewater
This plant will not discharge wastewater from the site.
No trucks will be rinsed out.
The mixing portion of the pugmill will not be rinsed out.
The aggregate piles will not be wet down.
19) Best Management Practices (BMP)
(1) Sediment fence will be installed along the downslope perimeters of the site. Fencing will be installed,
monitored, and maintained according the practice standards and specifications contained in the North Carolina
Erosion and Sediment Control Planning and Design Manual (section 662).
(2) All disturbed surfaces within the site boundaries will be stabilized with cement -indurated soil. Access roads
will be stabilized with graded aggregate base (GAB).

Stormwater Permitting Unit:
Terra Consulting, LLC is submitting the enclosed Notice of Intent (NOI) for General Permit No.
NCG 140000 on behalf of Morgan Corp for the operation of a concrete pugmill to be located 7376 Boston
Road near Roxboro, North Carolina.
This project will be discharging only storm water from the site.
Paving on this project is scheduled to begin approximately October 15, 2013; therefore we are requesting
an expedited review.
The following items are enclosed:
• Notice of intent form plus one additional page to provide space to adequately answer questions;
• A check for the $100.00 application fee; and
