coverage under Certificate of Coverage Number NCG080732. In accordance with your request,
Certificate of Coverage Number NCGO80732 is rescinded effective immediately.
Operating a treatment facility, discharging wastewater or discharging specific types of
stormwater to waters of the State without valid coverage under an NPDES permit is against
federal and state laws and could result in fines. If something changes and your facility would
again require stormwater or wastewater discharge permit coverage, you should notify this office
immediately. We will be happy to assist you in assuring the proper permit coverage.
If the facility is in the process of being sold, you will be performing a public service if you would
inform the new or prospective owners of their potential need for NPDES permit coverage.

Hi there —
These facilities were another conundrum for me! The addresses on the request form are too vague to locate and there
is no web presence for these facilities. The facility contact wasn't returning my calls or emails. When he finally did, he
told me he would have to talk to someone else about the addresses. I finally got some addresses— but I found no signs
for Frito-Lay and they appeared to be addresses for storage units or invisible.
566T ere's what I've got for you:
NCG080732 — at 5624 Hwy 64 West, Murphy— I could not find any such address and there is no industry in this area.
NCG080754 — at 427 Robbinsville Rd, Andrews — this is literally a storage unit facility with no industry. I also tried to go
to the address on the COC but Tatham Gap Rd does not connect to Hwy 129 as the address of record.
NCG080730 — at 2074 Hwy 69, Locker 7, Hayesville — this is also a storage unit as part of a series of units attached to a
Dr's office with no industry. I also tried to go to the address they gave as on the COC at Rt 2, Box57A. But it seems no
address exists.
Bottom line — these folks need to provide specific addresses and directions to the facility prior to our doing inspections
when the facility is in remote rural areas. I spent a great deal of time trying to track these down, as well as the
Rutherford and Buncombe sites I sent you emails about and we simply don't have the time to try to find these
lities. They need to provide better upfront addresses, directions and maps prior to our doing inspections.

Awhile back I did a Frito Lay site inspection in Murphy and if these are the same, they can be rescinded. I plan to be in
Cherokee and Clay counties before the end of June and I will report back.

Thank you for this follow up information. I understand that these permits are for facilities that are for storage or non -
processing activities. However, there remain discrepancies on the locations. Therefore I am informing our permitting
folks in Raleigh of these discrepancies for their records and files. If needed, I can go back to these facility locations to
confirm, however, it sounds as though these permits should be rescinded. If I need to actually look at these facilities, I
would not be able to do so until November.
Hayesville site:
The address that was previously provided (2074 Hwy 69 Locker ##7) is not on or near Creekside Circle. I did not inspect at
the Creekside Circle location and there is still not a physical address noted for this location. Rt 2, Box 57A is not an
adequate address for field inspections.
Andrews site:
The map provided says the facility is at Tatham Gap Rd and Hwy 129. Tatham Gap Rd is not Robbinsville Rd. I did
inspect a storage unit at 427 Robbinsville Rd. If this is the actual address of the facility, it should be shown as such on
the map provided with a physical address for the location.

Murphy site:
There is still no address provided for the Murphy site. I tried to inspect at the 5624 Hwy 64West address previously
provided but found that there is no such address. The map provided a lat/long that when plugged into google can't be
found in NC. No physical address is shown on the map provided. Therefore, I have not inspected at this location and
would still be unsure of the facility location in NC. A physical address should be provided.
As stated, it sounds to me as though these permits could be rescinded. However, records and files should be clarified
for site locations. If anything further is needed from me, please advise.

I am responding to your email response from Laura Alexander concerning the permit status of three below Frito-Lay
permits. I have attached site maps for the facilities that show their previous locations along with lat/long of their
locations (listed below). In your response you noted that some of these are storage units. This is correct the units are
used to store product for dispersal to the surrounding businesses that sell snack food items. The reason for the permit is
that routine maintenance is conducted on delivery vehicles periodically at the location. Frito has moved out of these
locations approximately a year ago when the Rescission Requests were filed. The site maps will give you the exact
location of where Frito operated at each location. We are wanting confirmation that the Rescission Requests have been
processed for our records and also due to the fact that the NC Industrial Permit is expiring soon so we wanted to clear
up the status before the renewing process begins.

We assist Frito-Lay with their stormwater permitting. Back in January, Frito-Lay submitted the attached letter to
terminate the industrial stormwater permit for 3 of their sites. While reviewing the list of Active NPDES Stormwater
Permit list on your website, these 3 permits are still showing as active. Please let me know what is needed to terminate
the permits.
Also, they applied for a permit for their facility listed below recently. I got the new permit number from your website.
Can I get a copy of the coverage letter please? Not sure who would have received it. Last I checked, the facility did not
receive it. Thanks.

Hi Bethany,
We assist Frito-Lay with their stormwater permitting. Back in January, Frito-Lay submitted the attached letter to
terminate the industrial stormwater permit for 3 of their sites. While reviewing the list of Active NPDES Stormwater
Permit list on your website, these 3 permits are still showing as active. Please let me know what is needed to terminate
the permits.
Also, they applied for a permit for their facility listed below recently. I got the new permit number from your website.
Can I get a copy of the coverage letter please? Not sure who would have received it. Last I checked, the facility did not
receive it. Thanks.
