Mineral, and Land Resources (DEMLR) is forwarding herewith the reissued Certificate of
Coverage (COC), This permit is reissued pursuant to the requirements of North Carolina
General Statute 143-215.1 and the Memorandum of Agreement between the state of North
Carolina and the U.S. Environmental Protection Agency, dated October 15, 2007 (or as
subsequently amended.) A new Certificate of Coverage (COC) is included with this
letter.
You must print a copy of the new NCG140000 General Permit from our website here:
permits jstormwater- eermits jnpdes-industrial-sw. In addition to the full permit, the 2017
print package on the website includes revised Discharge Monitoring Report (DMR) forms,
Annual Discharge Monitoring Report forms, and Qualitative Monitoring Report Forms.
The General Permit authorizes discharges of stormwater and wastewater (if applicable),
and it specifies your obligations with respect to discharge controls, management,
monitoring, and record keeping. Please review the new permit to familiarize yourself with
all changes in the reissued permit. Significant changes to the General Permit are
outlined in the Technical Bulletin, which is also available on the website above. Your
facility has six months from receipt of the permit to update your Stormwater Pollution
Prevention Plan (SPPP) to reflect any new permit requirements.

How does the new General Permit affect Tier Status?
The first sample period of your permit begins August 1, 2017. Tier 3 actions in Part IV of
your permit are triggered by benchmark exceedances on four occasions beginning on the
effective date of this permit and do not count prior exceedances. Likewise, Tier 2 actions
are triggered by two consecutive benchmark exceedances beginning on the effective date
of this permit and do not count prior exceedances. However if your facility is already in
Tier 2 (monitoring stormwater discharges monthly) or Tier 3, you must continue monthly
monitoring until relieved through conditions of the permit or by DEMLR staff approval.
When does electronic DMR reporting start?
We are setting up our database with final permit parameters and the outfalls that
permittees submitted on-line. All NCG14 Permittees will receive notification when our
eDMR system is ready for these permittees to register and begin reporting
monitoring data electronically. If you have any questions about the status, please
contact Bethany Georgoulias at (919) 807-6372 or Robert Patterson at (919) 807-6369. If
you did not submit outfall information already, we have included a form for you to return
to us. (Note: NO FORM IS ATTACHED if we already have your outfall information.)
Your coverage under the General Permit is transferable only through the specific action of
DEMLR. This permit does not affect the legal requirements to obtain other permits which
may be required by the N.C. Department of Environmental Quality (DEQ), nor does it
relieve the permittee from responsibility for compliance with any other applicable federal,
state, or local law, rule, standard, ordinance, order, judgment, or decree.

NESHAP APPLICABILITY:
The facility did not have a hot water heater or boiler

The facility has a diesel -fired emergency RICE that is used as backup power for the plant hydraulics if the
power grid goes down. The facility had not operated this RICE in over 2 years and this system is
currently inoperable.
SAFETY CONSIDERATIONS:
All standard DAQ FRO safety gear is required. Be aware of equipment moving around facility grounds,
as well as overhead conveyors.
DIRECTIONS:
From FRO travel about 35 mile on highway 24 East to Clinton. Continue following Hwy 24 East
traveling about 2-1/2 miles and at the stop light indicating to turn right to stay on this road, then travel for
about three miles and the facility is on left.
FACILITY DESCRIPTION:
Concrete is composed essentially of water, cement, fly -ash, sand (fine aggregate), and coarse aggregate.
Cement and fly -ash are pneumatically transferred from tanker truck to elevated silos. The silos discharge
a prescribed amount into the weigh hopper above the load -out for the concrete trucks. The silos and truck
load -out, including weigh hopper, are all ducted to a central bagfilter. Aggregate is transferred from stock
piles to an elevated storage bin by front-end loader and belt conveyor. The bin discharges the aggregate
to a separate weigh scale below and conveyors transfer the prescribed amounts to the truck load -out.
Water is also weighed and added to the truck. The load -out has been enclosed on three sides with a heavy
rubber shroud to reduce fugitive dust during loading. The facility has a diesel -fired emergency RICE that
is used as backup power for the plant hydraulics if the power grid goes down. The facility has no hot
water beater or boiler at this time.

One Concrete batch plant with fabric filter air pollution control system(s) installed on all sources;
1. One (1) cement mixing weigh hopper and loading operation; - Not operating. Controlled by a
central baelter.
2. Silos for cement and flyash storage
Insignificant Source: (This source has a separate weigh hopper/scale.) — Not operating.
INSPECTION SUMMARY:
On 21 September 2016, 1, Mark Brantley, of DWR FRO, met with William McDaniel, Plant Manager, for
a compliance inspection. Mr. McDaniel confirmed the accuracy of the contacts currently listed on FAC
FINDER for the facility, production data, and bagfilter I&M records. He stated that there had been no
changes at the facility since the last inspection. He stated that no complaints had been received from
anyone concerning air quality.
• Since the previous inspection, the permit has been rescinded for this facility on 09 August 2016.
Information is recorded by the facility in logbooks regarding monthly inspections of the baghouse
and weekly inspections of all other equipment.
REGULATORY REVIEW:
2D .0515, "PARTICULATE CONTROL REQUIREMENT" — Particulate emissions shall not
exceed allowable emission rates.
APPEARED IN COMPLIANCE — This facility was previously permitted, and the most recent
review indicated compliance so long as the facility operates and maintains the control devices, as
designed and recommended by the manufacturer, on all point sources at the facility. There have
not been any modifications to the facility since the facility's last permit review. All control devices
appeared to be operated and maintained as required.
2D .0521, "VISIBLE EMISSIONS CONTROL REQUIREMENT"— Facility -wide 20% opacity
limit
APPEARED IN COMPLIANCE —No emission sources were operating or being filled at the
time.
• 2D .0535 NOTIFICATION REQUIREMENT - The facility is required to notify DAQ if excess
emissions occur for more than a 4-hour period caused by a breakdown or other abnormal condition.
APPEARED IN COMPLIANCE - There were no indications of excessive emissions which
would require notification of DAQ.
• 2D .0.540, "FUGITIVE DUST CONTROL REQUIREMENT" -- Facility -wide fugitive dust
emission shall not causing excess VE or complaints beyond the property boundary.
APPEARED IN COMPLIANCE — I did not observe any dust leaving the facility boundaries
and Mr. McDaniel stated there have been no complaints. I did not observe excessive dust
deposits at the highway entrance to the facility. Most of the facility yard and entrance road were
covered with gravel.
NON-COMPLIANCE HISTORY SINCE CY2010:
No incidents of non-compliance on record since CY2010.

The facility does not use nor store any applicable materials in quantities that would require a written 112r
plan.
Recommendations and Compliance Statement:
Based on review of records and visual observations, this facility appeared to be operating in compliance
with air quality standards and regulations at the time of this inspection.

Enclosed please find the Compliance Inspection Report for the inspection conducted
November 7, 2012 of the Ready Mixed Concrete Company - Plant 120 located at 3095
Turkey Highway, Clinton North Carolina. The assistance and cooperation of Mr.
William McDaniel, Plant Manager during the inspection is greatly appreciated.
Stormwater, should it leave the site, would drain to an Unnamed Tributary of Abrams
Branch in the Cape Fear River Basin The subject facility is covered by NPDES
Stormwater General Permit No. NCG140000, Certificate of Coverage No. NCG140298'.
All items reviewed were considered compliant. The following items are items noted or
discussed during the inspection, please refer to the enclosed inspection report for other
comments.
The site was found to be very neat and well maintained.
- All records requested were present and well organized.
- Based on observations made during the inspection and comments made by Mr.
McDaniel the site does not discharge process wastewater, this was confirmed during
the site tour.
- Solids removed from settling basin(s) are dried and taken offsite as fill or road bed
material.

- As part of the inspection a tour of the site, that included walking nearly all of the property line,
was conducted. The only Stormwater Discharge Outfall (SDO), a point that discharges into a
roadside ditch (adjacent to Turkey Hwy), was inspected with no visible sign of past
discharges, Based on comments made by Mr. McDaniel and observations made of the
SDO stormwater appears to come from a small portion of the parking area in front of the
Control Building. This flow appears to travel toward the property line as sheet flow and is then
concentrated in a very shallow well vegetated onsite swale. Stormwater then travels into a
short section of pipe (+/- 20 foot long plastic pipe) and then exits the site.
When questioned about this area Mr. McDaniel stated that the samples taken from the SDO
may actually contain stormwater from the roadside ditch and it is unclear if the site routinely
discharges stormwater, except during instances of extreme weather events.
Per my discussion with Mr. McDaniel the Company should not feel compelled to
sample any point that is not a true Stormwater Discharge Outfall (SDO) or is not
representative of the stormwater leaving the site.
It is suggested that the Company review the site and consider placing a earthen/rock berm to
promote sheet flow. If the site is modified resulting in no point source discharge(s) a
stormwater permit may not be required. If you would like to discuss this item please do not
hesitate to contact me.
During the inspection there was a brief discussion pertaining to two 250 gallon
containers of truck washing liquid that is not kept inside the secondary containment
area. A review of the terms and conditions of your permit revealed that those containers
are not required to be in the containment area. It is however suggested that the
containers be placed in the secondary containment area to prevent the release of the
noted material to the land, surface or ground waters should the containers or
associated piping fail. This suggestion is made in the spirit of environmental protection
but the final decision is of course that of the Ready Mixed Concrete Company.
Secondary containment was noted to be clean. The containment area drain is
constructed of PVC pipe that has a valve that the handle has been removed. Mr.
Roberson indicated that the drain is not used as the area was constructed in a manner
that the low side is at the opposite corner than that of the drain. A portable pump is used
to drain the area. Control of this pump is accomplished by removing the extension cord
each time the area is drained.
Based on discussions with Mr. Roberson the noted valve will be capped and use of the
pump will be controlled by keeping the extension cord in the "Control Building".
- Employee training includes discussions of the stormwater permit and other
environmental items with the facility staff.
- No significant spills recorded. The definition of "Significant Spill" was discussed.

- As discussed all staff that are part of stormwater management on the site should be
encouraged to review the permit and associated plans as often as necessary to be
aware of the terms and conditions contained in the document.

Per my discussion with Mr. McDaniel the Company should not feel compelled to

representative of the stormwater leaving the site.
It is suggested that the Company review the site and consider placing a earthen/rock berm to
promote sheet Flow. If the site is modified resulting in no point source discharge(s) a

in the spirit of environmental protection but the final decision is of course that of the Ready
Mixed Concrete Company.
- Secondary containment was noted to be clean. The containment area drain is constructed of PVC pipe
that has a valve that the handle has been removed. Mr. Roberson indicated that the drain is not used
as the area was constructed in a manner that the low side is at the opposite corner than that of the
drain, A portable pump is used to drain the area, Control of this pump is accomplished by removing
the extension cord each time the area is drained.
Based on discussions with Mr. Roberson the noted valve will be capped and use of the
pump will be controlled by keeping the extension cord in the "Control Building".
- Employee training includes discussions of the stormwater permit and other
environmental items with the facility staff.
- No significant spills recorded. The definition of "Significant Spill" was discussed.

All items reviewed were considered compliant. The following items are items noted or

- The site was found to be very neat and well maintained.

- As part of the inspection a tour of the site, that included walking nearly all of the property line,
was conducted. The only Stormwater Discharge Outfall (SDO), a point that discharges into a
roadside ditch (adjacent to Turkey Hwy), was inspected with no visible sign of past
discharges. Based on comments made by Mr. McDaniel and observations made of the
SDO stormwater appears to come from a small portion of the parking area in front of the
Control Building. This flow appears to travel toward the property line as sheet flow and is then
concentrated in a very shallow well vegetated onsite swale. Stormwater then travels into a
short section of pipe (+1- 20 foot long plastic pipe) and then exits the site.

Dear Permittee:
In response to your renewal application for continued coverage under stormwater General Permit NCG140000
the Division of Water Quality (DWQ) is forwarding herewith the reissued General Permit. This permit is reissued
pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement
between the state of North Carolina and the U.S. Environmental Protection Agency, dated October 15, 2007 (or
as subsequently amended).
The following information is included with your permit package:
• A new Certificate of Coverage
• A copy of General Permit NCG140000
• A copy of the Technical Bulletin for the General Permit
• Two copies of the Discharge Monitoring Report (DMR) Form for Stormwater
• Two copies of the DMR Form for Wastewater
• Two copies of the Qualitative Monitoring Report Form
The General Permit authorizes discharges of stormwater and/or wastewater, and it specifies your obligations with
respect to discharge controls, management, monitoring, and record keeping. Please review the new permit to
familiarize yourself with all the changes in the reissued permit. If your Certificate of Coverage (COC) restricts
authorization to discharge stormwater only (or other limited terms), you must contact the Stormwater Permitting
Unit if you would like to request a modification: to your COC.
The first sample period of your permit begins July 1, 2011. However, because of the delayed mail -out of permit
packages, your facility is expected to attempt to sample during the first sampling period, and must comply with
sampling requirements beginning during the second sampling period. Your facility must sample a representative
storm event between January 1 and July 30, 2012 (for stormwater sampling), and/or sample wastewater between
October 1, 2011 and December 31, 2011 (or, if applicable, report "No Flow", as outlined in Part V, Section E).
Additionally, your facility has 6 months from the time of receipt of the permit to update SPPP to current standards.
The more significant changes in the General Permit since your last Certificate of Coverage include the following
(please note the names and numbers of sections have been changed from the previous permit revision):
Part II:
• Authorization to Construct (ATC) language was added to the permit and adapted to fit NCG140000.
Part III:
• The Stormwater Pollution Prevention Plan (SPPP) section has been updated to the most current
language of our permits. Some additional conditions specific to this industry have been added to the
SPPP language.
Part IV:
• Sections A & B: Revised provisions require stormwater discharges be sampled twice per year (instead of
annually), and sampling results shall be compared to new benchmark values. (The previous cut-off
concentrations have been removed).
• Sections A & B: Revised provisions require the permittee to execute Tier One, Two & Three response
actions: Tier One requirements for a first benchmark exceedence, and/or Tier Two response for a second
consecutive benchmark exceedence, if applicable. Tier Two requires that the permittee institute monthly
monitoring (reported to the appropriate Regional Offices) instead of twice -per -year monitoring. Monthly
monitoring shall be done until three consecutive monitoring events show no benchmark exceedences.
• Sections A & B: This permit now contains a provision that four exceedences of any benchmark will trigger
increased DWQ involvement in the permittee's stormwater management and control actions (Tier Three).
DWQ may direct the permittee to apply for an individual permit, institute specific monitoring, or may direct
the implementation or installation of specific stormwater control measures.
• Sections A & 8: Failure to perform analytical stormwater monitoring requires that the permittee begin a
monthly sampling scheme for both industrial stormwater discharges and/or stormwater discharges from
VMA areas (where applicable). After 6 months of monthly sampling, the permittee may return to semi-
annual sampling, unless otherwise specified by DWQ.
• Sections A & B: A lower TSS benchmark of 50 mg/I for HOW, ORW, PNA and Tr Waters was added to
this permit for these more sensitive waters.
• Section B: The monitoring parameter Oil & Grease has been replaced by the parameter Total Petroleum
Hydrocarbons.
• Sections A, B & C: inability to sample due to adverse weather must be reported in the SPPP.
• Sections A, B & C: The terminology "Representative Storm Event" has been replaced by "Measurable
Storm Event" in response to comments from industry groups. A measurable storm event is a storm event
that results in an actual discharge from the permitted site outfall. The previous measurable storm event
must have been at least 72 hours prior, unless the permittee is able to document that a shorter interval is
representative for local storm events during the sampling period, and obtains written approval from the
local DWQ Regional Office.
• Section C: Qualitative monitoring must now be conducted semi-annually and is tied to the same
measurable storm event as the analytical monitoring.
• Section C: If the permittee fails to respond effectively to problems identified by qualitative monitoring, the
permittee may be required by DWQ to perform corrective action.
• Section D: This section has now been consolidated to one section with one set of combined tables for all
types of authorized wastewater discharges.
• Section D: The wastewater analytical monitoring schedule has been set to quarterly.
• Section D: TSS limits have been revised for PNA, Trout and HOW waters per rule requirements in 15A
NCAC 02B .0224.
• Section D: 7Q10-based flow limits have been set for PNA waters per regulations in 15A NCAC 02B.
• Section D: TPH monitoring requirements have been added to wastewater monitoring when stormwater
runoff from VMA areas commingles with wastewater.
• Section D: This wording in the permit has been removed: "For facilities which route stormwater and all
process wastewater to a recycle system, no analytical monitoring of process wastewater discharges is
required if that recycle system discharges as a result of a 10-year 24-hour (or larger) precipitation event."
Part V:
• Section A: For existing facilities previously permitted and applying for renewal under this General Permit:
New elements of the Stormwater Pollution Prevention Plan for this permit renewal shall be developed and
implemented within 6 months of the effective date of the initial Certificate of Coverage issued pursuant to
this General Permit and updated thereafter on an annual basis.
Your coverage under the General Permit is transferable only through the specific action of DWQ. This permit does
not affect the legal requirements to obtain other permits which may be required by DENR, nor does it relieve the
permittee from responsibility for compliance with any other applicable federal, state, or local law, rule, standard,

located of 87 ur ey i , linton, NC. Our new plant will also be located on
Turkey Highway, the street address has not been assigned at this time. Ready
Mixed Concrete began its operation in the Sampson County market area in June
2000. In a new market, property was leased to operate on until a permanent site
location could be obtained. In December 2001, Pope Brothers Farm made a site
adjoining the leased property in use, which is located on the same side of Turkey
Highway, available to Ready Mixed Concrete. With. the approval of the County
Zoning and Planning Department the property was purchased.
Enclosed you will find a copy of the Zoning letter from Sampson County and a
copy of our current permit. Please feel free to contact me if there are any

In accordance with your application for discharge permit received on July 24, 2000, we are forwarding
herewith the subject certificate of coverage to discharge under the subject state - NPDES general permit. This permit
is issued pursuant to the requirements of North Carolina General Statute I43-2I5 .1 and the Memorandum of
Agreement between North Carolina and the US Environmental Protection agency dated December 6, 1983.
The stormwater permit you are receiving, NCG140000, does not permit wasting concrete, dumping excess
concrete, directly into storm sewer outfalls or into waters of the state. Any facility which either dumps excess
concrete or washes excess concrete into storm sewers or waters of the state will be operating in direct violation to
both the terms of this permit and the North Carolina General Statutes. Such a discharge shall be considered an illegal
discharge and may subject the owner to enforcement actions in accordance with North Carolina General Statutes
143-215.6A
If any parts, measurement frequencies or sampling requirements contained in this permit are unacceptable to
you, you have the right to request an individual permit by submitting an individual permit application. Unless such
demand is made, this certificate of coverage shall be final and binding.
Please take notice that this certificate of coverage is not transferable except after notice to the Division of
Water Quality. The Division of Water Quality may require modification or revocation and reissuance of the
certificate of coverage.
This permit does not affect the legal requirements to obtain other permits which may be required by the
Division of Water Quality or permits required by the Division of Land Resources, Coastal Area Management Act or
any other Federal or Local governmental permit that may be required.
