On November 3, 2010, Mandy Hall of the Raleigh Regional Office visited the subject
facility, The assistance of Mrs. Amy Moore, Public Utilities Director, was much appreciated.
The Holly Springs Reclamation Facility is permitted to discharge a monthly average flow of
2.4 MGD to Utley Creek, Class C Waters of the Cape Fear River Basin.
Components located at the facility and operated by the Town of Holly Springs include:
mechanical bar screens, grit chamber, influent pump station, oxidation ditches with BNR
capability, sludge stabilization/storage facilities, secondary clarifiers, tertiary filters, UV
disinfection, cascade aerator, etc.
Treatment Units and Process
The facility has recently completed an upgrade and all components associated with the
upgrade were fully operational.
Holly Springs is utilizing the Cannibal system for enriching organic matter and removing
additional inorganic matter from Return Activated Sludge before it is returned to the BNR
process. Mrs. Moore indicated that the process is producing sludge with up to a 30% solid
content. Sludge is lime -stabilized and land -applied by Synagro.,

Records
The March, 2010 Daily Monitoring Report was reviewed and compared to the data at the
facility. No discrepancies were found during the review. All records and reports are being
maintained by the facility in accordance with the NPDES Permit. The laboratory
certification is current.
The facility did have some difficulty during 2009 meeting Total Phosphorus permit limits
attributed to the start-up of the Bio-Phosphorus Removal portion of the process. With the
addition of Sodium Aluminate and diligence with process control measures, the facility is
currently in full compliance with the limits of the permit.
Stormwater
All requirements of the subject General Stormwater Permit are being met by the facility.
The site has been graded and seeded with stormwater BMPs in place that include ponds
and level spreaders. The SP3 was in order and employee training is being conducted
annually, in accordance with the permit.

Dear Mr. Byrd:
The subject inspections were conducted by me, Mitch Hayes of the Division of Water Quality, on
October 17, 2012. Yours and Terry Foster's assistance with this inspection was greatly appreciated. Below
is a list of findings and recommendations developed from the inspection:
1. The subject wastewater permit was issued April 05, 2012, became effective May 31, 2012 and will
expire July 31, 2016.
2. Changes to the permit include: monitoring for priority pollutant scan is changed from once a year to
three times during the current permit term; effluent limitations pages were added for design flows
of 6.0 MGD and 8.0 MGD; instream monitoring downstream of Outfall 002, designated D1, and at
the White Oak arm of Harris Lake, designated D2, will be required after plant expansion to and
above 6.0 MDG. This instream monitoring may not be waived or exempted by participation in the
Middle Cape Fear River Basin Association monitoring program.
3. Basinwide Information Management System (BIMS) lists the facility as a Grade 4 Wastewater
Treatment plant (VWIJ-4). Terry R. Foster, David Futrell, AmyJo Moore, Kai J. Vannoy are listed
as back-up Operator in Charge (bORC) WW-4 certified. A DWQ database review failed to reveal
an ORC designation for the Utley Creek Water Reclamation Facility. Please contact Gail
Wnley with Technical Assistance and Certification at 919.807.6344 to correct the ORC
designation and any bORC designation.
4. The 2.4 MGD Biological Nutrient Reduction (BNR) water reclamation facility consists of the
following units: mechanical bar screens; vortex -type grit collector; influent pump station with 6 ABS
pumps; 3 - oxidation ditches using Bardenpho 5-statge process for BNR removal; 3 secondary
clarifiers; coagulation basin with chemical feed for total phosphorus removal; 4 banks of Hydrotech
cloth discfilters; 2 UV banks for disinfection; reclaimed water pump station with conjunctive reuse
system; step cascade aeration; RAS pump station, a cyclone screenings separator; interchange
tanks with mixers and diffused aeration; decant clarifier; aerobic digester; sludge dewatering; and 2
on -site generators.
5. At the time of inspection, the automatic bar screen and vortex grit removal system were functioning
normally; one treatment train was being operated due to low inflow; two out of three secondary
clarifiers were in operation due to low inflow; two banks of Hydrotech cloth discfilters were being

operated due to low inflow; one bank of UV disinfection was in operation due to low inflow; the
cyclone screenings separatorwas operating normally; the aerobic digesterwas empty. The colorof
the influent in the ditches appeared medium to dark brown which is normal. Foam was covering
less than 10% of the surface area. Sludge judge reading in one of the secondary clarifiers was•4
feet with a sidewall depth of 16feet. There was no solids or scum build-up in the center well. The
weirs in the secondary are covered and easily accessible. Clear effluent was flowing through the
weirs. The effluent appeared clear flowing on the cascade aerator into Utley Creek. The on -site
generators were operating during the inspection and operate once a week.
6. Up to 1200 dry tons of sludge per year is land applied under Town of Holly Springs Land
Application Program covered under permit WQ0019098. In -plant reclaimed water is utilized
throughout the plant. Reclaimed water is also used for irrigation in the Twelve Oaks subdivision,
Green Oak Parkway subdivision, New Hill Development, and in non -contact cooling water at
Novartis. Reuse, reclaim water, and land application permits are currently managed by the Aquifer
Protection Section.
7. The laboratory was checked in a cursory manner. The laboratory certification is current. EPA 624
and 625 volatiles semi-volatiles, toxicity, low level mercury, Total Nitrogen analyses are conducted -
by Merritech Inc. All pH buffers were within expiration date. At the time of inspection the refrigerator
temperature was 4 degrees C, the BOD incubator was 20.5 degrees C, the effluent sampierwas 4
degrees C, and the fecal bath was 44.5 degrees C.
8. Discharge Monitoring Reports (DMR's) for the period September 2010 through August 2012 were
reviewed for compliance with permit limits and monitoring requirements. There were no violations
listed for the review period. Commercial lab results, chain of custody records, and bench sheets
were compared with data submitted on the DM for the month of August 2012. No discrepancies
were noted.

9. _ Inspection of the stormwater outfalls and plant site was conducted at the same time as the CEI for
the Water Reclamation Facility. Permit NCG110096 was reissued and became effective on June
01, 2008 and expires January 31, 2016,
10. There are five stormwater outfalls listed for the facility. The level spreaders for three (3) of the
outfalls are scheduled for maintenance.
11. Qualitative monitoring for all outfalls has not been conducted. There was miscommunication about
ending monitoring when construction was complete however; it is believed that this was for the
NCG010000 permit. It was explained during the inspection that qualitative monitoring, as required
by NCG110096, will be conducted as appropriate.
12. There has been one spill this year resulting from backwashing the disc screens. Employee training
on spill prevention and emergency response occurred in May 2012. All elements of the Stormwater
Pollution Prevention Plan, except qualitative monitoring, are being maintained.

qualitative monitoring is resumed at all outfalls.
There has been one spill this year resulting from backwashing the disc screens. Employee training on spill prevention and

2006, we are forwarding herewith the subject certificate of coverage to discharge under the
subject state -- NPDES general permit. This permit is issued pursuant to the requirements of
North Carolina General Statute 143-215.1 and the Memorandum of Agreement between North
Carolina and the US Environmental Protection Agency dated May 9, 1994 (or as subsequently
amended).
Please take notice that this certificate of coverage is not transferable except after notice to
the Division of Water Quality. The Division of Water Quality may require modification or
revocation and reissuance of the certificate of coverage.
This permit does not affect the legal requirements to obtain other permits which may be
required by the Division of Water Quality or permits required by the Division of Land
Resources, Coastal Area Management Act or any other federal or local governmental permit that
may be required.
