Dear Permittee:
For coverage under Stormwater General Permit NCG140000, the Division of Energy,
Mineral, and Land Resources (DEMLR) is forwarding herewith the reissued Certificate of
Coverage (COC). This permit is reissued pursuant to the requirements of North Carolina
General Statute 143-215.1 and the Memorandum of Agreement between the state of North
Carolina and the U.S. Environmental Protection Agency, dated October 15, 2007 (or as
subsequently amended.) A new Certificate of Coverage (COC) is included with this
letter.
You must print a copy of the new NCG140000 General Permit from our website here:

The General Permit authorizes discharges of stormwater and wastewater (if applicable),
and it specifies your obligations with respect to discharge controls, management,
monitoring, and record keeping. Please review the new permit to familiarize yourself with
all changes in the reissued permit. Significant changes to the General Permit are
outlined in the Technical Bulletin, which is also available on the website above. Your
facility has six months from receipt of the permit to update your Stormwater Pollution
Prevention Plan (SPPP) to reflect any new permit requirements.

How does the new General Permit affect Tier Status?
The first sample period of your permit begins August 1, 2017. Tier 3 actions in Part IV of
your permit are triggered by benchmark exceedances on four occasions beginning on the
effective date of this permit and do not count prior exceedances. likewise, Tier 2 actions
are triggered by two consecutive benchmark exceedances beginning on the effective date
of this permit and do not count prior exceedances. However, if your facility is already in
Tier 2 (monitoring stormwater discharges monthly) or Tier 3, you must continue monthly
monitoring until relieved through conditions of the permit or by DEMLR staff approval.
When does electronic DMR reporting start?
We are setting up our database with final permit parameters and the outfalls that
permittees submitted on-line. All NCG14 Permittees will receive notification when our
eDMR system is ready for these permittees to register and begin reporting
monitoring data electronically. If you have any questions about the status, please
contact Bethany Georgoulias at [phone] or Robert Patterson at [phone]. If
you did not submit outfall information already, we have included a form for you to return
to us. (Note: NO FORM IS ATTACHED if we already have your outfall information.)
Your coverage under the General Permit is transferable only through the specific action of
DEMLR. This permit does not affect the legal requirements to obtain other permits which
may be required by the N.C. Department of Environmental Quality (DEQ), nor does it
relieve the permittee from responsibility for compliance with any other applicable federal,
state, or local law, rule, standard, ordinance, order, judgment, or decree.

NCG140180. The NCG140000 General Permit has been reissued with no changes for one year, effective Friday, July 1,
2016. As we stated in our letter from May 31', we will not issue you a new Certificate of Coverage until next year,
when the General Permit is revised. However, the cover page of the permit has been revised to reflect new effective
dates.

Inspection Summary:
Melissa Joyner met with Dewey Jernigan, Operations Manager and Troy Wortham, Plant Manager on February 24, 2017 at
the facility located on 336 Hobson Road, Raeford, in Hoke county as part of a Multimedia inspection to determine
compliance with Air Quality regulations and the conditions of the NPDES General Permit NCG140000. This facility is
currently owned and operated by Argos Ready Mix. A Permit Name/Ownership Change Form for this facility or letter from
the current owner updating ownership of all facilities in the state needs to be submitted to staff with the Stormwater
Permitting Program at the Central Office in Raleigh in order to update our files. The Stormwater Pollution Prevention Plan
was reviewed and contained the required components as specified in the permit. No spills have taken place at the facility
since the previous inspection of the facility in October 2012. There is one outfall on the site. Since both stormwater and
processed wastewater would be discharged from this outfall when the facility is operational, the outfall is monitored on a
quarterly basis. The monitoring records indicated no flow/no discharge for the monitoring periods since the previous
inspection of the facility. The facility has one stormwater-only outfall, therefore the annual non-stormwater discharge
certification is not applicable. A tour was conducted of the facility's grounds where the outfall, secondary containment area
and concrete washout pit was observed. The outfall was at a low point on the property and received sheet flow. There was
no visual evidencs of recent discharge through the outfall. The facility appeared well -maintained and in compliance with the
conditions of General Permit NCG140000.

While most items covered were considered compliant there are items that need attention. These and
other findings are listed below:
-The site was found to be neat and well maintained,
- Located on the site is a second ready mixed plant # 121. The second plant appears to
generates wastewater and stormwater in the same manner as plant # 118. Ready Mixed Concrete
Company
is asked to make sure your Stormwater Pollution Prevention Plan and other required components of
your
permit reflect the presence of the plant # 121. Further, you are also asked to make sure that
onsite BMPs are designed and operated in manner to treat the additional loading.
- Considerable time was taken going over the permit. When discussing analytical and qualitative
monitoring and during the site tour it became apparent that site personnel have been sampling
a point that is NOT representative of the actual discharge from the site should a discharge
occur. The stormwater sampling point was noted to be a point in which stormwater would pool
near the rear of the property. Based on discussions with you and Mr. Underwood and based
on a rather lengthily tour of the property it is unclear if the site ever actually discharges.
Noted during the tour was a berm of soil and a old silt fence at the rear of the property. Also
observed was a depression or old ditch that had no point of discharge.
Per our discussion you should not feel compelled to sample any point that is not a
true Stormwater Discharge Outfall (SDO). You should never sample anything that is
not a true discharge and representative of the stormwater/process wastewater leaving
the site.

In keeping with our discussion it is understood that you are going to clear (mow) the rear of
the property in order to properly determine the stormwater flow path(s) and where the true
SDO point(s) are located. Also discussed was the possibility of determining site contours or
elevations to determine the "low spot".
In the event that you do not feel the site routinely or ever discharges you are asked to
establish and clearly mark the point a discharge would occur.
You are asked to contact the writer within 30 days of completion of the noted work to
schedule a second site visit. This second visit will be, at a minimum, for the
verification of SDOs should they exist.
Discharge Monitoring Reports. As noted above please do not feel compelled to sample if
your site(s) do not discharge. Please refer to Section E: Reporting Requirements, item 1 of
your permit. The permit states, in part
"When no discharge has occurred from the facility during the report period, the permittee is
required to submit a discharge monitoring report, within 30 days of the end of the sampling
period, giving all required information and indicating "NO FLOW" as per 15A NCAC 028 .0506."
please in the future submit the required discharge monitoring report with "NO FLOW "
indicated if called for. Further, make sure that qualitative monitoring is conducted as required.

This letter is to relay the findings of a follow-up site visit to the Ready Mixed Concrete
Company - Plant 118 located at 336 Hobson Road, Raeford -North Carolina October 31,
2012. The follow-up visit was to observe site clearing (mowing) that had been
performed at the rear of the property to determine the exact location of the stormwater
discharge outfall (SDO).
Based on your comments and the site visit it was determined that there is only one SDO
for the site. Further, it is believed that the site will only discharge during extreme
weather events and/or during periods of extended wet weather.
You are urged to keep the vegetation at this location managed in such a manner that
observations of the SDO can be performed easily and safely. Also please take samples
of stormwater discharges only as outlined in your permit and submit the data as
required. As discussed, if the site does not discharge please follow the instructions for
"No Flow" discharge reports set forth in your permit.

generator or other engine that would be subject to GACT 4Z.

From FRO travel about 10 miles on Business Hwy 401 South/Raeford Rd; about 3.5 miles after
entering Hoke County, turn right onto Hobson Road; travel about 0.2 miles; facility is on the
right.
3) Safety Equipment: Standard DAQ FRO safety gear. Be aware of moving equipment and overhead
conveyors when operating.
4) Process/Facility Description:
Concrete is composed essentially of water, cement, fly ash, and aggregate (fine/coarse). Fly ash used as a
specialty aggregate. The plant stores, conveys, measures and discharge these constituents into a truck for
mixing and transport to the job site. A dump pit is used to transfer sand and rock to one of three storage
silos via an open belt system. The sand and rock is then transferred to the three -bin hopper via an open
belt system. Sand and aggregate is then weighted for use by the weigh -hopper under the three -bin
hopper. The material is then transferred to the mixing hopper via a conveyor belt and is mixed with
water, cement and sometimes fly ash. The fly ash is conveyed to the mix hopper via a closed augur which
is later dropped into the truck.

7) Opening Conference:
On 21 October 2015, I, Mitchell Revels with DAQ FRO met with Ronnie Butner, Plant Manager and
Kevin Honecker. Mr. Honecker said that he is the plant manager for the facility located in Spring Lake
and that he was at this plant only to assist the new plant manager, Ronnie Butner. He also said that Mr.
Dennis Underwood was moved to the plant in Laurinburg. I explained the permit conditions and what is
expected during the compliance inspection.
Mr. Honecker provided the I/M logbook that contained the air permit and the bagfilter maintenance
records along with production records. It seemed that Mr. Butner was not as knowledgeable as Mr.
Honecker concerning the permit and records required by the permit. Mr. Honecker indicated that he
would be working with Mr. Butner to get him up to speed with the air permit requirements. Mr.
Honecker reviewed the FACFINDER and indicated that all was correct with exception to Mr. Underwood
replaced with Mr. Butner. My review of the log indicated an annual inspection occurred on 8 July 2015
by the contractors Filter Kleen Environmental Service for both plants. The log also indicated daily
general inspections are done by Ronnie Butner. Mr. Butner stated that Plant #2 was presently not needed
and Plant # 1 will produced about 9,500 cubic yards for cy 2015.
8) Walk Through:
Mr. Honecker and I went outside to walk the surrounding grounds. A truck was loaded with concrete to
be delivered and no dust appeared. The plant appeared to be well maintained and operated. According to
records, this facility produces a relatively small amount of concrete annually, around 9500 cubic yard.
The permit concrete production limit for NC Toxic, 15A NCAC 2D.1100 is 105,000 cubic yards.

AA 2D .0521 VISIBLE EMISSIONS CONTROL REQUIREMENT - Visible emissions from the
facility, manufactured after 1 July 1971 shall not exceed 20% opacity.
Appears in Compliance —No VE was observed, not operating. The facility was loading cement during
the previous inspection by Pam Vivian on 2014 Nov 21 and she stated no VE was observed.
A.5 2D .0535, "Notification Requirement" — subject sources are entire facility. Requires notification of
excess emissions that lasts more than 4 hours.
Appeared in Compliance - Mr. Honecker stated that to the best of his knowledge, this facility has not
had any excess emissions that exceeded limits for more than four.
A.6 2D .0540 FUGITIVE NON -PROCESS DUST EMISSIONS —The Permittee shall not allow
fugitive non -process dust emissions to contribute to substantive complaints.
Appears in Compliance — The facility does use water suppression to help control dust emissions on the
grounds. I did not observe excessive fugitive dust or any fugitive dust leaving the property. No dust
complaint have been received by DAQ during the past year.
A.7 2D .0611 FABRIC FILTER REQUIREMENTS — The Permittee shall record bag filter inspections
and maintenance in a logbook.
Appears in Compliance — The I/M logbook indicates contractors Filter Kleen Services performed an
annual compliance inspection on all control devices on 8 July 2015 and daily checks are done by plant
personnel. UM records indicate that cartridge filters are routinely changed during the annual inspection.
A.8 215.1100 TOXIC AIR POLLUTANT CONTROL REQUIREMENTS — Arsenic facility wide
limt: 0.05063 Ib/yr - The maximum concrete production is a function of the distance to the closest
property line; for this facility the distance is 154 feet and the maximum annual production can be 105,000
yards. The Permittee shall record concrete production each month and total annually. Maximum hourly
throughput must not exceed 138 cubic yards per hour. The Permittee shall submit an annual report by I
March of each year.
Appears in Compliance — The closest property line to the mixer is staked and was determined using a
range finder to be 162 ft, which is consistent with the required distance for this facility
. The 2014 yearly annual report was received on time; 2014 concrete production was about 9500 cubic
yards per year. The permit production is 105,000 cubic yards and the facility has produced around 9,500
cubic yards annually for the past several years.

This facility has no non-compliance history since 2010.

This facility neither uses nor stores chemical compounds that require a written risk management plan
under the Clean Air Act, Section 112R.

The facility appears to be well -maintained and operated. It appeared to be operating in compliance as of
this inspection.

forwarding herewith the subject certificate of coverage to discharge under the subject state - NPDES general
permit. This permit is issued pursuant to the requirements of North Carolina General Statute 143-215 .1 and the
Memorandum of Agreement between North Carolina and the US Environmental Protection agency dated
December 6, 1983.
The stormwater permit you are receiving, NCG140000, does not permit wasting concrete, dumping
excess concrete, directly into storm sewer outfalls or into waters of the state. Any facility which either dumps
excess concrete or washes excess concrete into storm sewers or waters of the state will be operating in direct
violation to both the terms of this permit and the North Carolina General Statutes. Such a discharge shall be
considered an illegal discharge and may subject the owner to enforcement actions in accordance with North
Carolina General Statutes 143-215.6A
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to request an individual permit by submitting an individual permit
application. Unless such demand is made, this certificate of coverage shall be final and binding.
Please take notice that this certificate of coverage is not transferable except after notice to the Division of
Water Quality. The Division of Water Quality may require modification or revocation and reissuance of the
certificate of coverage.
This permit does not affect the legal requirements to obtain other permits which may be required by the
Division of Water Quality or permits required by the Division of Land Resources, Coastal Area Management Act

In accordance with your application for a stormwater discharge permit received on January 2, 1995, we are
forwarding herewith the subject state - NPDES general permit. This permit is issued pursuant to the requirements
of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between North Carolina and
the US Environmental Protection agency dated December 6, 1983.
The stormwater permit you are receiving, NCG140000, does not permit wasting concrete, dumping excess
concrete, directly into storm sewer outfalls or into waters of the state. Any facility which either dumps excess
concrete or washes excess concrete into storm sewers or waters of the state will be operating in direct violation to
both the terms of this permit and the North Carolina General Statutes. Such a discharge shall be considered an
illegal discharge and may subject the owner to enforcement actions in accordance with North Carolina General
Statutes 143-215.6A
If any parts, measurement frequencies or sampling requirements contained in this permit are unacceptable to
you, you have the right to an adjudicatory hearing upon written request within thirty (30) days following receipt
of this letter. This request must be in the form of a written petition, conforming to Chapter 150B of the North
Carolina General Statutes, and filed with the Office of Administrative Hearings Post Office Drawer 27447,
Raleigh, North Carolina 27611-7447. Unless such demand is made, this decision shall be final and binding.
Please take notice that this certificate of coverage is not transferable. Part H, B.2, addresses the
requirements to be followed in case of change in ownership or control of this discharge.
This permit does not affect the legal requirements to obtain other permits which may be required by the
Division of Environmental Management or permits required by the Division of Land Resources, Coastal Area
Management Act or any other Federal or Local governmental permit that may be required.
