On behalf of Concrete Service Company, Inc., this letter serves as notification that no discharge
occurred from the above -referenced facility's Potential Outfall 001 over the previous calendar year
(January -December 2017). Stormwater runoff at the site is either retained on the site, recycled in
facility operations or leaves the site as sheetfiow and migrates onto the eastern adjacent FMI
Block Plant facility. In the event of a point -source discharge from the site, the runoff would flow
over a cinderblock retaining wall on the northern portion of the site and discharge off -site at
Potential Outfall 001.
To date, there have been no point -source discharges of stormwater from the site. Stormwater
flow will continue to be monitored during the current calendar year (January -December 2018)
and, in the event of a point -source discharge, stormwater samples will be collected per the

from the on -site detention pond, the runoff would flow over a weir and discharge off -site at

pond. Stormwater flow will continue to be monitored during the current calendar year (January -
December 2018) and, in the event of a discharge, stormwater samples will be collected per the

On behalf of Concrete Service Company, Inc., attached please find the Annual Summary
Discharge Monitoring Report (DMR) and supporting analytical results for the sampling
conducted over the previous calendar year (January -December 2017). A stormwater
sample was collected from Outfall 001 on February 15, 2017 and analyzed for pH and
Total Suspended Solids (TSS). As indicated on the attached DMR, all parameters were
reported below their benchmark values or within their benchmark range.
Please note that a stormwater sample was not collected during the final monitoring period
(July -December) of 2017. During this timeframe, a measurable storm event did not occur
at a time when a stormwater sample could be safely collected (i.e. during business hours
when the facility was occupied, and preceded by 72 hours with no measurable storm
