In accordance with your application for a discharge permit received on November 21 St, 2007, we are
forwarding herewith the subject certificate of coverage to discharge under the subject state — NPDES general
permit. This permit is issued pursuant to the requirements of North Carolina General Statute 143-215.1 and the
Memorandum of Agreement between North Carolina and the US Environmental Protection Agency dated May
9, 1994 (or as subsequently amended).
PIease take notice that this certificate of coverage is not transferable except after notice to the Division
of Water Quality. The Division of Water Quality may require modification or revocation and reissuance of the
certificate of coverage.
This permit does not affect the legal requirements to obtain other permits which may be required by the
Division of Water Quality or permits required by the Division of Land Resources, Coastal Area Management
Act or any other federal or local governmental permit that may be required.

Hi Jennifer,
I have inspected both of these facilities in the past year and have no objections to
issuance of both general permits. They both have fully implemented stormwater
plans. There are no potential impacts to wetlands.

We've received an NOI from the Town of Cary for their water reclamation facility in
Wake County for coverage of their water reclamation facility under NCGll (Sewerage
Systems). The site discharges stormwater into Crabtree Creek (C, NSW). The NOI is
attached (under our contacts for the Raleigh Regional office you are both listed as
contacts for Wake county so I have addressed this to you both).
Does the Raleigh Regional Office have any concerns about issuing this facility a COC
for this general permit, and are there any potential impacts to wetlands? From their

Subject: Compliance Evaluation Inspections
North Cary Water Reclamation Facility
NPDES Wastewater Permit NCO048879 and
NPDES Stormwater Permit NCG1101 16
Wake County
1, Dave Parnell of the Raleigh Regional Office of the Division of Water Quality conducted two compliance
evaluation inspections (CEI) on November 23, 2010, consisting of the NPDES wastewater and stormwater
inspections. The assistance of Chris Parrisher, Operator in Responsible Charge (ORC), Paul Ray, Backup ORC,
and Jonathon Bulla, Laboratory Supervisor, was appreciated. This letter replaces a letter dated December 19,
2010. This letter addresses several inaccuracies identified in the prior letter. DWQ apologizes for any
inconvenience that may have been created. Please review the attached Basinwide Information Management
System (RIMS) inspection checklists summarizing the inspections.
Below is a list of findings and recommendations developed from the inspections:
1. NPDES Wastewater Inspection:
1. The current permit was available during the CEI.
2. The 12.0 MGD (permitted) wastewater treatment plant consists of the following units: influent pump
station, mechanical screening, grit and grease removal, nutrient removal system consisting of
anaerobic selectors, oxidation ditches, anoxic zones and re -aeration zones, duel secondary clarifiers,
deep bed sand filters, ultraviolet disinfection, cascade aeration, belt thickeners, & sludge digesters.

The inspection began in the water reclamation facility's office. All paperwork was well organized
and easily accessible. Discharge Monitoring Reports were reviewed for compliance with permit
limits and monitoring requirements. Chain of custody records were available and found to be in
order. The laboratory is state certified and performs the majority of permit required parameters. A
contract lab, Meritech, performs the remainder of the parameters required by the permit.
4. Overall maintenance was found to be in good condition. The mechanical band screen -with
compaction was in good condition. Grit removal was free of organic matter and excessive odor.
Debris is removed as needed from the site.
5. The clarifiers were functioning properly and were free of weir blockages and showed no sign of
short-circuiting. The sludge blanket was reported to be 1.5' feet, which is within the acceptable
range of sidewall depth of 14'.
6. BiologicaI nutrient removal of Nitrogen and Phosphorous is conducted at the facility. The nutrient
removal process at the facility is operating properly.
7. The deep bed sand filters appeared to be functioning properly. They were covered to prevent algae
growth.
8. There are two aerated sludge holding tanks in use which are used concurrently to store biosolids
until transported to the South Cary Water Reclamation Facility for further processing.
9. Standby power is available and is tested as required. The generators have the capacity to run the
entire water reclamation facility.
10. The effluent pipe discharges to a unnamed tributary to Crabtree Creek, a class B-NS W water in the
Neuse River Basin. The effluent appeared to be clear. The receiving water is free of foam and other
debris.
II. NPDES Stormwater Inspection:
The stormwater pollution prevention plan (SP3) was briefly reviewed and found to be adequate. All
regular inspections and duties, including training, annual review, qualitative monitoring, knowledge
and inspections of secondary containment are being accomplished as required by the stormwater
permit.
2. During a subsequent file review it was discovered that there were two stormwater permits for this
facility. NCG1 10116 is current and NCGI 10027 had expired or had been replaced by the current
one. Unfortunately the December 19, 2010 letter referenced the expired permit. I and DWQ,
apologizes for the confusion.
