2014, we are forwarding herewith the subject certificate of coverage (COC) to discharge under
the subject state — NPDES general permit. This permit is issued pursuant to the requirements of
North Carolina General Statute 143-215.1 and the Memorandum of Agreement between North
Carolina and the US Environmental Protection Agency dated October 15, 2007 (or as
subsequently amended).
Please take notice that this certificate of coverage is not transferable except upon
approval of the Division of Energy, Mineral, and Land Resources. The Division of Energy,
Mineral, and Land Resources may require modification or revocation and reissuance of the
certificate of coverage. This permit does not affect the legal requirements to obtain other
permits which may be required by the Division of Energy, Mineral, and Land Resources, the
Coastal Area Management Act, or any other federal or local government permit that may be
required.
Per the requirements of the Neuse Riparian Buffer Rule, all stormwater drainage to
stream buffers, from portions of this site that have been constructed after July 22, 1997, must be
discharged through a correctly designed level spreader or another device that meets diffuse flow
requirements per 15A NCAC 2B .0233. Diffuse flow requirements are described in Chapter 8
of the North Carolina Stormwater BMP Manual, available at:

Mid -Atlantic Associates, Inc. (Mid -Atlantic), on behalf of Wesbell Investment Recovery, appreciates
the opportunity to submit the attached Notice of Intent for the facility located at 555 Lucy Garrett
Road in Roxboro, North Carolina. The site consists of two industrial activity drainage areas, as
shown on the attached Site Map. Stormwater at Drainage Area No. 1 either infiltrates the site soils
or enters an on -site drainage ditch prior to discharging off -site at Outfall No. 1 and ultimately to an
intermittent, unnamed tributary to the North Flat River. Additionally, stormwater from the industrial
portion of Drainage Area No. 2 enters an on -site network of drainage ditches, which directs flow
toward the wooded and undeveloped area along the northern portion of the site. Stormwater runoff
from this portion of the site ultimately leaves as sheetflow.
A Stormwater Pollution and Prevention Plan (SPPP) has been developed for this facility and was
implemented at the site in July 2014. This Notice of Intent was prepared prior to implementation,
but was inadvertently not submitted to the Division of Water Resources upon completion. Please
review the attached documentation and, if you have any questions or need additional information,

Mid -Atlantic Associates, Inc. (Mid -Atlantic), on behalf of Wesbell Investment Recovery, appreciates
the opportunity to submit the attached Notice of Intent for the facility located at 555 Lucy Garrett
Road in Roxboro, North Carolina. The site consists of two industrial activity drainage areas, as
shown on the attached Site Map. Stormwater at Drainage Area No. 1 either infiltrates the site soils
or enters an on -site drainage ditch prior to discharging off -site at Outfall No. 1 and ultimately to an
intermittent, unnamed tributary to the North Flat River. Additionally, stormwater from the industrial
portion of Drainage Area No. 2 enters an on -site network of drainage ditches, which directs flow
toward the wooded and undeveloped area along the northern portion of the site. Stormwater runoff
from this portion of the site ultimately leaves as sheetflow.
A Stormwater Pollution and Prevention Plan (SPPP) has been developed for this facility and was
implemented at the site in July 2014. This Notice of Intent was prepared prior to implementation,
but was inadvertently not submitted to the Division of Water Resources upon completion. Please
review the attached documentation and, if you have any questions or need additional information,
