Skvarla, III Secretary Enclosed please find a copy of the Compliance Evaluation Inspection form for the stormwater inspection conducted by Mack Wiggins and of the Raleigh Regional Office on December 7, 2012. The assistance offered by Mr. Bill Perkinson was appreciated. The facility was found to be in Compliance with stormwater permit NCG 110000 at the time of this inspection. 1. The following storrwater observations were noted: 1. The Stormwater General Permit Certificate of Coverage (NCG 1 10087) was issued to this facility on June 1, 2008 to the Town of Warrenton's WWTP, The General Permit expires May 31, 2013. 2. Warrenton's Wastewater Treatment Plant has six stormwater outfalls. All outfalls were observed during the inspection and each was assessable for sampling at the time of the inspection. 3. The facility does not perform vehicle maintenance activities on -site which uses more than 55 gallons of new motor oil when averaged over a calendar year. Therefore, only qualitative monitoring is required. Qualitative monitoring was conducted as required in NCG 110000. The Stormwater Pollution Prevention Plan (SPPP) has been implemented. The plan was complete and comprehensive. 5. All chemicals have secondary containment. Sullins Director November 4, 2010 Subject: Compliance Evaluation Inspections Warren County Wastewater Treatment Plant NPDES Permit No. NCO020834 and Stormwater Permit No. NCG 110087 Warren County Dear Mr. Freeman: Dee Freeman Secretary On October 28, 2010 Mr. Myrl Nisely of the Raleigh Regional Office conducted compliance inspections for two permits held by the Warren County Wastewater Treatment Plant, with the assistance of Mr. William Perkinson, ORC of the facility. His help was appreciated. The plant was compliant with both permits. Please see the attached inspection checklists for specific comments. The following observations were noted: NCO020834 The plant consists of an influent pump station with three pumps, a mechanical bar screen followed by a manual bar screen in series, vortex grit removal; dual oxidation ditches, only one of which is in use; two secondary clarifiers, four tertiary sand filters, gas chlorination followed by sulfur dioxide dechlorination; two aerobic digesters used in an alternating pattern, a lime silo and slurry mixer; cascade post aeration, and effluent flow measurement. Plans are underway to upgrade nearly all parts of this aging plant. So far, money has been obtained to replace the SCADA system and to replace the mechanical parts of both secondary clari Piers. Appropriate levels of nitrification are being achieved, as well as operational efforts to affect biological nutrient removal. The plant is part of the Tar -Pam Basin Association. Standby generator power has been expanded since the last inspection to include the disinfection system. The Stormwater Pollution Prevention Plan was reviewed and updated on 4/13/2010. One round of semi-annual Qualitative Monitoring has been done. Employee training was given on 10/ 18/2010. Qualitative monitoring has yet been carried out as required. Even though a list of contact phone numbers are posted in the lab/office area, it is recommended that the list in the Plan be moved to become the first page. Sullins Director March 16, 2009 Subject: Compliance Evaluation Inspections Warren County Wastewater Treatment Plant NPDES Permit NCO020834 and Stormwater Permit NCGI 10087 Warren County Dear Mr. Freeman: Natural Resources Dee Freeman Secretary I, Dave Parnell of the Raleigh Regional Office of the Division of Water Quality, conducted two inspections on March 11, 2009, consisting of the NPDES and Stormwater inspections. The assistance of Mr. Bill Perkinson, Operator in Responsible Charge, was appreciated. Please review the attached Basinwide Information Management System (BIMS) inspection checklists summarizing the inspections. Below is a list of findings and recommendations developed from the inspections: I. NPDES Permit NCO020834 Inspection: 1. The current permit was reissued November 1, 2004 and expires October 31, 2009 2. The 2.0 MGD wastewater treatment plant consists of the following units: influent pump station, influent pumps, manual and mechanical bar screens, mechanical grit removal system, duel oxidation ditches (one in use), two secondary clarifiers, four tertiary sand filters, gas chlorination, chlorine contact tank, de -chlorination (sulfur dioxide), aerobic digester, cascade post aeration and effluent flow measurement. 3. The inspection began in the treatment plant office. All paperwork was well organized and easily accessible. Discharge Monitoring Reports were reviewed for compliance with permit limits and monitoring requirements. Chain of custody records were available and in order. The facility uses a contract laboratory. Sample storage was kept at a temperature of 4.5 degrees Celsius. The effluent composite flow proportional refrigerator temperaturewas kept at 2.0 degrees Celsius. Overall maintenance was above average. The manual and mechanical bar screens were in acceptable condition. Grit removal was free of organic matter and excessive odor. One, of two, oxidation ditches is currently being operated. The second is not needed and is available as storage during extreme storm events Very little foam was noted. The Dissolved Oxygen level was within the acceptable range of 1.0 to 3.0 at 1.2 mg/l. The settleometer results were in the acceptable range at 650 ml/I in 30 minutes. 6. Nutrient removal is not required by the permit, since the facility is a partner in the Tar -Pam River Association. 7. The clarifiers were functioning properly and were free of weir blockages and showed no sign of short-circuiting. The sludge blanket was reported to be 2.5 feet, which is in the acceptable range of sidewall depth. 8. The aerobic digester appears to have adequate capacity with acceptable odor. 9. The disinfection (chlorine) cylinders were secured adequately, and were a safe distance from the dechlorination cylinders. All of the cylinders were under a shelter and out of direct sunlight. 10. Standby power is available and was tested during the inspection. The generator has adequate capacity to run the plant. The chlorination system is now integrated into the standby power system. 11. The effluent pump station appears to be in good operational condition. Pumps and float controls are operable. The effluent composite sample is flow proportional and collected below all treatment units. 12. The effluent pipe discharges into Fishing Creek adjacent to the plant. Access to the pipe is well maintained. The effluent appeared clear. The receiving water is free of foam and other debris. II. Stormwater Permit NCGI 10087 Inspection: A Iist of significant spills occurring in the last three years must be included as a part of the Stormwater Pollution Prevention Plan. PIease implement this requirement immediately. 2. No method of secondary containment is required at this facility. 3. Continue semi-annual qualitative monitoring and document these inspections. Warrenton WWTP CEI Page 3 4. All outfalls were inspected. Outfall #4 has been impacted by sediment from erosion occurring above the outfall. This situation must be corrected by removing the sediment from the outfall, as well; correcting the erosion on the hillside. The wastewater treatment plant is considered to be in compliance with the NPDES permit, as well as the general stormwater permit, at the time of these inspections.
