Dear Mr. Allen:
On March 25, 2019, a site inspection was conducted for the Concrete Service - Dunn facility located at 401 North
Fayetteville Avenue, Dunn, Harnett County, North Carolina. A copy of the Compliance Inspection Report is enclosed for
your review. Mr. Ricky Parker, Plant Manager, and Mr. Donald Blue, Operations Manager, were also present during the
inspection and their time and assistance is greatly appreciated. The site visit and file review revealed that the subject
facility is covered by NPDES General Permit NCG140000 under Certificate of Coverage NCG140037. Permit coverage
authorizes the discharge of stormwater and process wastewater from the facility to receiving waters designated as a UT
(unnamed tributary) of Stony Run, a Class C;Sw waterbody in the Cape Fear River Basin.
As a result of the inspection, the facility was found to be in compliance with the conditions of the NCG140000 permit.
Please refer to the enclosed Compliance Inspection Report for additional comments and observations made during the
inspection.
Please be advised that violations of the NPDES General Permit are subject to a civil penalty assessment of up to

Inspection Summary:
Inspection conducted in response to a complaint received on March 22, 2019 made by staff with the City of Dunn concerning
possible illicit discharges coming off the site onto city street. Met with Ricky Parker, Plant Manager, and Donald Blue,
Operations Manager, vvho relayed that the facility recently completed a concrete resurfacing project on a portion of the site
and that the area was washed off, however the water was directed into the facility's settling basin (reclaimer pit) through
concrete swales that were installed as part of the resurfacing project. There was no water in the street at the time of
inspection. Mr. Parker and Mr. Blue expressed that another portion of the site would be resurfaced in the near future. Both
were advised to ensure that any wash water, rinse water, process water, etc, is directed into the settling basin prior to any
discharges as authorized under the NCG140000 permit. Reviewed facility's Stormwater Pollution Prevention Plan (SPPP)
and monitoring records under the current permit cycle. The SPPP appeared to contain all permit -required components.
Completion of associated documents related to the required annual review and update (spills, employee training, etc.) was
discussed. Monitoring records for 2018 when facility ownership changed indicate no discharges. Site observations were
made including the settling basin, admixture storage with secondary containment as well as an active project regarding a
clogged storm drain on King Avenue and cleaning out of the associated ditch. A recommendation was made to coordinate
this project with the City of Dunn.

MEMORANDUM OF LEASE
THIS MEMORANDUM OF LEASE ("Memorandum") is signed as of December 15,

Now does the new General Permit affect Tier Status?
The first sample period of your permit begins August 1, 2017. Tier 3 actions in Part IV of
your permit are triggered by benchmark exceedances on four occasions beginning on the
effective date of this permit and do not count prior exceedances. Likewise, Tier 2 actions
are triggered by two consecutive benchmark exceedances beginning on the effective date
of this permit and do not count prior exceedances. Holyever if your facility is already in
Tier 2 (monitoring stormwater discharges monthly) or Tier 3, you must continue monthly
monitoring until relieved through conditions of the permit or by DEMLR staff approval.
When does electronic DMR reporting start?
We are setting up our database with final permit parameters and the outfalls that
permittees submitted on-line. Ali NCG14 Permittees will receive notification when our
eDMR system is ready for these permittees to register and begin reporting

3) Facility/Process Description:
Argos Ready Mix - Dunn Plant is a truck mix concrete batch plant. The concrete batch plant consists of a
cement silo, a flyash silo, aggregate storage and handling system, three aggregate supply bins, an
aggregate weigh -hopper, an aggregate mixing conveyor, a flyash/cement weigh -hopper, and a truck load -
out operation. The silos, flyash/cement weigh -hopper and truck loadout are all controlled by a central
bagfiiter Fine (sand) and coarse (gravel) aggregates are front-end loaded into a pit, belt conveyed to one
of three storage bins, the sand and aggr weigh hopper weighs out material, and it's transported by belt
conveyor to the truck mix loadout. Cement and fly ash are stored in separate silos, gravity fed into the
flyash/cement weigh hopper, and added to the truck mix. Water is added throughout this entire process.
A plastic shroud curtain surrounds this area and a rubber boot lowers into the truck during a mix.

S) Inspection Summary:
On 18 March 2016, 1, Heather Carter with DAQ FRO, arrived at the facility to conduct a multimedia
compliance inspection along with Mike Lawyer, Melissa Joyner, Andrew Hammonds, Tony Honeycutt,
and Mark Brantley all of FRO DEQ. We met with Ricky Parker, Plant Manager, and Dewey Jernigan,
Operations Manager, who graciously agreed to host our group for this training inspection. Mr. Jernigan
reviewed the current facility contact information and made no changes. Mr. Parker provided the bagfilter
1/M log and the facility's current air permit. He stated no complaints have been received from the
surrounding area and there have been no excess emissions. Facility personnel perform daily visual checks
of the sources and control device and record the results (including the magnehelic gauge readings) in the
logbook. Mr. Parker and Mr. Jernigan escorted us on the tour of the facility.
We observed several trucks being loaded out and sand/aggregate bin loading, both with Oho VE. There
were no fugitive emissions observed leaving the site and no odors were detected off -site. The small NG-
fired boiler (not subject to permitting, NSPS or NESHAP) for heating hot water for concrete batches
during the winter months was on site but not operating. A 5 mph speed limit sign is posted and enforced
by Mr. Parker to prevent fugitive emissions. And a sign is posted to limit silo filling to a pressure Iess
than 12psig to prevent over -pressurizing the system and exceeding opacity limit.

6) Permit Stipulations:
A.2 2D_.0202 -"_Permit Renewal and Emission Inventory Requirement" - Facility is required to submit
the permit renewal application and mission inventory 90 days prior to permit expiration.
APPEARED IN COMPLIANCE - The most recent permit renewal and El was due 1 January 2016 (due
to the I" being a holiday and weekend following, the due date was really 1/4/16). The permit renewal
and EI were postmarked on 1/4/16.
A,3.2D .0515 "Particulates from Miscellaneous Industrial Processes " Particulate matter emissions
from the emission sources shall not exceed allowable emission rates.
APPEARED IN COMPLIANCE - The last permit review indicated compliance based on the sources
being operated as permitted. There have been no changes since the last review.
A.4.21) .0521, "Control of Visible Emissions," Visible emissions shall not be more than 20 percent
opacity when averaged over a six -minute period.
APPEARED IN COMPLIANCE - All emission points were observed operating during this inspection
with 0% opacity. Mr. Parker stated he has not observed any excess emission releases while operating.
A.5 2D .0535 "Notification Requirement," Facility is required to notify the DAQ of excess emissions
that last for more than four hours.
APPERARED IN COMPLIANCE - Mr. Parker stated there have been no periods of excess emissions.
A.6. 2D .0540 "Particulates from Pugjtive Dust Emission Sources," Fugitive dust emissions from the
plant shall not cause or contribute to substantive complaints or excess visible emissions beyond the
property boundary.
APPEARED IN COMPLIANCE - There was no indication of dust accumulations on the property. The
facility grounds are mostly paved. The aggregate stock piles are sprayed with water as needed to suppress
fugitive emissions. Mr. Parkef stated that no complaints have been received at the facility.
A.7.2D .0611 "Fabric Filter_ Requirements," Particulate matter emissions from truck nix loadout and
silos shall be controlled by a fabric filter. Follow manufacturer's I&M, with minimum annual internal
inspection, and record all I&M activities in logbook.
APPEARED IN COMPLIANCE - Logbook indicated periodic maintenance activities and daily visual
checks. Filter Clean Environmental Services performed the most recent annual inspection on 17
November 2015 and changed all the bags,

In response to your renewal application for continued coverage under stormwater General Permit NCG140000
the Division of Water Quality (DWQ) is forwarding herewith the reissued General Permit. This permit is reissued
pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement
between the state of North Carolina and the U.S. Environmental Protection Agency, dated October 15, 2007 (or
as subsequently amended).
The following information is included with your permit package:
• A new Certificate of Coverage
• A copy of General Permit NCG140000
• A copy of the Technical Bulletin for the General Permit
• Two copies of the Discharge Monitoring Report (DMR) Form for Stormwater
• Two copies of the DMR Form for Wastewater
• Two copies of the Qualitative Monitoring Report Form
The General Permit authorizes discharges of stormwater and/or wastewater, and it specifies your obligations with
respect to discharge controls, management, monitoring, and record keeping. Please review the new permit to
familiarize yourself with all the changes in the reissued permit. If your Certificate of Coverage (COC) restricts
authorization to discharge stormwater only (or other limited terms), you must contact the Stormwater Permitting
Unit if you would like to request a modification to your COC.
The first sample period of your permit begins July 1, 2011, However, because of the delayed mail -out of permit
packages, your facility is expected to attempt to sample during the first sampling period, and must comply with
sampling requirements beginning during the second sampling period. Your facility must sample a representative
storm event between January 1 and July 30, 2012 (for stormwater sampling), and/or sample wastewater between
October 1, 2011 and December 31, 2011 (or, if applicable, report "No Flow", as outlined in Part V, Section E).
Additionally, your facility has 6 months from the time of receipt of the permit to update SPPP to current standards.
The more significant changes in the General Permit since your last Certificate of Coverage include the following
(please note the names and numbers of sections have been changed from the previous permit revision):
Part II:
• Authorization to Construct (ATC) language was added to the permit and adapted to fit NCG140000.
Part Ili:
• The Stormwater Pollution Prevention Plan (SPPP) section has been updated to the most current
language of our permits. Some additional conditions specific to this industry have been added to the
SPPP language.
Part IV:
• Sections A & B: Revised provisions require stormwater discharges be sampled twice per year (instead of
annually), and sampling results shall be compared to new benchmark values. (The previous cut-off
concentrations have been removed).

• Sections A & B: Revised provisions require the permittee to execute Tier One, Two & Three response
actions: Tier One requirements for a first benchmark exceedence, and/or Tier Two response for a second
consecutive benchmark exceedence, if applicable. Tier Two requires that the permittee institute monthly
monitoring (reported to the appropriate Regional Offices) instead of twice -per -year monitoring. Monthly
monitoring shall be done until three consecutive monitoring events show no benchmark exceedences.
• Sections A & B: This permit now contains a provision that four exceedences of any benchmark will trigger
increased DWQ involvement in the permittee's stormwater management and control actions (Tier Three).
DWQ may direct the permittee to apply for an individual permit, institute specific monitoring, or may direct
the implementation or installation of specific stormwater control measures.
• Sections A & B: Failure to perform analytical stormwater monitoring requires that the permittee begin a
monthly sampling scheme for both industrial stormwater discharges and/or stormwater discharges from
VMA areas (where applicable). After 6 months of monthly sampling, the permittee may return to semi-
annual sampling, unless otherwise specified by DWQ.
• Sections A & 8: A lower TSS benchmark of 50 mgll for HOW, ORW, PNA and Tr Waters was added to
this permit for these more sensitive waters.
• Section B: The monitoring parameter Oil & Grease has been replaced by the parameter Total Petroleum
Hydrocarbons.
• Sections A, 8 & C: Inability to sample due to adverse weather must be reported in the SPPP.
• Sections A, B & C: The terminology "Representative Storm Event" has been replaced by "Measurable
Storm Event" in response to comments from industry groups. A measurable storm event is a storm event
that results in an actual discharge from the permitted site outfall. The previous measurable storm event
must have been at least 72 hours prior, unless the permittee is able to document that a shorter interval is
representative for local storm events during the sampling period, and obtains written approval from the
local DWQ Regional Office.
• Section C: Qualitative monitoring must now be conducted semi-annually and is tied to the same

New elements of the Stormwater Pollution Prevention Plan for this permit renewal shall be developed and
implemented within 6 months of the effective date of the initial Certificate of Coverage issued pursuant to
this General Permit and updated thereafter on an annual basis.
Your coverage under the General Permit is transferable only through the specific action of DWQ. This permit does
not affect the legal requirements to obtain other permits which may be required by DENR, nor does it relieve the
permittee from responsibility for compliance with any other applicable federal, state, or local law, rule, standard,

inspection of the subject facility. The following items should be addressed to prevent problems in the future:
Your office should take steps to correct the fact that Oil & Grease monitoring has not been done. Also, please
evaluate wastewater treatment facilities for "not" being properly maintained in that monitoring data shows pH levels have
continued to increase over the last several years. In addition; your office_ is required to submit all the signed analytical
monitoring data forms to DWO's Stormwater Unit, Raleigh, NC.
Within 14 days of receipt of this notice, we ask that you notify this office in writing of what corrective actions have been
taken to address the above noted items. Please be advised that this letter does not preclude the Division of Water Quality from
taking enforcement actions for any past or future violations.
