On October 23, 2013, the Division of Energy, Mineral and Land Resources received your request to
rescind your coverage under Certificate of Coverage Number NCG030283. In accordance with your
request, Certificate of Coverage Number NCG030283 is rescinded effective immediately.
Operating a treatment facility, discharging wastewater or discharging specific types of stormwater to
waters of the State without valid coverage under an NPDES permit is against federal and state laws and
could result in fines. If something changes and your facility would again require stormwater or
wastewater discharge permit coverage, you should notify this office immediately. We will be happy to
assist you in assuring the proper permit coverage.
If the facility is in the process of being sold, you will be performing a public service if you would inform
the new or prospective owners of their potential need for NPDES permit coverage.

Your facility is currently covcral for stormwater discharge under General Permit NCG030000. This permit expires
on August 3I, 2002. The Division staff is currently in the process of rewriting this pernlil and is scheduled to have
the pernlit reissued by late summer ol'2002. Once the permit is reissued, your facility would he eligible for
continued coverage under the reissued permit.
In order to assure your continued coverage under the general permit, you rtnust apply to the Division of Water
Quality (DWQ) for renewal of your permit coverage. 'I'o make this renewal process easier, we are informing you in
advance that your pernlit will be expiring. Enclosed you will find a General Permit Coverage Renewal
Application Form. 'I -he application must he contpicted and returned by March 4, 2002 in order to assure continual
coverage under the general permit.
Failure to request renewal within this tinge period play result in a civil assessi nenl ol' at least $250,00. Larger
penalties relay he assessed depending on the delinquency ol' the request. Discharge of storrrlwater from your facility
without cnvcragc under a valid stormwater NPDI S permit would constitute a violation of NCGS 143-215.1 and
could result in assessments of civil penalties of up to $10'.000 Per day.
Please note that recent Icderal legislation has extended the "no exposure exclusion" to all operators of industrial
Facilities in any of the I I categories of "storm water discharges associated Willi industrial activity," (except
construction activities). If you feel your facility can certify a condition of "no exposure", i.e. the facilty industrial
materials and operations are not exposed to stormwater, you can apply for the no exposure exclusion. For additional

Perfecting Coupling Company located at 332 Atando Avenue in Charlotte, North Carolina is
petitioning to establish.that'Sampling Point 1 puts employees in a potentially dangerous
situations and that Sampling Point.2 can be considered representative of the stormwater from the
Perfecting Coupling property.
Perfecting coupling has one (1) stormwater outfall. Stormwater at Outfall SW001 is
commingled with offsite run-off from several offsite sources. Consequently, the facility has
previously collected samples from two (2) designated sample locations (No. 1 and No. 2) each
upstream of the outfall. Sampling Point 1 is located inside the shop building at a junction
manhole or vault (see attached Stormwater Site Map). At the junction, five stormwater lines
join. Three (3) of these lines come from off -site catch basins. It is therefore necessary for an
employee to crawl into the approximately 15 foot deep vault to sample the line that contains only
stormwater from the Perfecting Coupling property. It is an old stormwater system and there are
no real safety precautions that can be put in place to truly protect the employee. The vault is
located in a work area and there is also a danger of falling in the vault when open.

Part III Section D.1 of Stormwater General Permit NCG030000 allows for representative
sampling if the sample collected is characteristic of the volume and nature of the discharge.
Sample Point 2 is located at a ditch near the recycling center. This location has the highest
potential for stormwater contamination from raw materials and scrap metals. Data collected
from sampling dates April 26, 1996 and November 21, 1997 are attached. Perfecting Coupling
Company believes that both the analytical and visual sampling of Sample Point 2 would be
representative of the stormwater run-off from the Perfecting Coupling site.

Form returned to the Division.
Your certificate of coverage is not transferable except after notice to DWQ. The Division of Water
Quality may require modification or revocation and reissuance of the certificate of coverage.
This permit does not affect the legal requirements to obtain other permits which may be required by
DWQ or permits required by the Division of Land Resources, Division of Air Quality, Coastal Area
