The first sample period of your permit begins August 1, 2017. Tier 3 actions in Part IV of your permit are triggered by benchmark exceedances on four occasions beginning on the effective date of this permit and do not count prior exceedances. Likewise, Tier 2 actions are triggered by two consecutive benchmark exceedances beginning on the effective date of this permit and do not count prior exceedances. However, if your facility is already in Tier 2 (monitoring Stormwater discharges monthly) or Tier 3, you must continue monthly monitoring until relieved through conditions of the permit or by DEMLR staff approval. When does electronic DMR reporting start? We are setting up our database with final permit parameters and the outfalls that permittees submitted on-line. All NCG14 Permittees will receive notification when our eDMR system is ready for these permittees to register and begin reporting monitoring data electronically. Be aware of equipment moving around facility grounds, as well as overhead conveyors. DMCTIONS: From FRO travel on I-95 south about 35 miles to Lumberton; take exit 20 for Roberts Avenue/Hwy 211 and turn left at the stoplight; travel for about 3 miles; facility is on the left. FACILITY DESCRIPTION: Concrete is composed essentially of water, cement, sand (fine aggregate), and coarse aggregate, Flyash is added as a specialty aggregate. The plant stores, conveys, measures and discharge these constituents into trucks for mixing and transport to the job. Raw materials are delivered to the facility via truck.. The cement and flyash are transferred to the elevated silos by pneumatics. The aggregates are dropped into a storage bin with a front-end loader and then belt conveyor to the weigh hopper. This is fed into the truck and then proper amounts of each water, cement, and flyash are added to the truck. The concrete is mixed on the way to the site where it is poured. The facility has a separate weigh hopper for weighing sand and aggregate. Emission sources are the dumping of the mix into the truck, the transfer of flyash and cement into the silos, the auger transfer of flyash from the silo to the hopper, and the transfer of the aggregate to the bin. The facility has a central pulse jet bagfilter, with magnehelic gage, that controls emissions from the permitted silos, weigh hopper, and shrouded truck mix load -out. The facility has an anti -overfill system with bin -indicator lights for both the cement and flyash silos. The facility has a kerosene -fired process hot water heater that can be used if needed, to heat water for the mix. Currently, the facility has 7 employees, and operates 7:00 am-5:00 pm, Monday through Friday. INSPECTION SUMMARY: On 19 August 2015,1, Mark Brantley, of Division of Water Resource -Fayetteville Regional Office, along with Josh Harris and Drew Hammonds, of FRO DEQ, met with Mr. Hawk Gay, Environmental Director, for a multimedia inspection. Mr. Gay confirmed the accuracy of the contacts currently listed for the facility, and provided a copy of the current permit. He stated that there had been no changes made at the facility since the last inspection. He stated that no recent complaints had been received from anyone concerning air quality. During"discussions with Mr. Gay and Mr. Robbie Nobles, Plant Manager, it was confirmed that the hot water heater had been operated 5 or 6 times during the winter of 2016. There were no written records as required by NESHAP 6J requirements. Since this was a multimedia inspection this deficiency was referred to the parent agency, NC Division of Air Quality, for follow up. I did not observe any new equipment or process that would require further permitting at this time. I did not observe any waste concrete crushers on the site that day. On the day of the inspection the plant was operating, but no visible emissions were noted. REGULATORY REVIEW: A.2 "EMISSION INVENTORY_REOUIREMENT" — Requires complete permit renewal with AQEI for CY 2021submittal at least 90 days prior to permit expiration. APPEARED IN COMPLIANCE — Last year's permit renewal, including CY2013 AQEI, was submitted to DAQ on time. The current permit expires on 11/30/2022. A.3 2D .0515, "PARTICULATE CONTROL REQUIREMENT" — Particulate emissions shall not exceed allowable emission rates. APPEARED IN COMPLIANCE — Compliance was determined by the permit writer, based on operating these sources as in the permit application. There have been no changes to these sources. A.4 2D .0521, "VISIBLE EMISSIONS CONTROL REQUIREMENT" — Facility -wide 20% opacity limit., APPEARED IN COMPLIANCE — Each emission source was observed and 0% opacity was noted. A.5 2D .0535, "NOTIFICATION REQUIREMENT"— Facility is required to notify DAQ of excess emissions lasting longer than four hours. APPEARED IN COMPLIANCE — There are no indications of excess emissions requiring a notification. Mr. Gay is aware of the notification requirement. A.6 21) .0540 "FUGITIVE DUST CONTROL REQUIREMENT" — Fugitive dust emissions shall not cause or contribute to substantive complaints or excess visible emissions beyond the property boundary. APPEARED IN. COMPLIANCE — I did not observe any dust leaving the facility boundaries and Mr. Gay stated there had been no recent complaints. The DAQ has not received any recent complaints either. The facility uses sprinklers for dust suppression on the stockpiles. A.7 2D .0611, "FABRIC FILTER REQUIREMENTS" — Requires annual internal inspection and maintenance activities documented in logbook. APPEARED IN COMPLIANCE - Logbook indicated that the last internal inspection on the central bagfilter was performed on 31 May 2016. The facility performs these inspections on a monthly basis. A.8 213.1806 "CONTROL AND PROHIBITION OF ODOROUS EMISSIONS" — Prevent odorous emissions from the facility from causing or contributing to objectionable odors beyond the facility's boundary. APPEARED IN COMPLIANCE — No objectionable odors were noticed while onsite; no complaints have been received. A.9 40 CFR 63 SUBPART 6J "NESHAP FOR COMMERCIAL BOILERS" — Subject source is kerosene -fired hot water heater permit ID No. I-WH. Requires an Initial Notification submitted by 20 January 2014; initial tune up within 30 days of startup; Notice of Compliance Status submitted by CEDRI within 120 days after tune up; subsequent tune ups every 5 years. An Energy Assessment is not required. Records maintained on -site. DEFICIENCY NOTED -- The hot water heater I-WH had been operated 5 or 6 days during the 2016 winter months. The plant manager, Mr. Robbie Nobles, provided this information and also stated he had no written records associated with the boiler. There were also no records which indicated that a tune-up was performed, or that the required Notification of Compliance Status has been submitted During the 2014 inspection, Mr. Gay was provided GACT 6J Forms and instructions, including instructions on submitting the CDX CEDRI notification, and appeared to the inspector to be aware that the facility is required to be in compliance with this regulation before using the hot water heater again at this site. Mr. Gay was provided with this information again during this inspection. Since this was a multimedia inspection, the issue was referred to the North Carolina•Division of Air Quality for follow up. NON-COMPLIANCE HISTORY SINCE CY2010: No non-compliance issues since CY2010. 112r STATUS: The facility does not use or store any chemicals that make it subject to or require a written Risk Management Program as required under 40 CFR Part 68. RECOMMENDATIONS AND COMPLIANCE STATEMENT: During discussions with Mr. Gay and Mr. Robbie Nobles, Plant Manager, it was confirmed that the hot water boiler had been operated 5 or 6 times during the winter of 2016, There were no written records as required by NESHAP 6J requirements. Since this was a Multi Media inspection this deficiency item was referred to the parent agency, NC Division of Air Quality, for follow up. The S&W Ready Mix Co — Lumberton appeared to be operating IN COMPLIANCE with their air permit with the exception of the noted deficiencies regarding GACT 6J. This permit is reissued pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between the state of North Carolina and the U.S. Environmental Protection Agency, dated October 15, 2007 (or as subsequently amended). The following information is included with your permit package: • A new Certificate of Coverage • A copy of General Permit NCG140000 • A copy of the Technical Bulletin for the General Permit • Two copies of the Discharge Monitoring Report (DMR) Form for Stormwater • Two copies of the DMR Form for Wastewater • Two copies of the Qualitative Monitoring Report Form The General Permit authorizes discharges of stormwater and/or wastewater, and it specifies your obligations with respect to discharge controls, management, monitoring, and record keeping. If your Certificate of Coverage (CDC) restricts authorization to discharge stormwater only (or other limited terms), you must contact the Stormwater Permitting Unit if you would like to request a modification to your COC. The first sample period of your permit begins July 1, 2011. However, because of the delayed mail -out of permit packages, your facility is expected to attempt to sample during the first sampling period, and must comply with sampling requirements beginning during the second sampling period. Your facility must sample a representative storm event between January 1 and July 30, 2012 (for stormwater sampling), and/or sample wastewater between October 1, 2011 and December 31, 2011 (or, if applicable, report "No Flow", as outlined in Part V, Section E). Additionally, your facility has 6 months from the time of receipt of the permit to update SPPP to current standards. The more significant changes in the General Permit since your last Certificate of Coverage include the following (please note the names and numbers of sections have been changed from the previous permit revision): Part II: • Authorization to Construct (ATC) language was added to the permit and adapted to fit NCG140000. Part III: • The Stormwater Pollution Prevention Plan (SPPP) section has been updated to the most current language of our permits. Some additional conditions specific to this industry have been added to the SPPP language.. Part IV: • Sections R & 8: Revised provisions require stormwater discharges be sampled twice per year (instead of annually), and sampling results shall be compared to new benchmark values. (The previous cut-off concentrations have been removed). • Sections A & B: Revised provisions require the permittee to execute Tier One, Two & Three response actions: Tier One requirements for a first benchmark exceedence, and/or Tier Two response for a second consecutive benchmark exceedence, if applicable. Tier Two requires that the permittee institute monthly monitoring (reported to the appropriate Regional Offices) instead of twice -per -year monitoring. Monthly monitoring shall be done until three consecutive monitoring events show no benchmark exceedences. • Sections A & B: This permit now contains a provision that four exceedences of any benchmark will trigger increased DWQ involvement in the permittee's stormwater management and control actions (Tier Three). DWQ may direct the permittee to apply for an individual permit, institute specific monitoring, or may direct the implementation or installation of specific stormwater control measures. • Sections A & B: Failure to perform analytical stormwater monitoring requires that the permittee begin a monthly sampling scheme for both industrial stormwater discharges and/or stormwater discharges from VMA areas (where applicable). After 6 months of monthly sampling, the permittee may return to semi- annual sampling, unless otherwise specified by DWQ. • Sections A & B: A lower TSS benchmark of 50 mg/I for HQW, ORW, PNA and Tr Waters was added to this permit for these more sensitive waters. • Section B: The monitoring parameter Oil & Grease has been replaced by the parameter Total Petroleum Hydrocarbons. • Sections A,"B & C: Inability to sample due to adverse weather must be reported in the SPPP. • Sections A, B & C: The terminology "Representative Storm Event" has been replaced by "Measurable Storm Event" in response to comments from industry groups.
