Dear Mr. Blake:
On June 4, 2018, a site inspection was conducted for the Safety-Kleen Systems, Inc., Saint Pauls facility
located at 934 N. 51h Street, in Robeson County, North Carolina. A copy of the Compliance Inspection Report
is enclosed for your review. Mr. Jason Hoffman, General Manager was also present during the inspection
and his time and assistance is greatly appreciated. The site visit and file review revealed that the subject
facility is covered by NPDES Stormwater General Permit NCG130000 under Certificate of Coverage
NCG130024. Permit coverage authorizes the discharge of stormwater from the facility to receiving waters
designated as Mercer Branch, a Class C SW waterbody in the Lumber River Basin.
As a result of the inspection, the facility was found be in compliance with the conditions of the NCG130000
permit. Please refer to the enclosed Compliance Inspection Report for additional comments and
observations made during the inspection.

Inspection Summary:
Melissa Joyner met with Jason Hoffman, General Manager and spoke via phone with Todd Blake, Sr. Environmental
Compliance Manager at the Safety-Kleen Systems, Inc. facility, in St. Pauls, NC. The Stormwater Pollution Prevention Plan
(SPPP) was reviewed. It appeared well -organized and contained the information required by General Permit NCG130000.
The Site Map was detailed but should indicate impervious areas and the percentage of each drainage area that is
impervious. There should be a separate statement in the SPPP about there not having been any spills during the previous
three years. This is not shown in the SPPP For 2015-2017. The Qualitative Monitoring forms were reviewed. In 2018, separate
monitoring Forms were used for Outfalls #1 and #2 but they were combined on the same monitoring form for previous years.
Please continue using separate monitoring forms for these outfalls. Outfalls #1 and #2 were inspected and appeared to be in
good condition.

This COC is issued pursuant to the requirements of North Carolina General Statute 143-215.1 and the
memorandum of Agreement between the state of North Carolina and the U.S. Environmental Protection
Agency, dated October 15, 2007 (or as subsequently amended.)
The General Permit authorizes discharges of stormwater and specifies your obligations for discharge controls,
management, monitoring, reporting, and record keeping. Please review the new permit to familiarize yourself
with all of the changes. Parts III and IV contain the Standard Conditions, including Compliance and Liability,
Reporting, Monitoring and Records requirements; Operation and Maintenance obligations; and Definitions.
Your facility has six months from receipt of the COC to update its Stormwater Pollution Prevention Plan
(SPPP) to comply with changes in SPPP requirements. Other changes are effective immediately. Please note
that Tier 3 Actions (if applicable) are triggered by four benchmark exceedances beginning on the effective
date of the renewal permit and do not count prior exceedances. Please visit our website above to review the
new General Permit carefully.
Your coverage under the General Permit is transferable only through the specific action of DWQ. This permit
does not affect the legal requirements to obtain other permits which may be required by DENR, nor does it
relieve the permittee from responsibility for compliance with any other applicable federal, state, or local law,

As a follow-up to my initial notification this morning regarding the used oil spill at our St. Pauls facility, please see the
attached discharge notification report form that is required to be submitted for a non -UST related spill. I wasn't exactly
sure to whom it should be submitted too, so I'm including it in response to your request for additional information about
the spill and subsequent clean-up. I can tell you that we removed all visually impacted soils and containerized the
material into metal drums. The storm water outfall/swale was dry at the time of spill so there were no impacts to
surface waters. We are still in the process of investigating and evaluating the root cause of the release and I would be
glad to provide additional follow-up once we complete our internal investigation. If you know who else needs to receive
this completed report, please forward to them. Otherwise, forward the name of the person to me and I will be glad to
do so. If you have any additional questions concerning the spill please let me know.

On February 11, 2009, 1, Trent Allen from the Fayetteville Regional Office of the Division of Water
Quality, conducted a site inspection at the Safety Kleen Corp -Robeson facility located off hwy. 301, Robeson
County, North Carolina. A copy of the Compliance Inspection Report is enclosed for your review. James Willis
was also present during the inspection and his time and assistance is greatly appreciated. Stormwater from this
facility drains to Mercer Branch, a Class C. SW stream located in the Lumber River Basin, The site visit and file
review revealed that the subject facility is covered by NPDES Stormwater General Permit-NCG 13 0024.
Accordingly, the following observations were noted during the Division of Water Quality inspection

Your facility is currently covered for stormwater discharge under General Permit NCG130000. This permit expires
on March 31, 2003. The Division staff is currently in the process of rewriting this permit and is scheduled to have
the permit reissued by early spring of 2003. Once the permit is reissued, your facility would be eligible for
continued coverage under the reissued permit.
In order to assure your continued coverage under the general permit, you must apply to the Division of Water
Quality (DWQ) for renewal of your permit coverage. To make this renewal process easier, we are informing you in
advance that your permit coverage will be expiring. Enclosed you will find a Permit Coverage Renewal
Application Form. The application must be completed and returned by October 2, 2002 in order to assure
continued coverage under the general permit. Due to staff and budget constraints, letters confirming our receipt of
the completed application will not be sent.
Failure to request renewal within the time period specified, may result in a civil assessment of at least $250.00.
Larger penalties may be assessed depending on the delinquency of the request. Discharge of stormwater from your
facility without coverage under a valid stormwater NPDES permit would constitute a violation of NCGS 143-215.1
and could result in assessments of civil penalties of up to $10,000 per day.
Please note that recent federal legislation has extended the "no exposure exclusion" to all operators of industrial
facilities in any of the 11 categories of "storm water discharges associated with industrial activity," (except
construction activities). If you feel your facility can certify a condition of "no exposure", i.e. the facility industrial
materials and operations are not exposed to stormwater, you can apply for the no exposure exclusion. For additional
information contact the Central Office Stormwater Staff member listed below or check the Stormwater & General
Permits Unit Web Site at http://h2o.enr.state.nc.us/sulstormwater.htmi
If the subject stormwater discharge to waters of the state has been terminated, please complete the enclosed
Rescission Request Form. Mailing instructions are listed on the bottom of the form. You will be notified when the
rescission process has been completed.
