Thanks for renewing your permit using our online option. No further action is necessary. The new General
Permit is available for printing from our website at http://portal.ncdenr.org/web/lr/stormwater .

Certificate of Coverage (COC), you can resubmit the data and print another copy.
This COC is issued pursuant to the requirements of North Carolina General Statute 143-215.1 and the
memorandum of Agreement between the state of North Carolina and the U.S. Environmental Protection
Agency, dated October 15, 2007 (or as subsequently amended.)
The General Permit authorizes discharges of stormwater and specifies your obligations for discharge controls,
management, monitoring, reporting, and record keeping. Please review the new permit to familiarize yourself
with all of the changes. Parts III and IV contain the Standard Conditions, including Compliance and Liability,
Reporting, Monitoring and Records requirements; Operation and Maintenance obligations; and Definitions.
Your facility has six months from receipt of the COC to update its Stormwater Pollution Prevention Plan
(SPPP) to comply with changes in SPPP requirements. Other changes are effective immediately. Please note
that Tier 3 Actions (if applicable) are triggered by four benchmark exceedances beginning on the effective
date of the renewal permit and do not count prior exceedances. Please visit our website above to review the
new General Permit carefully.
Your coverage under the General Permit is transferable only through the specific action of DEMLR. This
permit does not affect the legal requirements to obtain other permits which. may be required by DENR, nor does
it relieve the permittee from responsibility for compliance with any other applicable federal, state, or local law,
rule, standard, ordinance, order, judgment, or decree.

On September 8, 2010, Mark Brantley, Environmental Chemist, from the Fayetteville Regional Office
of the Division of Water Quality, conducted a site inspection at the KORDSA facility located 1.7780 Armstrong
Road, Laurel Hill, in Scotland County, North Carolina. A copy of the Compliance Inspection Report is enclosed
for your review. Mr. Richard Massey was also present during the inspection and his time and assistance is
greatly appreciated. Stormwater from this facility drains to a Ut of Gum Swamp Creek, a Class C: SW creek
located in the Lumber River Basin. The site visit and file review revealed that the subject facility is covered by
NPDES Stormwater General Permit-NCG 1.70000.
Accordingly, the following observations were noted during the Division of Water Quality inspection:

Reference is made to your recent request for rescission of coverage under the subject NPDES General
Permit. in your letter received on 10/20/2006, you stated that you are no longer operating a production
facility at this site. In accordance with your request, NPDES General Permit NCG 170365 is rescinded,
effective immediately.
Operating a treatment facility, discharging wastewater or discharging specific types of stormwater to
waters of the State without a valid NPDES permit will subject the responsible party to a civil penalty of
up to $25,000 per day. Enforcement action will be certain for persons that have voluntarily relinquished
permit coverage when, in fact, continuing permit coverage was necessary. If, in retrospect, you feel the
site still requires permit coverage, you should notify this office immediately. Furthermore, if in the future
you wish to again discharge to the State's surface waters, you must first apply for and receive a new
NPDES permit.
If the facility is in the process of being sold, you will be performing a public service if you would inform
the new or prospective owners of their potential need for NPDES permit coverage.

Site visit to this facility on 12/6. Met with Mr. Dan Pelton, Plant Manager, Mr.
Richard Massey, IS/EHS Manager, Mr. Raj Mahadevaiah, consulting engineer, and a few.
others who's names I don't recall! Contrary to their NOI, which identified 4
outfalls, the facility technically has 2. One near the back left of the facility and
one in the front. One of the other 2 they identified to'be taken away consists of a
small drainage: ditch that runs between their property and an agricultural field,
which I feel would not be representative runoff from their operations and would
undoubtedly receive some solids from the field when not planted. The second
'outfall' to be taken away is at the very back of the facility and is nothing more
than a low spot in the ground, which holds a small amount of water that would most
likely be absorbed or evaporate. I relayed to the gentlemen that they should at
least keep an eye on these other 2 areas, but would not need to document semi-annual
visual inspections. Facility does not perform any on site vehicle maintenance and
therefore are only required to do the twice per year visual monitoring. Their
consulting engineer said that they were approximately 95% complete with an SPPP.
There are a few barrels stored outside that are labeled and within a secondary
containment area with an overhead shelter. There are also some pallets, dumpsters
and empty barrels outside in areas with appropriate signage. As with the others, the
Fayetteville Regional Office recommends issuing coverage under the NCG170000 general
permit for this facility.'

Re: Initial Notification under I-�PA's Audit Policy,
KORDSA, Inc., 17780 Armstrong Road, I.,aurel Hill, NC 28351,
Dear Ms. Sisario:
Pursuant to EPA's Audit Policy, please accept this letter as an initial notification of KORDSA Inc's
voluntary implementation of an environmental audit at its facility located at 17780 Armstrong Road,
Laurel Hill, NC 28351. To accomplish this objective, KORDSA retained Environmental
International Corporation (EIC) based in Alpharetta, Georgia.
On September 25, 2006, a team comprised of personnel from both 13IC and KORDSA initiated the
audit process. Mr. Raj Mahadevalah of E;IC was designated as the lead auditor. I have assumed the
role as a primary point of contact for KORDSA. Other personnel from KORDSA assisting me in
the audit process were the technical and quality manager, plant engineer, Information
Systems/Environmental Hcalth(IS/I-*H) manager, and maintenance personnel.
On October 3, 2006, the team discovered that KORDSA failed to file an I_.PCRA 'Pier II report for
the 2005 reporting year due to personnel reassignments. On October 5, 2006, the team further
discovered that KORDSA had failed to obtain a General Stormwater Discharge Permit for
industrial activities under the ""Textile Mill Products" category pertaining to the National Pollutant
Discharge Elimination System (N13DES). This infraction resulted from KORDSA's erroneous
interpretation that other NPDES permits associated with construction activities and wastewater
discharge issued by the North Carolina Department of Environment and Natural Resources
(NCDIINR) satisfied the regulatory requirements.
KORDSA intends to rectify the f-'PCRr1 and NPDLS infractions on or before December. 3rJ, 2006.
Since, KORDSA has voluntarily disclosed the violation within the 21-day notification period, we
request waiver of all penalties under the EPA Audit policy.
Since the audit is currently in progress to verify compliance with other regulations, KORDSA is
uncertain if additional notifications are necessary. Upon discovery of any other violations,
KORDSA will promptly notify E`PA and utitiate corrective action(s) as necessary.
