The Division of Energy, Mineral and Land Resources received your request to rescind your
coverage under Certificate of Coverage Number NCGO80754. In accordance with your request,
Certificate of Coverage Number NCGO80754 is rescinded effective immediately.
Operating a treatment facility, discharging wastewater or discharging specific types of
stormwater to waters of the State without valid coverage under an NPDES permit is against
federal and state laws and could result in fines. If something changes and your facility would
again require stormwater or wastewater discharge permit coverage, you should notify this office
immediately. We will be happy to assist you in assuring the proper permit coverage.
If the facility is in the process of being sold, you will be performing a public service if you would
inform the new or prospective owners of their potential need for NPDES permit coverage.

Hi there —
These facilities were another conundrum for me! The addresses on the request form are too vague to locate and there
is no web presence for these facilities. The facility contact wasn't returning my calls or emails. When he finally did, he
told me he would have to talk to someone else about the addresses. I finally got'some addresses — but I found no signs
for Frito-Lay and they appeared to be addresses for storage units or invisible.
So' ere's what I've got for you:
NCG080732 — at 5624 Hwy 64 West, Murphy— I could not find any such address and there is no industry in this area.
NCG080754 — at 427 Robbinsville Rd, Andrews — this is literally a storage unit facility with no industry. I also tried to go
So the address on the COC but Tatham Gap Rd does not connect to Hwy 129 as the address of record.
NCGO80736 — at 2074 Hwy 69, Locker 7, Hayesville - this is also a storage unit as part of a series of units attached to a
Dr's office with no industry. I also tried to go to the address they gave as on the COC at Rt 2, Box57A. But it seems no
address exists.
Bottom line — these folks need to provide specific addresses and directions to the facility prior to our doing inspections
when the facility is in remote rural areas. I spent a great deal of time trying to track these down, as well as the
Rutherford and Buncombe sites I sent you emails about and we simply don't have the time to try to find these
facilities. They need to provide better upfront addresses, directions and maps prior to our doing inspections.

Hi Bethany,
We assist Frito-Lay with their stormwater permitting. Back in January, Frito-Lay submitted the attached letter to
terminate the industrial stormwater permit for 3 of their sites. While reviewing the list of Active NPDES 5tormwater
Permit list on your website, these 3 permits are still showing as active. Please let me know what is needed to terminate
the permits.
Also, they applied for a permit for their facility listed below recently. I got the new permit number from your website.
Can I get a copy of the coverage letter please? Not sure who would have received it. Last I checked, the facility did not
receive it. Thanks.

subject state — NPDES general permit. This permit is issued pursuant to the requirements of
North Carolina General Statute 143-215 .1 and the Memorandum of Agreement between North
