• Turbidity limits on effluent discharges have been removed --however turbidity standards still apply in the receiving
waters.
• Exceedences or violations must be sent to the Regional Office within 30 days of receipt from lab. .
• Failure to monitor according to permit terms triggers monthly monitoring for the remainder of the permit.
Changes Specific to Stormwater .
• Benchmark exceedences shall be reported to the Regional Office.
Benchmarks are not limits.
• Stormwater discharges shall be monitored from Vehicle Maintenance Areas
for Total Petroleum Hydrocarbons (TPH) rather than Oil & Grease (0&G).
• Stormwater discharges shall be monitored from basins designed to contain
the 10-year storm.
• The SBMP plan has been replaced by a specific mining SPPP.
• Stormwater Analytical Monitoring has been -increased to twice per year.
• When an analytical benchmark is exceeded, a Tiered Response applies. Cut
-off concentrations have been removed.
• Qualitative monitoring must be recorded during the same representative
storm event as analytical monitoring.
• 48 hours of little or no precipitation may now precede a "Representative
Storm Event".
Changes Specific to Wastewater
• A Lower TSS Wastewater limit for
PNA waters has been added.
• All mines must monitor for TSS—
but only industrial sand has limits.
• Approval of a Pumping 0&M Plan
maybe required prior to coverage
(for sites that may drain wetlands,
or where dewatering discharge may
affect sensitive waters like shellfish-
ing (SA) waters ).
• Fecal coliform monitoring is re-
quired for all WW discharges to SA
waters. I

Frequently Asked Questions
Do I have to monitor all outfalls? Do I need an ATC?
Yes. However, you may request Repre-
sentative Qutfall Status (ROS). If ap-
proved, this status allows analytical &
now qualitative monitoring at fewer
outfalls. To request ROS, submit a ROS
Request Form SWU-ROS (from our web -
site) to the DWQ Regional Office.
What if I can't collect a Storm -
water discharge in 30 minutes?
When distances separate multiple out -
falls and preclude collection within 30
minutes of each discharge event, begin
collection within 30 minutes and con-
tinue until all outfalls with discharges
are sampled. Documentation must be
kept in the SPPP.
Do I still need to monitor my
inactive mine?
Yes. BMP inspections of inactive mines
are reduced (to at !east 1x/month and
within 24 hours of any storm event 0.5"
or more). However, monitoring of inac-
tive mine discharges remains the same.
Inactive mines must possess an
"Inactive Renewal" from DLR and notify
the DWQ Regional office, or certify to
DWQ the site has been stabilized.
Where to do I have to sample
for turbidity?
Turbidity may be monitored at the
Stormwater or Wastewater Outfal!. The
permittee may choose to monitor tur-
bidity In the receiving water, directly
upstream and downstream of the Out -
fall. If you have a question about where
to monitor turgidity, contact your Re-
gldna! Office for assistance.
What if I don't address qualita-
tive monitoring problems?
If mines do not respond to problems
seen in visual monitoring, DWQ may
require that mines change the monitor-
ing frequency, apply for an individual
permit, implement in -stream monitor-
ing, install or modify structural storm -
water, controls, or implement other
controls.
What if I forget to monitor?
Mines that do not monitor per permit
conditions, shall monitor every month
until the permit is renewed. .
No, Per North Carolina Session Law 2011
-394, industrial wastewater permit dis-
chargers with an NPDES permit no
longer need additional approval "...for
the construction, installation, or altera-
tion of any treatment works ". However,
wastewater discharges must still meet
effluent limitations. Exceeding these
limitations is a violation of permit terms.
Where & when do I send DMRs?

It is no longer a requirement of this per-
mit application for a Professional Engi-
neer to sign off on plans and calcula-
tions. However it may be a requirement
of other North Carolina regulations, such
as those of the N.C. Board of Examiners
for Engineers and Surveyors.
What if I can't sample because
of bad weather?
Adverse weather is dangerous or creates
inaccessibility for personnel. Documen-
tation of adverse weather and the rea-
sons for not sampling must be included
Who can help me? v
Division of Water Quality (DWQ) Offices:
with your SPPP records. A substitute
sample must be taken during the next
qualifying storm event.
When do I need an O&M Plan?
When a mine with dewatering activities
has the potential to drain nearby wet-
lands or surfaces waters, the Division
may require a Pumping 0&M Plan be
approved prior to Issuance. These O&M
Plans shall include -but are not limited to:
groundwater monitoring strategies &
detailed plans to maintain the surround-
ing hydrology.
Why did you replace Oil &
Grease with TPH in VM areas?
The TPH test only targets chemicals de-
rived from crude oil. The Oil & Grease
(O&G) test recovers fats from animal &
vegetable sources and chemicals from
crude oil. Because TPH is more specific,
it is a better parameter for vehicle main-
tenance areas. The TPH method we are
specifying for NCG02 is EPA 1664A (SGT-
HEM). It is important to mote that DWQ
is not specifying the more expensive Gas
Chromatograph (GC) TPH method.
DWQ found lab costs for EPA 1664A
(SGT-HEM) to be comparable to current
0&G test costs. To test these parame-
ters in Stormwater, labs must perform
this test in accordance with EPA proce-
dures, but do not need to be certified.
Note: A lower benchmark applies for
TPH:15 mg/I (not 30 mg/1).
What about the Coastal Rules?
If you have a new mine in one of the 20
Coastal Counties with >10,000 BUA, you
may also need a State Stormwater Per-
mit. If you are in the 20 Coastal Coun-
ties, check with the Regional Office.
Can I use PAMS at my site?
Yes, floccuiants may be used. A list of
evaluated products is on our website.

Dear Mr. Bean:
John E. Skvarla, III
Secretary
On April 10, 2013, a site inspection was conducted for the Lamont Road C&D Landfill facility located at Lamont
Road, Cumberland County, North Carolina. A copy of the Compliance Inspection Report is enclosed for your
review. William Decarmine and Jim Duncan were also present during the inspection and their time and assistance
is greatly appreciated. Permit coverage authorizes the discharge of stormwater from the facility to receiving
waters designated as an UT to Bones Creek, a Class C stream in the Cape Fear River Basin. The site visit and
file review revealed that the subject facility is covered by NPDES Stormwater General Perm it-NCG1 20000,
Certificate of Coverage- NCG120093.
As a result of the inspection, the facility was found to be in compliance with the conditions of the NCG120000
permit. Please refer to the enclosed Compliance Inspection Report for additional comments and observations
made during the inspection.
Please be advised that violations of the NPDES Stormwater General Permit are subject to a civil penalty
assessment of up to $25,000 per day for each violation. If you or your staff has any questions, comments, or
needs assistance with understanding any aspect of your permit, please contact me at (910) 433-3300 or by e-mail

Inspection Summary:
During the inspection, records were available for review. Past samples have shown that there is a problem with meeting
fecal coliform benchmark values. This facility is now under the tier two requirments due to benchmark exceedance of fecal
coliform. Mr Decarmine is trying different things to determine what is causing the fecal count. He is sampling at an
upstream manhole at this time to determine if an old septic drainfield may be causing the problem. The stormwater that
leaves the landfill may never reach the receiving stream due to the distance between the landfill and the stream. Please
keep the Regional Office informed with your findings.

Subject: NPDES Stormwater Permit Coverage Renewal
Lamont Road C&D Landfill
COC Number NCG120093
Cumberland County
In response to your renewal application for continued coverage under stormwater General Permit NCG120000
the Division of Water Quality (DWQ) is forwarding herewith the reissued General Permit. This permit is
reissued pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of
Agreement between the state of North Carolina and the U.S. Environmental Protection Agency, dated October
15, 2007 (or as subsequently amended).
The following information is included with your permit package:
• A new Certificate of Coverage (COC)
• A copy of General Permit NCG120000
• A copy of the Technical Bulletin for the General Permit
• Two copies of the Discharge Monitoring Report (DMR) Form
• Two copies of the Qualitative Monitoring Report Form
The General Permit authorizes discharges of stormwater, and it specifies your obligations for discharge
controls, management, monitoring, and record keeping. Please review the new permit to familiarize yourself
with all the changes in the reissued permit. Your facility has six (6) months from the time of receipt of the
permit to update your current SPPP to reflect all new permit requirements.
The first sample period of your permit begins January 1, 2013. Your facility must sample a "measureable
storm event" beginning during the periods beginning January 1 and July 1 of every year (or, if applicable,
report "No Flow," as outlined in Part III, Section E). Also, please note that Tier 3 Actions in Part II of your
permit are triggered by benchmark exceedances on four occasions beginning on the effective date of this
permit and do not count prior exceedances.
The more significant changes in the General Permit since your last COC was issued are noted either in the
Draft Permit Fact Sheet that accompanied the public notice (http://portal.ncdenr.ore/web/wg/ws/su/current-
notices), or in the Response to Comments / Summary of Changes and Technical Bulletin documents that are
posted on the Stormwater Permitting Unit's website with the new General Permit. Please visit
http://Portal.ncdenr.ore/web/waws/su/npdessw (click on 'General Permits' tab) tb review that information
for your specific General Permit carefully.

Some of the changes include:
Part II:
• Section A: The Stormwater Pollution Prevention Plan (SPPP) section, if applicable, has been updated
to the most current language of our permits. Additional conditions for specific industry sectors have
been added to the SPPP requirements in some cases.
• Sections B, C: Failure to perform analytical stormwater monitoring may result in the Division requiring
that the permittee begin a monthly sampling scheme.
• Sections B, C: A lower TSS benchmark of 50 mg/I for HQW, ORW, PNA and Tr Waters applies to these
more sensitive waters.
• Sections B, C: The monitoring parameter Oil & Grease (O&G) has been replaced by the parameter
Total Petroleum Hydrocarbons (TPH) for vehicle maintenance areas, and in some cases, other
analytical monitoring requirements.
• Sections B, C, D: Inability to sample due to adverse weather must be recorded in the SPPP, or in
separate on -site records if your General Permit does not require an SPPP. Adverse weather is defined
in the "Definitions" section of the permit.
• Sections B, C. The term "Representative Storm Event" has been replaced by "Measurable Storm
Event." A measurable storm event is defined in the permit.
• Section A: If the permittee fails to respond effectively to problems identified by qualitative
monitoring, DWQ may require the permittee to perform corrective action.
Please review Parts III and IV to understand the Standard Conditions of your new NPDES General Permit,
including Compliance and Liability, Reporting, Monitoring and Records requirements; Operation and
Maintenance obligations; and Definitions. Please note that all samples analyzed in accordance with the terms
of this permit must be submitted to the Division on Discharge Monitoring Report (DMR) forms available on
the Stormwater Permitting Unit's website above. DMR forms must be delivered to the Division no later than
30 days from the date the facility receives the sampling results from the laboratory. Also note that existing
permittees do not need to submit a renewal request prior to expiration unless directed by the Division.
Your coverage under the General Permit is transferable only through the specific action of DWQ. This permit
does not affect the legal requirements to obtain other permits which may be required by DENR, nor does it
relieve the permittee from responsibility for compliance with any other applicable federal, state, or local law,
rule, standard, ordinance, order, judgment, or decree. If you have any questions regarding this permit
package, please contact the Division's Stormwater Permitting Unit at (919) 807-6300.

Subject: RE: NCG120093 - Lamont Rd. C&D Landfill (Fort Bragg)
Robert,
This landfill at Bragg has been under an erosion control plan with DLR for some time and while inspecting it along with
DLR staff a few weeks back I realized they did not have permit coverage under NCG12. I spoke with Bragg personnel
about whether or not their MS4/Phase II permit covered this facility and they decided it was best to apply for coverage
under the NCG12 for this individual facility much like the general permit coverage they have for a mine/borrow pit under
NCG02. The FRO has no objections to issuance of a COC for this facility.
Thanks,
Mike

Mike,
Please see NOI attached.
We have received an application for coverage of an exiting C&D facility under NCG 12 from Fort Bragg. The facility
drains to an LIT to Bones Creek (class C) in Fort Bragg (Cumberland County). They have vehicle maintenance, but no haz.
waste. Does the FRO have any concerns about issuing this facility a COC for this general permit?
Thanks.
