Dear Mr. Ferrell:
On 2/8/2013, the Division of Water Quality received your request to rescind your organization's
stormwater permit coverage under Certificate of Coverage Number NCG020774. In accordance with your
request, Certificate of Coverage Number NCG060035 is rescinded effective immediately.
In your letter of April 26rh, you also requested that your certificate of coverage for NCG060034 be
renewed. NCG060034 was renewed as noted in our letter to Marty Nealey on December 4, 2012.
Unfortunately, the renewal was in a routinely -scheduled renewal process and was not coordinated with
your request to cover plants R 1 and R2 under the same permit. Therefore, I have attached a revised
"Certificate of Coverage" for your facility that makes it clear that plants R1 and R2 are both covered
under the same permit. We appreciate the efforts you have made to cover both facilities under one
Stormwater Pollution Prevention Plan.

In compliance with the provision of North Carolina General Statute 143-215.1, other lawful
standards and regulations promulgated and adopted by the North Carolina Environmental
Management Commission, and the Federal Water Pollution Control Act, as amended,

to receiving waters designated as Beech Branch, a class B; NSW water in the Tar -Pamlico Basin,
in accordance with the effluent limitations, monitoring requirements, and other conditions set
forth in Parts I, 11, 111, IV, V, and VI of General Permit No. NCG060000 as attached.
This certificate of coverage became effective December 4, 2012 but modified to include both
plants in NCG060034 on March 8, 2013.
This Certificate of Coverage shall remain in effect for the duration of the General Permit.

First, thanks for the Hospira document. Hospira is combining their two adjacent plant sites under one stormwater
permit. Please co-ordinate with the issuance of the new permit, when rescinding the RM2 permit. I have visited
the site and understand that it makes a lot more sense for the entire site to be permitted as one. At the time of the
re -issuance of the permit (which would include both plant sites), it is permissible to rescind NCG060035.
(Waste Industr as closed and the fence was locked as of 0.23.12. It is permissible to rescind the permit for
NC _nonA 3. q_

On June 9, 2009, Autumn Hoban and I, Dave Parnell, of the Raleigh Regional Office of the
Division of Water Quality, conducted sampling at several conveyances during a follow up visit
to the January 13, 2009 compliance evaluation inspections (CE1). Thank you to you and the
Hospira staff for your assistance. Observations during the follow up visit, as well as sample
data, are summarized below. Inspection checklists are attached for your records:
Autumn and I concentrated our sampling in the area of the facility known as RM2, which is
the northern most portion of Hospira's Rocky Mount facility. This facility has 8 Stonnwater
Discharge Outfalls (SDO's), all of which discharge to unnamed tributaries to Beech Branch.
Hospira has been granted Representative Outfall Status (ROS) for the SDO's at RM2. DWQ
has approved your facility t6use SDO 017 as the representative outfall for RM2. We re -
sampled at this outfall, since our sampling data fromtheJanuary inspection indicated the
possibility of process flow or groundwater now at the stormwater outfall. Upon re -
inspection and further sampling at SDO 017, it is evident that flow is present in the
stormwater line from a source other than stormwater runoff. The sample result for total
recoverable Aluminum (A1) was 1400 ug/L. This may indicate an exceedance of the aquatic
life water quality action level for Al, which is 87 ug/L, for class C, NSW waters in the Tar-
Pamilico River Basin. The sample result for Copper (Cu) was 11 ug/L. This may indicate an
exceedance of the aquatic life water quality action level for Cu, which is 7 ug/L. The Total
Dissolved Solids (TDS) result was 492 ug/L and would be approaching the 500 ug/L
maximum, when applied to the 2L groundwater standards. The Iron (Fe) results were 6700
ug/L, exceeding the 0.3 mg/L maximum, when applied to the 2L groundwater standards.
The Zinc result was 48 ug/L, just below the aquatic life water quality action level of 50 ug/L.

Autumn and I looked at groundwater standards, because it is possible that the flow in the
stormwater outfall may be groundwater. Tracing lines in this area of the plant and sample
analysis conducted to identify this flow show that it is unlikely that this flow is due to a
process or sanitary sewer leak, so we have concluded that the flow could be groundwater.
Further sampling by the plant of the local ground water may provide the information needed
to determine if this is indeed groundwater.
Please be aware that the results of these sampling events confirm water Quality concerns with
regards to the stormwater discharge at SDO 017. Continue your diligence in attempting to
determine the source of the flow. Sample results have been copied to the NPDES Permitting
Unit for their review during your current permit renewal process. If you have any questions

■ A Stormwater Pollution Prevention Plan Certification Form. This form certifies that you have
developed and implemented the Stormwater Pollution Prevention Plan (SPPP) required in your
permit. This form must be completed and returned to the Division within 30 days of receipt of
this letter. DO NOT send the SPPP with the signed form.
■ Five copies of Analytical Monitoring forms.
■ Five copies of Qualitative Monitoring forms.
■ A copy of a Technical Bulletin on the stormwater program with outlines program components
and addresses frequently asked questions.
■ A corrected Certificate of Coverage if you indicated a name or address change on the Renewal
Form returned to the Division.
Your certificate of coverage is not transferable except after notice to DWQ. The Division of Water
Quality may require modification or revocation and reissuance of the certificate of coverage.
This permit does not affect the legal requirements to obtain other permits which may be required by
DWQ or permits required by the Division of Land Resources, Division of Air Quality, Coastal Area
Management Act or any other Federal or Local governmental permits that may be required.
