Mineral, and Land Resources (DEMLR) is forwarding herewith the reissued Certificate of
Coverage (COC). This permit is reissued pursuant to the requirements of North Carolina
General Statute 143-215.1 and the Memorandum of Agreement between the state of North
Carolina and the U.S. Environmental Protection Agency, dated October 15, 2007 (or as
subsequently amended.) A new Certificate of Coverage (COC) is included with this
letter.

print package on the website includes revised Discharge Monitoring Report (DMR) forms,
Annual Discharge Monitoring Report forms, and Qualitative Monitoring Report Forms.
The General Permit authorizes discharges of stormwater and wastewater (if applicable),
and it specifies your obligations with respect to discharge controls, management,
monitoring, and record keeping. Please review the new permit to familiarize yourself with
all changes in the reissued permit. Significant changes to the General Permit are
outlined in the Technical Bulletin, which is also available on the website above. Your
facility has six months from receipt of the permit to update your Stormwater Pollution
Prevention Plan (SPPP) to reflect any new permit requirements.

How does the new General Permit affect Tier Status?
The first sample period of your permit begins August 1, 2017, Tier 3 actions in Part IV of
your permit are triggered by benchmark exceedances on four occasions beginning on the
effective date of this permit and do not count prior exceedances. Likewise, Tier 2 actions
are triggered by two consecutive benchmark exceedances beginning on the effective date
of this permit and do not count prior exceedances. Howeverif your facility is already in
Tier 2 (monitoring stormwater discharges monthly) or Tier 3, you must continue -monthly
monitoring until relieved through conditions of the permit or by DEMLR staff approval.
When does electronic DMR reporting start?
We are setting up our database with final permit parameters and the outfalls that
permittees submitted on-line. All NCG14 Permittees will receive notification when our
eDMR system is ready for these permittees to register and begin reporting
monitoring data electronically. If you have any questions about the status, please
contact Bethany Georgoulias at (919) 807-6372 or Robert Patterson at (919) 807-6369. If
you did not submit outfall information already, we have included a form for you to return
to us. (Note: NO FORM IS ATTACHED if we already have your outfall information.)
Your coverage under the General Permit is transferable only through the specific action of
DEMLR. This permit does not affect the legal requirements to obtain other permits which
may be required by the N,C. Department of Environmental Quality (DEQ), nor does it
relieve the permittee from responsibility for compliance with any other applicable federal,
state, or local law, rule, standard, ordinance, order, judgment, or decree.

1. Location/Directions
Concrete Supply Co., LLC - Aberdeen Plant is located at 1250 Highway 5 West, in Aberdeen, NC,
Moore County. From the FRO, take Hwy 401 South toward Raeford. After you cross into Hoke
County, take a right at the stoplight at Wayside Road (PURE gas station is on the left, 13P on the
right). Go — 1 '/z miles until road ends, then turn left onto Plank Road (you will be on Ft. Bragg).
Drive — 13 miles until you get to Hwy 211. Turn right and go west on Hwy 211 for almost 7 miles.
Veer right onto South Street, and go - 1/2 mile until light at US Hwy 1. Continue forward onto NC
Hwy 5, go about 1 mile; entrance will be on right hand side.
2. Safety Considerations
All standard DAQ safety equipment is required. Be alert for moving overhead conveyors and vehicle
traffic.
3. Facility/Process Description
Concrete Supply Co., LLC - Aberdeen Plant is a small concrete batch plant that produces mixed
concrete for commercial use. Concrete is composed essentially of water, cement, sand (fine
aggregate), and coarse aggregate. Flyash is used as a specialty aggregate. This plant stores, conveys,
measures and discharges these constituents into trucks for mixing and transport to the job. Flyash and
cement are brought in by transfer truck and loaded pneumatically into silos for storage. Other
materials such as sand and gravel are stockpiled on the yard until needed. Cement is made when
delivery trucks. pull under the loading chute. Once the truck is in place under the chute, sand and
gravel are loaded into a bin on the yard and belt conveyed through the chute and into the truck. A
measured amount of cement and flyash from the storage silos is transferred into a weigh hopper
above the chute and dumped into the truck and mixed while in transport to the job site. Water is
added during this entire process.
The permit was for this facility was rescinded on 9 August 2016. Pam Vivian conducted the last
compliance inspection on 06 November 2014.

5. Inspection Conference
On 26 August, 2016, as part of a multi -media inspection, 1, Melissa Joyner of FRO (DEMLR), met
with Johnie Alexander, Environmental Manager and J.D. Dalrymaple, Plant Manager. We discussed
the following:
a) We verified the FACFINDER information. There are now 7 employees at the facility. At the
time of the previous inspection, on 10/24/2014, there were 9 employees.
b) Since the previous inspection, the permit has been rescinded for this facility on 09 August
2016. Information is recorded by the facility in logbooks regarding monthly inspections of
the baghouse and weekly inspections of all other equipment. The next annual inspection of
the baghouse will take place on 15 October 2016.
6. Inspection Summary
Mr. Alexander led me on a tour of the facility. No new emission sources had been installed. The
facility was in operation but a truck load out was not able to be observed, nor were the flyash and
cement silos being filled. The load out chute appeared to be in good condition. The areas near the
baghouse, cement and flyash silos also appeared clean. No fugitive dust emissions were noted in
regards to the entrance, haul roads or stockpiles.
7. Applicable Air Quality Regulations
a) 15A NCAC 2D .0515 -PARTICULATE CONTROL REQUIREMENT — Particulate matter
emissions from the emission's sources shall not exceed allowable rates.
Appeared to be in compliance — This facility was previously permitted, and the most recent
permit review indicated compliance so long as the facility operates and maintains the control
devices, as designed and recommended by the manufacturer, on all point sources at the facility.
There have not been any modifications to the facility since the facility's last permit review. All
control devices appeared to be operated and maintained as required.
b) 15A NCAC 2D .0521 VISIBLE EMISSIONS CONTROL REQUIREMENT - Visible emissions
from the facility's emission sources shall not be more than 20%.
APPEARED IN COMPLIANCE - The facility maintains logbooks regarding inspections and
maintenance of the facility's bagfilter, which they continue to perform monthly. The next
planned internal inspection is 15 October 2016. There were no indications the visible emissions
limit had been exceeded. Mr. Alexander stated that no complaints had been received about visible
emissions.
c) 15A NCAC 2D .0535 NOTIFICATION REQUIREMENT - The facility is required to notify
DAQ if excess emissions occur for more than a 4-hour period caused by a breakdown or other
abnormal condition.
APPEARED IN COMPLIANCE - There were no indications of excessive emissions which
would require notification of DAQ.
d) 15A NCAC 2D.0540 FUGITIVE DUST CONTROL REQUIREMENT — The facility shall not
cause complaints or excess visible emissions beyond the property line.
APPEARED IN COMPLIANCE -The facility has not received any direct complaints nor has
DAQ received complaints about fugitive dust beyond the property line. I saw no excess fugitive
dust emissions during the inspection.
8. 112R Status
The facility does not use or store compounds that require a written plan under Clean Air Act Section
112R, Risk Management.
9. Non -Compliance History since 2010
None has occurred at this facility.
10. Comments/Compliance Statement
The Concrete Supply Co., LLC Aberdeen facility appeared to be in compliance with all the
applicable Air Quality regulations on 23 August 2016.

Inspection Summary:
Melissa Joyner conducted an inspection on August 26, 2016, meeting with Johnie Alexander, Environmental Manager. His
time was very much appreciated. The Stormwater Pollution Prevention Pian was reviewed and contained in an orderly
fashion the required information as stipulated in the NCG140000 General Permit. This information had been annually
updated where applicable. The Anaytical and Qualitative monitoring results were also reviewed. No benchmark values had
been exceeded. Mr. Alexander led Ms. Joyner on a tour of the facility. The single outfall appeared to be welt maintained.
The stormwater infiltrates prior to discharge. There was no discharge from the outfall observed during the inspection and
there has not been any stormwater discharges for several years. There have also been no history of wastewater discharges
at the facility.

Thank you for submitting the permit renewal request (received on 06/08/2016 - 11:16am) for Permit COC No.
NCG140196. The NCG140000 General Permit has been reissued with no changes for one year, effective Friday, July 1,
2016. As we stated in our letter from May 31", we will not issue you a new Certificate of Coverage until next year,
when the General Permit is revised. However, the cover page of the permit has been revised to reflect new effective
dates.
For your records, you may print a copy of the new General Permit cover gage from our website (in the NPDES Industrial
SW section, General Permits). Or, you can go to this NCG14 Renewals Map and search for your permit. The "More info"
link next the General Permit field in the pop-up box has been revised with the new permit. You only need to print the
cover page that reflects the new dates for your records. Please continue to use the same data monitoring forms (DMR).
As a reminder, if there has been a company ownership change, or if the Owner Affiliation needs to be updated, you
must submit the appropriate signed form to DEMLR's Stormwater Program. Please contact us if you need one of those
forms or have any other questions.

If any parts, requirements, or limitations contained in this permit are unacceptable to you,

the entire permit. This hearing request must be in the form of a written petition, .conforming to G.S.
150B-23 of the North Carolina General Statutes, and filed with the Office of Administrative

provisions or issues for which the modification is sought. Please note that the permit will become
final and binding regardless of a request for informal modification unless a request for a hearing is
also made under G.S. 150B-23.
. Unless exempted by a condition of this permit or the regulations, construction of new
air pollution sources or air cleaning devices, or modifications to the sources or air cleaning
devices described in this permit must be covered under a permit issued by the Division of Air
Quality prior to construction. Failure to do so is a violation of G.S. 143-215.108 and may
subject the Permittee to civil or criminal penalties as described in G.S. 143-215.114A and 143-
215.11413.
This permit shall be effective from October 22, 2013 until February 28, 2017, is
nontransferable to future owners and operators, and shall be subject to the conditions and limitations
as specified therein.
Changes have been made to the permit stipulations. The Permittee is responsible for
carefully reading the entire permit and evaluating the requirements of each permit
stipulation. The Permittee shall comply with all terms, conditions, requirements, limitations
and restrictions set forth in this permit. Noncompliance with any permit condition is grounds

incorporated as part of this permit.
This permit is subject to the following specified conditions and limitations including any

A. SPECIFIC CONDITIONS AND LIMITATIONS
Any air emission sources or control devices authorized to construct and operate above must
be operated and maintained in accordance with the provisions contained herein. The
Permittee shall comply with applicable Environmental Management Commission
Regulations, including Title 15A North Carolina Administrative Code (NCAC), Subchapter
2D .0202, 2D .0515, 2D .0521, 2D .0535, 2D .0540 and 2D .0611.
2. PERMIT RENEWAL AND EMISSION INVENTORY REQUIREMENT - The Permittee,
at least 90 days prior to the expiration date of this permit, shall request permit renewal by
letter in accordance with 15A NCAC 2Q .0304(d) and (0, Pursuant to 15A NCAC 2Q
.0203(i), no permit application fee is required for renewal of an existing air permit (without
a modification request). The renewal request (with AA application form) should be
submitted to the Regional Supervisor, DAQ. Also, at least 90 days prior to the expiration
date of this permit, the Permittee shall submit the air pollution emission inventory report
(with Certification Sheet) in accordance with 15A NCAC 2D .0202, pursuant to N.C.
General Statute 143 215.65. The report shall be submitted to the Regional Supervisor, DAQ
and shall document air pollutants emitted for the 2015 calendar year.
PARTICULATE CONTROL REQUIREMENT - As required by 15A NCAC 2D .0515
"Particulates from Miscellaneous Industrial Processes," particulate matter emissions from
the emission sources shall not exceed allowable emission rates. The allowable emission rates
are, as defined in 15A NCAC 2D .0515, a function of the process weight rate and shall be
determined by the following equation(s), where P is the process throughput rate.in tons per
hour (tons/hr) and E is the allowable emission rate in pounds per hour (lbslhr).
E = 4.10 * (P) 0.67 for P <= 30 tons/hr, or
E = 55 * (P) "' - 40 for P >30 tonslhr
4. VISIBLE EMISSIONS CONTROL REQUIREMENT - As required by 15A NCAC 2D
.0521 "Control of Visible Emissions," visible emissions from the emission sources,
manufactured after July 1, 1971, shall not be more than 20 percent opacity when averaged
over a six -minute period, except that six -minute periods averaging not more than 87 percent
opacity may occur not more than once in any hour nor more than four times in any 24-hour
period. However, sources which must comply with 15A NCAC 2D .0524 "New Source
Performance Standards" or. 1110 'National Emission Standards for Hazardous Air
Pollutants" must comply with applicable visible emissions requirements contained therein.

5. NOTIFICATION REQUIREMENT -As required by 15A NCAC 2D .0535, the Permittee of
a source of excess emissions that last for more than four hours and that results from a
malfunction, a breakdown of process or control equipment or any other abnormal
conditions, shall:
a. Notify the Director or his designee of any such occurrence by 9.00 a.m. Eastern time
of the Division's next business day of becoming aware of the occurrence and
describe:
i. the name and location of the facility,
ii. the nature and cause of the malfunction or breakdown,
iii, the time when the malfunction or breakdown is first observed,
iv. the expected duration, and
v, an estimated rate of emissions.
b. Notify the Director or his designee immediately when the corrective measures have
been accomplished.
This reporting requirement does not allow the operation of the facility in excess of
Environmental Management Commission Regulations.
6. FUGITIVE DUST CONTROL REQUIREMENT - As required by 15A NCAC 2D .0540
"Particulates from Fugitive Dust Emission Sources," the Permittee shall not cause or allow
fugitive dust emissions to cause or contribute to substantive complaints or excess visible
emissions beyond the property boundary. If substantive complaints or excessive fugitive
dust emissions from the facility are observed beyond the property boundaries for six minutes
in any one hour (using Reference Method 22 in 40 CFR, Appendix A), the owner or
operator may be required to submit a fugitive dust plan as described in 2D .0540(f).
"Fugitive dust emissions" means particulate matter that does not pass through a process
stack or vent and that is generated within plant property boundaries from activities such as:
unloading and loading areas, process areas stockpiles, stock pile working, plant parking lots,
and plant roads (including access roads and haul roads).

7. FABRIC FILTER REQUIREMENTS including cartridge filters, baghouses, and other dry
filter particulate collection devices - As required by 15A NCAC 2D .0611, particulate matter
emissions shall be controlled as described in the permitted equipment list.
a. Inspection and Maintenance Requirements - To comply with the provisions of this
permit and ensure that emissions do not exceed the regulatory limits, the Permittee
shall perform, at a minimum, an annual (for each 12 month period following the
initial inspection) internal inspection of each bagfilter system. In addition, the
Permittee shall perform periodic inspections and maintenance as recommended by
the equipment manufacturer.
b. Recordkeeping Requirements - The results of all inspections and any variance from
manufacturer's recommendations or from those given in this permit (when
applicable) shall be investigated with corrections made and dates of actions recorded
in a logbook. Records of all maintenance activities shall be recorded in the logbook.
The logbook (in written or electropic format) shall be kept on -site and made
available to DAQ personnel upon request.
B. GENERAL CONDITIONS AND LIMITATIONS
1. TWO COPIES OF ALL DOCUMENTS, REPORTS, TEST DATA, MONITORING DATA,
NOTIFICATIONS REQUESTS FOR RENEWAL AND ANY OTHER INFORMATION
REQUIRED BY THIS PERMIT shall be submitted to the:

For identification purposes, each submittal should include the facility name as listed on the
permit, the facility identification number, and the permit number.
2. RECORDS RETENTION REQUIREMENT - Any records required by the conditions of this
permit shall be kept on site and made available to DAQ personnel for inspection upon
request. These records shall be maintained in a form suitable and readily available for
expeditious inspection and review. These records must be kept on site for a minimum of 2
years, unless another time period is otherwise specified.
ANNUAL FEE PAYMENT - Pursuant to 15A NCAC 2Q .0203(a), the Permittee shall pay
the annual permit fee within 30 days of being billed by the DAQ. Failure to pay the fee in a
timely manner will cause the DAQ to initiate action to revoke the permit.

4. EQUIPMENT RELOCATION - A new air permit shall be obtained by the Permittee prior to
establishing, building, erecting, using, or operating the emission sources or air cleaning
equipment at a site or location not specified in this permit,
5. REPORTING REQUIREMENT - Any of the following that would result in previously
unpermitted, new, or increased emissions must be reported to the Regional Supervisor,
DAQ:
a. changes in the information submitted in the application regarding facility emissions;
b, changes that modify equipment or processes of existing permitted facilities; or
c. changes in the quantity or quality of materials processed.
If appropriate, modifications to the permit may then be made by the DAQ to reflect any
necessary changes in the permit conditions. In no case are any new or increased emissions
allowed that will cause a violation of the emission limitations specified herein.
6. This permit is subject to revocation or modification by the DAQ upon a determination that
information contained in the application or presented in the support thereof is incorrect,
conditions under which this permit was granted have changed, or violations of conditions
contained in this permit have occurred. The facility shall be properly operated and
maintained at all times in a manner that will effect an overall reduction in air pollution.
Unless otherwise specified by this permit, no emission source may be operated without the
concurrent operation of its associated air cleaning device(s) and appurtenances.
7. This permit is nontransferable by the Permittee. Future owners and operators must obtain a
new air permit from the DAQ.
8. This issuance of this permit in no way absolves the Permittee of liability for any potential
civil penalties which may be assessed for violations of State law which have occurred prior
to the effective date of this permit.
9. This permit does not relieve the Permittee of the responsibility of complying with all
applicable requirements of any Federal, State, or Local water quality or land quality control
authority.
10. Reports on the operation and maintenance of the facility shall be submitted by the Permittee
to the Regional Supervisor, DAQ at such intervals and in such form and detail as may be
required by the DAQ. Information required in such reports may include, but is not limited
to, process weight rates, firing rates, hours of operation, and preventive maintenance
schedules.
11. A violation of any term or condition of this permit shall subject the Permittee to enforcement
pursuant to G.S. 143-215.114A, 143-215.114B, and 143-215.114C, including assessment of
civil and/or criminal penalties.

12. Pursuant to North Carolina General Statute 143-215,3(a)(2), no person shall refuse entry or
access to any authorized representative of the DAQ who requests entry or access for
purposes of inspection, and who presents appropriate credentials, nor shall any person
obstruct, hamper, or interfere with any such representative while in the process of carrying
out his official duties, Refusal of entry or access may constitute grounds for permit
revocation and assessment of civil penalties.
13. The Permittee must comply with any applicable Federal, State, or Local requirements
governing; the handling, disposal, or incineration of hazardous, solid, or medical wastes,
including the Resource Conservation and Recovery Act (RCRA) administered by the
Division of Waste Management,
14. PERMIT RETENTION REQUIREMENT - The Permittee shall retain a current copy of the
air permit at the site. The Permittee must make available to personnel of the DAQ, upon
request, the current copy of the air permit for the site.
15. CLEAN AIR ACT SECTION 112 r REQUIREMENTS - Pursuant to 15A NCAC 2D .2100
"Risk Management Program," if the Permittee is required to develop andregister a risk
management plan pursuant to Section 112(r) of the federal Clean Air Act, then the
Permittee is required to register this plan with the USEPA in accordance with 40 CFR Part
68.
16. PREVENTION OF ACCIDENTAL RELEASES - GENERAL DUTY - Pursuant to Title I
Part A Section 112(r)(1) of the Clean Air Act "Hazardous Air Pollutants - Prevention of
Accidental- Releases - Purpose and General Duty," although a risk management plan may not
be required, if the Permittee produces, processes, handles, or stores any amount of a listed
hazardous substance, the Permittee has a general duty to take such steps as are necessary to
prevent the accidental release of such substance and to minimize the consequences of any
release. This condition is federally -enforceable only.
17, GENERAL EMISSIONS TESTING AND REPORTING REQUIREMENTS - If emissions
testing is required by this permit, or the DAQ, or if the Permittee submits emissions testing
to the DAQ in support of a permit application or to demonstrate compliance, the Permittee
shall perform such testing in accordance with 15A NCAC 2D .2600 and follow all DAQ
procedures including protocol approval, regional notification, report submittal, and test
results approval.

Enclosed please find the Compliance Evaluation Inspection report for the inspection
conducted September 20, 2012 for the CEMEX facility located at 1250 Highway 5 West
Aberdeen. Mr. Jarren Dalrymple, Plant Manager is thanked for his time and participation
during the inspection.
The inspection and file review revealed that the NPDES Stormwater General Permit and
associated Certificate of Coverage authorizes discharges of wastewater and stormwater
to Pages Lake ( Aberdeen Lake), Class "B" Waters in the Lumber River Basin.
The following observations were noted during the inspection:
-The site was found to be neat and well maintained,
-Site reportedly has only one discharge point,
-There was no visible signs of a discharge from this site,
-Reportedly there has never been a discharge of wastewater from this facility,
-An inspection of the wastewater holding pond revealed no visible signs of discharge,
- Wastewater is managed as makeup water or the plant,
-Stormwater appears to infiltrate prior to discharge. Stormwater discharges from this site
are reportedly "rare",
-It is suggested that new pH buffers are obtained and managed as labeled,
-All records were on hand and VERY neat and organized. GREAT JOB!
-Employee training includes videos, class work and testing. Very Good!

forwarding herewith the subject certificate of coverage to discharge under the subject state - NPDES general
permit. This permit is issued pursuant to the requirements of North Carolina General Statute 143-215.1 and the
Memorandum of Agreement between North Carolina and the US Environmental Protection agency dated
December 6, 1983.
The stormwater permit you are receiving, NCG140000, does not permit wasting concrete, dumping excess
concrete, directly into storm sewer outfalls or into waters of the state. Any facility which either dumps excess
concrete or washes excess concrete into storm sewers or waters of the state will be operating in direct violation to
both the terms of this permit and the North Carolina General Statutes. Such a discharge shall be considered an
illegal discharge and may subject the owner to enforcement actions in accordance with North Carolina General
Statutes 143-215.6A
If any parts, measurement frequencies or sampling requirements contained in this permit are unacceptable to
you, you have the right to request an individual permit by submitting an individual permit application. Unless
such demand is made, this certificate of coverage shall be final and binding.
Please take notice that this certificate of coverage is not transferable except after notice to the Division of
Environmental Management. The Division of Environmental Management may require modification or revocation
and reissuance of the certificate of coverage.
