This permit is reissued pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between the state of North Carolina and the U.S. Environmental Protection Agency, dated October .l5, 2007 (or as subsequently amended.) A new Certificate of Coverage (COC) is included with this letter. You must print a copy of the new NCG140000 General Permit from our website here: http.//deq.nc.goy/about/divisionsI- _... _.... permits /stormwater- e=-_mits.Jnpdes-industrial-sw. In addition to the full permit, the 2017 print package on the website includes revised Discharge Monitoring Report (DMR) forms, Annual Discharge Monitoring Report forms, and Qualitative Monitoring Report Forms. The General Permit authorizes discharges of stormwater and wastewater (if applicable), and it specifies your obligations with respect to discharge controls, management, monitoring, and record keeping. Please review the new permit to familiarize yourself with all changes in the reissued permit. Significant changes to the General Permit are outlined in the Technical Bulletin, which is also available on the website above. The first sample period of your permit begins August 1, 2017. Tier 3 actions in Part IV of your permit are triggered by benchmark exceedances on four occasions beginning on the effective date of this permit and do not count prior exceedances. Likewise, Tier 2 actions are triggered by two consecutive benchmark exceedances beginning on the effective date of this permit and do not count prior exceedances. However if your facility is already in Tier 2 (monitoring stormwater discharges monthly) or Tier 3, you must continue Monthly monitoring until relieved through conditions of the permit or by DEMLR staff approval. When does electronic DMR reporting start? We are setting up our database with final permit parameters and the outfalls that permittees submitted on-line. All NCG14 Permittees will receive notification when our eDMR system is ready for these permittees to register and begin reporting monitoring data electronically. Hildreth Ready Mix is approximately 2 miles on the right. I£ no one is at the plant, take Hildreth Road, which is just past plant, up to the office on the right. II, SAFETY: Standard PRO safety gear is required. The inspector should be mindful of vehicular and heavy equipment traffic. III. FACILITY/PROCESS DESCRIPTION: Hildreth Ready Mix is a small concrete batch plant. Cement is stored in silos and mixed with aggregate and sand (both stockpiled on site), all of which is mixed with water inside a cement truck. The loadout and silos are controlled by a central dust collector. A second, unused bagfilter, which was previously used as a dedicated control for one of the silos is still in place, but. it no longer operating. To produce the septic tanks, cement is poured into 1,000-gallon, 1,200-gallon, or 1,500-gallon molds and allowed to cure for 28-30 days. The molds are removed and septic tanks are delivered and installed on site. Although septic tanks Are still produced on site, most of Mr. Hildreth's business now is delivering concrete from his batch plant, which is operated on an "as needed" basis. The facility requested to have the air quality permit rescinded on 01 July 2016 under the newly adopted 2Q .0102(d) exemption. The facility's permit was rescinded on 19 July 201 b. The rescission request was an apparent error, and the facility subsequently requested to have the permit reinstated. The facility's air permit was reinstated on 13 September 2016. INSPECTION SUMMARY: On 18 Apri12017, Mike Lawyer, FRO DF,MI,R, and I, Joshua Harris FRO DAQ, arrived on site to conduct a multimedia inspection of Hildreth Ready Mix, LLC. While. entering the property, we briefly observed 4 silo being loaded, and noted intermittent visible emissions around *e. connections at ^-5% opacity. These emissions dissipated before reaching the property boundary, and no other visible emissions were noted, and the facility was otherwise not operating during the inspection. At the Admin Office, we met Karl liildreth, Owner, and Chanel Little, Secretary. Mr. Hildreth verified the FacFindet information, and requested that Ms. Little be listed as the invoice contact. We reviewed the facility's records, which, while messy, appeared to be complete. Mr. Hildreth described his methods for maintaining the bagfilter, which appears -to be sufficient. Mr. Hildreth stated that he has not had to change any bags in nearly a decade, and no visible emissions concerns were noted during previous inspections in which the facility was observed operating. V. PERMITTED EMISSION SOURCES: A. One cement mixing weigh hopper and loading operation, and Not operating H. Silos for cement and Elyash storage. Cement loading briefly observed with 0% opacity at bagJilter exhaust Light visible emissions, —5% opacity, were noted at the connections. No fugitive dust was visible past the property boundary. APPLICABLE AIR QUALITY REGULATIONS: A. 15A NCAC 2D .0202 — PERMIT RENEWAL AND EMISSION INVENTORY REQUIREMENT - The permittee shall submit an application for renewal of the facility's permit no later than; 90 days prior to expiration, and shall include documentation of air pollutants emitted for the 2017 calendar year. APPEARED INQOMPLIANCE: We reminded Mr. Hildreth of this requiremen4 and reminded hint that his permit will be expiring next year on 30 June 2018. Previously, the facility's emission inventories, and applications for renewal have been received on time. The facility will be deemed in non-compliance # the emission inventory and renewal application are not received by 01 April 2018 B. 15A NCAC 2D .0515 —PARTICULATES FROM MISC. INDUSTRIAL PROCESSES — Particulate emissions shall not exceed allowable emission rates as calculated by the. foliowitig equations: E = 4.10 * (P) ° 6'. for P :5 30,tonslhr, or E = 55 * (P) ° 11- 40 for P >30 tons/hr 9AMARBD IN COMPLIANCE; These calculations were made when the facility was permitted, and the previous permit reviews indicated that that facility, when controls are properly maintained and operated -would not exceed these limits. The facility's particulate emissions are controlled by bagJllters. Each bagfilter appeared to have regular maintenance conducted, and there were no dust accumulations near their exhaust points which would indicate an operational problem. C. 15A NCAC 2D .0521— CONTROL OF VISIBLE EMISSIONS— Visible omissions shall not exceed 20% opacity. APPEARED 12V COMPLIANCE_ The facility was not operating at the time of the, inspection, though we did observe a silo load as we entered, during which light visible emissions, �-S% opacity, were noted around the connections. Mr. Hildreth stated that he does not see visible emissions while the plant is operating, outside of occasional fugitive emissions at loadout, and that he hasn't received any recent dust complaints.- DAQ has not received any recent complaints pertaining to this facility. D. 15A NCAC 2D .0535 — NOTIFICATION REQUIREMENT— Notify, DAQ in the event of excess emissions lasting longer than 4 hours resulting from malfunctions, breakdowns, or abnormal conditions. ,APPEARED &V COMPLIANCL- There are no indications of any excess emissions which would require a not (cation by the facility. We reminded Mr. Hildreth of the notification requirement, There have been no recent complaints received by the facility or DAQ. E. 15A NCAC 2D .0540 —PARTICULATES FROM FUGITNE DUST EMISSION SOURCES — liugitive dust emissions shall not contribute to substantive complaints, or excessive dust emissions across the property boundary. APPEARED IN COMPLIANCE: No fugitive dust concerns at the property boundaries were noted during the inspection, though the facility is located just off the street. The facility was not operating at the time of the inspection, but we were able to briefly observe a silo load. Brief, light visible emissions were noted at the connections, and no dust was visible past the property boundary. There have been dust complaints in the past associated with the truck loadout, but none have been received by DAQ in the past two years. F. 15A NCAC 2D .0511— FABRIC FILTER REQUIREMENT —Particulate emissions controlled by fabric filters; actnual internal inspections; record keeping. APPEARED IN COMPLIANCE: Mr. Hildreth performs the maintenance himself, and described his method for cleaning and inspecting the bagfilter. Mr. Hildreth stated that he removes the bags, inspects them for damage, rinses them; and lets them dry before reinstalling them. He uses a pump truck to pump out any collected cement, then he sprays the slurry onto his lawn, which he claims helps the grass grow. Mr. Hildreth's production log has an entry on 28 January 2017, which states simply that the baelter was cleaned. G. 15A NCAC 2D .1104 —TOXIC AIR POLLUTANT CONTROL REQUIREMENTS —Arsenic emissions shall not exceed the AAL. Concrete production limit based on distance to property boundary (35,000 cubic yards at 75 feat), which shall be marked. Monthly recordkceping, Notification of changes. Annual reporting. . APPEARED IN COMPLIANCE: The nearest property boundary is marked and has been verified at —75 feet which limits the facility to 35, 000 cubic yards of production annually. Mr. Hildreth keeps a running log of concrete produced each day, and there is a spreadsheet available which shows the monthly and annual totals; the facility produced 4,147 cubic yards in 2016 A. IBldreth stated that there have been -no.-changes to the facility, and we reminded him of the notiflcation.requirement. The annual report was received late, on 13 March 2017, and a Notice of Violation was issued on 21 March 2017. DIRECTIONS TO SITE: In Wadesboro, take Hwy 742 south for-3.5 miles to City Pond Road and turn right. Hildreth Ready Mix is approximately 2 miles on the right. If no one is at the plant, take Hildreth Road, which is just past plant, up to the office on the right. II. SAFETY: Standard FRO safety gear is required. The inspector should be mindful of vehicular and heavy equipment traffic. III. FACILITY/PROCESS DESCRIPTION: Hildreth Ready Mix is a small concrete batch plant. Cement is stored in a silos and mixed with aggregate and sand (both stockpiled on site), all of which is mixed with water inside a cement truck. The loadout, and two of the three silos are controlled by a central dust collector, one of these two silos has its own baghfilter in series with the central dust collector; the third silo is controlled by a single, dedicated bagfilter. To produce the septic tanks, cement is poured into 1,000-gallon,1,200-gallon, or 1,500-gallon molds and allowed to cure for 28-30 days. The molds are removed and septic tanks are delivered and installed on site. Although septic tanks are still produced on site, most of Mr. Flildreth's business now is delivering concrete from his batch plant, which is operated on an "as needed" basis. The facility requested to have the air quality permit rescinded on 01 July 2016 under the newly adopted 2Q .0102(d) exemption. The facility's permit was rescinded on 19 July 2016. The rescission request was granted based on calculations which assume the facility's emission sources are controlled by bagfilters which are properly maintained and operated. INSPECTION SUMMARY: On 08 September 2016, Mike Lawyer, FRO DEMLP, and I, Joshua Harris FRO DAQ, arrived on site to conduct a multimedia inspection of Hildreth Ready Mix, LLC. We met Karl Hildreth, Owner, and Chanel Little, Secretary, in the facility's office. Mr. Hildreth verified the FacFinder information, and noted no changes. We began discussing the current process DAQ is using regarding inspections for facilities which have rescinded their air permits, and Mr. Hildreth stated that he did not want to rescind his permit. I informed him that the permit was already rescinded, and that a letter requesting rescission was received. We were able to locate copies of the original request documents, for registration and rescission, that were submitted to DAQ. Mr. Hildreth reiterated that he did not want to rescind the permit, and would like to have the permit reissued. After the rescission discussion, we attempted to locate the facility's production records and the maintenance records for the bagfilters. Ms. Little was able to provide the requested production records, but W. Hildreth stated that he does not keep records of maintenance conducted on the bag houses. He stated that every two months, be removes the bags, washes them, cleans out the bag house, and reinstalls the bags. I reminded Mr. Hildreth that when his permit is reinstated, he will be required to keep records for inspections and maintenance performed on the facility's bagfilters. Mr. Hildreth stated that he understood and would start keeping those records. We then toured the batch plant. 16 The facility was not operating at the time, but Mr. Hildreth pointed out how each source is controlled, and pointed out each emission point. We asked if there are any visible dust emissions while the plant is operating, and Mr. Hildreth stated that some dust will come out of the enclosure during loadout, but that there is never any dust visible at the baghouse exhausts. ' After touring other parts of the property for the stormwater inspection, we departed. V. EQUIPMENT ON SITE: A. One weigh hopper and loadout operation; B. Three silos for cement storage; and C. Sand and Aggregate stockpiles and handling equipment. VI. APPLICABLE AIR QUALITY REGULATIONS: A. 15A NCAC 2D .0515 — PARTICULATES FROM MISC. INDUSTRIAL PROCESSES — Particulate emissions shall not exceed allowable emission rates as calculated by the following equations: E = 4.10 * (P) "7 for P c 30 tons/hr, or E = 55 * (P)'-" - 40 for P >30 tons/hr APPEARED INCOMPLIANCE: These calculations were made when the facility was permitted, and the previous permit reviews indicated that that facility, when controls are properly maintained and operated, would not exceed these limits. The facility's particulate emissions are controlled by hagf lters. Each bagfilter appeared to have regular maintenance conducted, and there were no dust accumulations near their exhaust points which would indicate an operational problem. B. 15A NCAC 2D .0521— CONTROL OF VISIBLE EMISSIONS — Visible emissions shall not exceed 20% opacity. APPEARED INCOMPLIANCE: The facility was not operating at the time of the inspection. Mr. Hildreth stated that he does not see visible emissions while the plant is operating, outside of occasional fugitive emissions at loadout and that he hasn't received any recent dust complaints. DAQ has not received any recent complaints pertaining to this facility. C. 15A NCAC 2D .0535 —NOTIFICATION REQUIREMENT —Notify DAQ in the event of excess emissions lasting longer than 4 hours resulting from malfunctions, breakdowns, or abnormal conditions. APPEARED IN COMPLL4NCE: There are no indications of any excess emissions which would require a notification by the facility. Mr. Hildreth eagerly pointed out that there are no dust accumulations near the baghouse exhausts. There have been no recent complaints received by the facility or DAQ. D. 15A NCAC 2D .0540 —PARTICULATES FROM FUGITIVE DUST EMISSION SOURCES — Fugitive dust emissions shall not contribute to substantive complaints, or excessive dust emissions across the property boundary. APPEARED I1V COMPLIANCE: No fugitive dust concerns at the property boundaries were noted during the inspection, though the facility is located just off of the street. The facility was not operating at the time of the inspection, and we did not observe a truck loadout. There have been dust complaints in the past associated with the truck loadout, but none have been received by DAQ in the past year. Mr. Hildreth stated that he has received past dust complaints which prompted him to upgrade his dust collection system. The system was observed in operation during the previous inspection, and visible emissions at the loadout were noted to be —10% opacity at that time. VII. NON-COMPLIANCE HISTORY SINCE 2010: 03/28/16 NOD issued for late annual reporting. 05/28/10 NOV issued for late annual reporting. VIII. RISK MANAGEMENT (112r): This facility does not use or store any 112(r) subject materials above threshold limits. Therefore, it is not required to maintain a written Risk Management Plan (RMP). IX. Design Calculations and Basis for Recycle System: The application included demonstration that the recycle basin is large enough to contain the 25-year, 24-hour rain event, as well as room for solids accumulation, freeboard, and assumptions about truck wash inputs. However, there is no water balance provided for the recycle system. How much water will be used in the concrete mixers on a daily basis? The plans note that the wet well pump will activate when water is 0.5 ft deep, but that does not demonstrate that the 6000-gal tank shown on the plans is sufficient to contain any accumulated water after several days' time. A water balance should show all Inputs - Outputs = No Accumulation. (In other words, please provide assumptions regarding what will be drawn out on a daily basis to show the system can maintain freeboard). It would be advisable to address how rainwater that enters the system in a large event could be used in the process (or properly disposed of) within a reasonable time frame so that capacity is restored before subsequent rains. 3. Site Plans: A more detailed view of the recycle system itself would be helpful. The plans do note a bottom elevation of 460', but spot elevations at 465' at the rim of the system are different from the 463.85' noted in the calculations (or is this a demonstration that the walls are tall enough?). It is also unclear whether this system is a sloped, earthen pit or constructed concrete basin with vertical walls. The plans indicate where grab samples are to be taken at two locations. However, a stormwater or wastewater discharge point is not noted at the end of the `Prop. Grass Line Swale' in the center of the property. This swale appears to drain any stormwater that would fall on aggregate piles. According to the application, the aggregate piles are not irrigated. [However, please remember that should water stored in the tank from the recycle system ever be used to spray those aggregate piles down, any discharges coming from those piles would be wastewater and will have to be treated to meet effluent limits. We note in this submittal that you are not proposing any pH adjustment, which would be unacceptable if the system were not a non -discharging, closed -loop recycle system.] Please indicate whether this is a stormwater discharge point and ensure that the application accurately reflects any plans to irrigate piles, even if very infrequent.
