On September 18, 2012, a site inspection was conducted for the North Harnett Regional WWTP located on 607
Edwards Drive, Harnett County, North Carolina. A copy of the Compliance Inspection Report is enclosed for your
review. Permit coverage authorizes the discharge of stormwater from the facility to receiving waters designated
as the Cape Fear River, a Class WS-IV water in the Cape Fear River Basin. The site visit and file review revealed
that the subject facility is covered by NPDES Stormwater General Permit-NCG110000, Certificate of Coverage-
NCG110136.
Asa result of the inspection, the facility was found to be in non-compliance with the conditions of the NCG110000
permit. Please refer to the enclosed Compliance Inspection Report for additional comments and observations
made during the inspection.
As a result of the site inspection, the following permit conditions had not been met:
1) Stormwater Pollution Prevention Plan (SPPP)
A Stormwater Pollution Prevention Plan (SPPP) has not been developed, recorded, or properly implemented.
2) Qualitative Monitorinci
Qualitative monitoring has not been conducted and recorded in accordance with permit requirements.
Required Corrective Actions:
• Develop and properly implement a stormwater pollution prevention plan as soon as possible
• Begin the Qualitative Monitoring requirements as required by the stormwater permit, NCG110000.

In accordance with your application for a discharge permit received on February 12, 2008
we are forwarding herewith the subject certificate of coverage (COC) to discharge under the
subject state — NPDES general permit. This permit is- issued pursuant to the requirements of
North Carolina General Statute 143-215.1 and the Memorandum of Agreement between North
Carolina and the US Environmental Protection Agency dated May 9, 1994 (or as subsequently
amended).
Please take notice that this certificate of coverage is not transferable except after notice to
the Division of Water Quality. The Division of Water Quality may require modification or
revocation and reissuance of the certificate of coverage. This permit does not affect the legal
requirements to obtain other permits which may be required by the Division of Water Quality or
permits required by the Division of Land Resources, Coastal Area Management Act or any other
federal or local governmental permit that may be required.
Please note that the attached permit expires on April 30, 2008. Your COC will be
automatically renewed to cover the full 5 year term of the soon to be reissued permit, In the
interim you must abide by the terms of the expiring permit. You can review the proposed

plan or other document filed or required to be maintained under Article 21 or regulations of the Environmental
Management Commission implementing that Article, or who falsifies, tampers with or knowingly renders inaccurate any
recording or monitoring device or method required to be operated or maintained under Article 21 or regulations of the

Robert,
Another staff member in our office, Mark Brantley, has been involved with this
facility's recent expansion from a 0.75MGD plant to a 5.6MGD plant, hence the
submittal of the NOI. I just spoke with Mark and he is comfortable with the
information on the NOI. He did mention that a couple of the facility's outfalls,
which consist of drainage ditches along a fairly steep slope, are in need of some
repair as there has been a significant amount of erosion within and around the
ditches. Not sure if this has any bearing on issuing permit coverage, just wanted to
relay the information.. We recommend proceeding with issuing a Certificate of Coverage
and we'll address the conditions of the permit and necessary BMP/outfall maintenance
with the permittee.

We have received an application for coverage of an existing facility under NCG 11
from Harnett County Public Utilities for their North Harnett Regional WWTP. The
facility drains to the Cape Fear River (class WS-IV) in Lillington (Harnett
County).' They also have the following ww permits: NC0021636, WQ0014855, and
WQ0028562. No hazardous waste or vehicle maintenance. Does the Fayetteville
Regional Office have any concerns about issuing this facility a COC for this
general permit?
