I. DIRECTIONS TO SITE: From FRO, take Highway 87 south to Elizabethtown, stay on bypass. After
crossing Highway 701, take the first left onto Mercer Mill Road. The facility is on the left about %4 mile.

U. SAFETY: Standard FRO safety gear is required, including a reflective vest. The inspector should be
mindful of vehicular and heavy equipment traffic.
III. FACII.ITY/PROCESS DESCRIPTION: S&W Ready Mix Concrete Company, Inc. — Eli;%bethtown
Plant is a small, 100 cu yd/hr, truck mix concrete batch plant. The facility consists of two silos containing
cement and flyash, sand and aggregate storage bins, storage bins/weigh hoppers for sand and aggregate,
and a truck loadout area. The silo emissions are controlled via bagfilters, and all other emission sources
are uncontrolled.
Aggregate and sand are conveyed from large stockpiles behind the plant to the elevated storage bins from
which they are dispensed as needed. The elevated bins rely on gravity or screw for moving products to
truck mix weigh hopper, feeding the proper amounts of each component to the truck. An internal
computer system controls each mix. Fugitive dust emissions from the facility's stockpiles are controlled
using sprinkler heads for dust suppression, and there are 5 MPH speed limits posted to minirruze dust
generated by truck traffic.

IV. INSPECTION SUMMARY: On 22 September 2017, Mark Brantley, FRO DWR, and .1, Joshua Harris
FRO DAQ, arrived on site to conduct a multimedia inspection. We met with Hank Gay, Environmental r•z
Manager, and Allen Britt, Plant Manager. The facility was not operating at the time of the inspection.';.
Mr. Gay reviewed the FacFinder data, and did not note any changes to the facility's contact information. z
We then reviewed the facility's records, which were well kept and up-to-date. Mr. Gay quickly produced
a copy of the facility's permit, and I&M records for the facility's bagfilter. The records were complete
and up-to-date, and showed that the facility inspects and maintains the bagfilter on a monthly schedule,
which is more stringent than that required by the permit. Mr. Britt stated that he's never had any visible
emissions from the bagfilters, and that there haven't been any complaints from local residents. After
reviewing the facility's air and stormwater records, we toured the plant, and did not note any concerns.

VI. APPLICABLE AIR QUALITY REGULATIONS: .
A. 15A NCAC 2D .0202 — PERMIT RENEWAL AND EMISSION INVENTORY REQUIREMENT
The facility is required to subunit an application for permit renewal no later than 90 days prior to
expiration, and shall include documentation of air pollutants emitted for the 2024 calendar year.
APPEARED IN COMPLIANCE: The facility's permit was recently renewed, and Mr. Gay is aware
of the requirement. The previous El and permit application was received on time.
B. 15A NCAC 2D .0515 —PARTICULATES FROM MISC INDUSTRIAL PROCESSES —
Particulate emissions shall not exceed allowable emission rates as calculated by the following
equations:

APPEARED IN COMPLLANCE: The permit review indicates compliance with these emission rates
when the facility is operated in accordance with the permit. There have not been any modifications to
the facility, permitted control devices are installed and operated, and all I M is being performed as
required.
C. 15A NCAC 2D .0521— CONTROL OF VISIBLE EMISSIONS —Visible emissions shall not
exceed 20%.
APPEARED IN COMPLIANCE: The facility was not operating during the inspection. Mr. Britt
stated that he has never seen visible emissions from the fabric filters, No complaints have been
received by the facility or by DAQ.
D. 15A NCAC 2D .0535 —NOTIFICATION REQUIREMENT —Notify DAQ in the event of excess
emissions lasting longer than 4 hours resulting from malfunctions, breakdowns, or abnormal
conditions.
APPEARED IN COMPLIANCE: There are no indications of, and neither Mr. Gay nor Mr. Britt
were not aware of, any excess emissions which would require a notification by the facility. There
have been no complaints made by local residents.
E. 15A NCAC 2D .0540 — PARTICULATES FROM FUGITIVE DUST EMISSION SOURCES —
Fugitive dust emissions shall not cause or contribute to substantive complaints or excess visible
emissions beyond property boundaries.
APPEARED IN COMPLLANCE: No fugitive dust concerns at the property boundaries were noted
during the inspection. Speed limits are posted, and wet suppression is used on the stockpiles. Mr.
Britt stated that emissions from the loadout are brief, and the dust might only cross the property
boundary if the wind is blowing very hard. There have been no complaints made by local residents.

F. 15A NCAC 2D .0611 —FABRIC FILTER REQUIREMENTS — PM emissions controlled by fabric
filters; annual inspections; record keeping.
APPEARED IN COMPLIANCE: The facility conducts regular maintenance and internal
inspections on the bagfilters on a monthly basis. Any maintenance required at the time of each
inspection is completed and recorded.
VII. INSIGNIFICANT ACTIVITIES:
VM. NON-COMPLIANCE HISTORY SINCE 2010: This facility has no negative compliance history since
2010.
IX. RISK MANAGEMENT (112r): This facility does not store any 112(r) subject materials above threshold
limits. Therefore, it is not required to maintain a written Risk Management Plan (RMP).
X. CONCLUSION AND RECOMMENDATIONS:

COMPLIANCE with their air quality permit on the date of inspection.

How does the new General Permit affect Tier Status?
The first sample period of your permit begins August 1, 2017. Tier 3 actions in Part 1V of
your permit are triggered by benchmark exceedances on four occasions beginning on the
effective date of this permit and do not count prior exceedances. Likewise, Tier 2 actions
are triggered by two consecutive benchmark exceedances beginning on the effective date
of this permit and do not count prior exceedances. However, if your facility is already in
Tier 2 (monitoring stormwater discharges monthly) or Tier 3, you must continue monthly
monitoring until relieved through conditions of the permit or by DEMLR staff approval.
When does electronic DMR reporting start?
We are setting up our database with final permit parameters and the outfalls that
permittees submitted on-line. All NCG14 Permittees will receive notification when our
eDMR system is ready for these permittees to register and begin reporting
monitoring data electronically. If you have any questions about the status, please

you did not submit outfall information already, we have included a form for you to return

Your coverage under the General Permit is transferable only through the specilic action of

On March 2, 2007, I, Michael Lawyer from the Fayetteville Regional Office of the Division of
Water Quality, conducted a site inspection for the S & W Ready Mix Concrete-Bladen facility located at
1460 Mercer Mill Road in Bladen County, North Carolina. A copy of the inspection report is attached
for your review. Stormwater from the facility drains to an unnamed tributary to Browns Creek, a Class
C stream located in the Cape Fear River Basin. The site visit and file review revealed that the subject
facility is covered by NPDES Stormwater General Pernit-NCG140023.
Accordingly, the following observations were noted during the Division of Water Quality
inspection:

Other Observations:
Some of the qualitative monitoring data was outside the required time frame ol'April through June. The
facility's one outfall receives overflow from the recycle system as well as runoff from stockpile wetting,
however analytical monitoring is only performed on discharges from the recycle system. Discharges
that occur as a result of runoff from stockpile wetting should be monitored separately. Also, an annual
monitoring event should occur for discharges consisting solely of stormwater.
Please be advised that violations of the NPDES Storniwater General Permit are subject to a
civil penalty assessment of up to $25,000 per day for each violation. I1' you or your staff has any

and a recycle system that receives wash water from truck cleaning. There is one outfall, which receives overflow from the
recycle system, runoff from wetting of the stockpile area as well as stormwater, Analytical and qualitative monitoring data
was available. Some of the qualitative monitoring data was outside the required time frames and sampling for the
analytical monitoring was conducted only for recycle system overflow. Separate monitoring should be conducted for
