Inspection Summary:
Safety-Kleen is located at 2320 Yadkin Avenue on approximately 1.47 acres. The facility has three buildings. Safety-Kleen
is an environmental services company. The Yadkin Avenue facility's primary business is distribution and collection of
solvents from parts washers. The facility also collects a variety of other waste materials. The facility is permitted as a
Treatment, Storage, and Disposal Facility (TSDF), although waste material is currently kept
on -site for a maximum of ten days.
Overall, the facility had good housekeeping practices. One deficiency was observed for contaminated gravel due to a
leaking truck radiator. Several other recommendations and best management practices are included in the report. The
facility had several components missing from its Stormwater Pollution Prevention Plan (SPPP). Recommendations for
compliance with NPDES stormwater regulations are provided.

Stormwater Pollution Prevention Plan
Comment: No spills in the .past 3 years.

Stormwater collected in the concrete containment dike for the ASTs should be evaluated for
signs of pollution and documented before being released. Records should be maintained in the
SPPP for a minimum of five years. The dike should have a lockable manual release valve .
The spill response plan is kept is a separate binder from the SPPP. CM_S_WS recommends
including the sections that pertain to stormwater in the same location as the SPPP.
Training has not been documented. CMSWS recommends adding a training record to the annual
SPPP update.
CMSWS recommends including a current list of responsible parties in the binder with the SPPP.
The SPPP was most recently updated on 12/14/2015.
No inspection was recorded for the first half of 2016. Two facility inspections should be
documented every year „with at least 60 days in between inspections.

Enclosed is a copy of the Compliance Evaluation Inspection (CEI) report for the inspection
conducted at the subject facility on March 10, 2009, by Jeff Mitchell of Charlotte -Mecklenburg Storm
Water Services. This inspection was conducted as part of a cooperative working agreement between
Mecklenburg County and the Division of Water Quality. in addition, the inspection was conducted
on behalf of the City of Charlotte in compliance with the City's NPDES Permit, NCS000240, Part 11
Section H. Thank you for your assistance and cooperation during the inspection.
Overall, the facility's Stormwater program was well developed and implemented and this
Office commends the permittee's efforts to ensure compliance with your NPDES permit.

Inspection Summary:
Facility Description:
Safety-Kieen Systems, Inc. operations consist of collecting and processing used fluids from customers that are primarily
engaged in industrial maintenance and auto repair. The facility consists of a warehouse, offices, a return fill station for
emptying containers with additional storage area, and three 14,000-gallon aboveground storage tanks (ASTs) with
appropriate secondary containment.
Compliance History:
On March 27, 2007, Charlotte -Mecklenburg Storm Water Services (CMSWS) conducted an inspection of the
permittee/facility. The result of the inspection found the permittee needed to move a hose with residue to an area not
exposed to stormwater in order to comply with the City of Charlotte Stormwater Ordinance and good housekeeping
recommendations.
Industrial Operations/City of Charlotte Stormwater Ordinance Review:
The facility was well maintained at the time of the inspection; thus the inspector has no recommendations for the
permittee.

This annual fee is required by the North Carolina Administrative Code. It covers the administrative costs associated with your
permit. It is required of any person holding a permit for any time during the annual fee period, regardless of the facility's operating
status. Failure to pay the fee by the due date will subject the permit to revocation. Operating without a valid permit is a violation
and is subject to a $10,000 per day fine. If the permit is revoked and you later decide a permit is needed, you must reapply, with the

Your facility is currently covered for stormwater discharge under General Permit NCG 130000. This permit expires
on March 31, 2003. The Division staff is currently in the process of rewriting this permit and is scheduled to have
the permit reissued by early spring of 2003. Once the permit is reissued, your facility would be eligible for
continued coverage under the reissued permit.
In order to assure your continued coverage under the general permit, you must apply to the Division of Water
Quality (DWQ) for renewal of your permit coverage. To make this renewal process easier, we are informing you in
advance that your permit coverage will be expiring. Enclosed you will find a Permit Coverage Renewal
Application Form. The application must be completed and returned by October 2, 2002 in order to assure
continued coverage under the general permit. Due to staff and budget constraints, letters confirming our receipt of
the completed application will not be sent.
Failure to request renewal within the time period specified, may result in a civil assessment of at least $250.00.
Larger penalties may be assessed depending on the delinquency of the request. Discharge of stormwater from your
facility without coverage under a valid stormwater NPDES permit would constitute a violation of NCGS 143-215.1
and could result in assessments of civil penalties of up to $10,000 per day.
Please note that recent federal legislation has extended the "no exposure exclusion" to all operators of industrial
facilities in any of the I I categories of "storm water discharges associated with industrial activity," (except
construction activities). if you feel your facility can certify a condition of "no exposure", i.e. the facility industrial
materials and operations are not exposed to stormwater, you can apply for the no exposure exclusion. For additional
information contact the Central Office Stormwater Staff member listed below or check the Stormwater & General
Permits Unit Web Site at http:llh2o.enr.state.nc.us/su/stormwater.html
If the subject stormwater discharge to waters of the state has been terminated, please complete the enclosed
Rescission Request Form. Mailing instructions are listed on the bottom of the form. You will be notified when the
rescission process has been completed.

Dear Permittee:
Payment of the required annual administering and compliance monitoring fee of $80.00 for this year has not
been received for the subject permit. This fee is required by Title 15 North Carolina Administrative Code 2H.0105, under
the authority of North Carolina General Statutes 143-215.3(a)(1), (1a) and (1b). Because this fee was not fully paid within
30 days after being billed, this letter initiates action to revoke the subject permit, pursuant to 15 ncac 2H.0105(b) (2) (k)
(4), and G.S. 143-215.1 (b) (3).
Effective 60 days from receipt of this notice, subject permit is hereby revoked unless the required Annual
Administering and Compliance Monitoring Fee is received within that time. Discharges without a permit are subject to the

In accordance with your application for discharge permit received on April 1, 1996, we are forwarding
herewith the subject certificate of coverage to discharge under the subject state - NPDES general permit. This
permit is issued pursuant to the requirements of North Carolina General Statute 143-215 .1 and the
Memorandum of Agreement between North Carolina and the US Environmental Protection agency dated
December 6, 1983.
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to request an individual permit by submitting an individual permit
application. Unless such demand is made, this certificate of coverage shall be final and binding.
Please take notice that this certificate of coverage is not transferable except after notice to the Division
