In accordance with your application for a discharge permit received on March 16, 2018, we are
forwarding herewith the subject certificate of coverage (COC) to discharge under the subject state —
NPDES general permit. This permit is issued pursuant to the requirements of North Carolina General
Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the US
Environmental Protection Agency dated October 15, 2007 (or as subsequently amended).
Please take notice that this certificate of coverage is not transferable except upon approval of the
Division of Energy, Mineral and Land Resources (DEMLR). DEMLR may require modification or
revocation and reissuance of the certificate of coverage. This permit does not affect the legal
requirements to obtain other permits which may be required by DEMLR, the Coastal Area
Management Act, or any other federal, state or local government permit that may be required.
In addition to your application for coverage under NCG130000, we also received your request to
rescind coverage under NCG080000. In accordance with your request, your coverage under COC
NCGO80647 has been rescinded effective immediately.

for the consolidation and beginning of operations is January 1O- 2017. As of January 13t, 2017, the
company will be dissolved as a result of the merger, and Covanta Environmental Solutions, LLC

Dear Mr. Russell:
On December 9, 2013, Sue White of this office performed a Stormwater Permit
Inspection at GARCO, Inc, located on Randleman Road and Carl Drive in Randolph County.
This inspection evaluated five (5) areas as designated on the attached EPA inspection form.
Observations from each area are addressed below:
1. Permit
These facilities hold General Stormwater Permits to discharge Stormwater from activities
associated with Vehicle Maintenance Areas under the National Pollutant Discharge
Elimination System (NPDES). The permits became effective November 1, 2012 and expire
October 31, 2017.
2. Records/Reports
Part 11, Section A of the permit requires the development of a Stormwater Pollution Prevention
Plan (SPPP). Plans were onsite and complete.
3. Facility Site Review
Randleman Road Site: Stormwater reaching secondary containment areas or material handling
and storage areas is routed to the industrial pretreatment system for disposal. This site no
longer does vehicle maintenance and may be required to rescind the current permit and apply
for a NC6130000 for recycling materials and processing waste.
Carl Drive Site: This facility stores some rubber waste and sawdust. The site has a diesel tank
with secondary containment and a double walled tank. No notable concerns associated with
this site.
4. Effluent/Receiving Waters
Stormwater from outfalls at both locations discharge into unnamed tributaries to Haskett Creek,
Class "C" waters in the Cape Fear River Basin.

In accordance with your application for discharge permit received on June 27, 2001, we are
forwarding herewith the subject certificate of coverage to discharge under the subject state - NPDES
general permit. This permit is issued pursuant to the requirements of North Carolina General Statute
143-215 .1 and the Memorandum of Agreement between North Carolina and the US Environmental
Protection agency dated December 6, 1983.
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to request an individual permit by submitting an individual
permit application. Unless such demand is made, this certificate of coverage shall be final and binding.
Please take notice that this certificate of coverage is not transferable except after notice to the
Division of Water Quality. The Division of Water Quality may require modification or revocation and
reissuance of the certificate of coverage.
This permit does not affect the legal requirements to obtain other permits which may be required
by the Division of Water Quality or permits required by the Division of Land Resources, Coastal Area
Management Act or any other Federal or Local governmental, permit that may be required.
