Dear Mr. Greeley:
On December 4, 2012, Cheng Zhang of the Raleigh Regional Office (RRO) conducted a
compliance evaluation inspection of the subject facility. The assistance provided by Mr. Charles
Cocker, the operator in responsible charge (ORC) of the facility, was appreciated as it facilitated
the inspection process. The inspection report is attached. The following observations were
made:
South Durham WRF is permitted to discharge at a rate of 20.0 MGD and consist of -
mechanical cleaned bar screens, lord; channel grit chambers, primary clarifiers, aeration
basins, secondary clarifiers, tertiary sand filters, flocculation basins, anaerobic digesters,
aerobic digesters, gravity belt thickener, belt filter press, sand drying beds, and UV
disinfection. The plant discharges to an unnamed tributary to New Hope Creek, a Class WS-
IV NSW water in the Cape Fear River basin.
All primary, secondary, and tertiary treatment and post -treatment processes, especially
the BNR system, were in good operational condition. Two of the four primary clarifiers,
four of the eight five -stage activated sludge basins, and two of the six secondary clarifiers
were in use because of low flow. The chemical feed systems for sodium aluminate,
sodium hydroxide and polymers were all functioning well.
2. Three standby power generators are tested under load monthly. The generators will be
activated automatically when primary power is out. The generators have capacity to
operate the entire facility during power outrage.
3. The ORC maintains a daily logbook. Lab results, chain -of -custody forms, and DMRs
were complete and current, kept in good order and ready for review. April 2012 DMR
data were compared to lab bench sheets; no data transcription errors were noted during
the injection.

4. The right of way to the outfall was well maintained. The effluent was observed to be
clear and free of solids. No visible impacts were noted immediately downstream the
effluent pipe.
Field lab parameter (Certificate No. 9498) calibration and analysis records were reviewed
during the inspection.
6. A compliance evaluation inspection for the NPDES stormwater permit (No.
NCG110082) was also conducted. The facility has developed and implemented a
Stormwater Pollution Prevention Plan (SPPP), which includes all required components.
All stormwater discharge outfalls were observed; these outfalls and the right-of-way were
well maintained.

Mack Wiggins and Vicki Webb of the North Carolina Division of Water Quality, Surface Water
Protection Section, Raleigh Regional Office, conducted a stormwater compliance evaluation inspection
of the South Durham WRF, at 6605 Farrington Road on May 26, 2011. The assistance offered by Mr.
Robert Dodson and Mr. Charlie Cocker was very helpful and appreciated during the inspection process.
The following observations were made during the stormwater inspection:
1. The Stormwater Pollution Prevention Plan for this facility contained all the components as
required by NCG110000. The facility must continue to aggressively implement the plan.
2. Analytical monitoring is not required at this facility. The facility does not do any vehicle
maintenance.
3. Qualitative monitoring was performed at the stormwater outfalls as required by NCGI 10000.
Continue to monitor your outfalls. Please include your stormwater Permit No. NCG 110082
on all stormwater monitoring forms.
4. In October 2009 manually operated gates were installed at the stormwater discharge point
near the influent pumps station in order to contain any spills from the majority of the plant
site.
All outfalls were visited and were easily accessible during the inspection. Outfalls 4 & 5 are
internal outfalls. Sampling should and has been discontinued at these locations. The SP3
plan has been modified accordingly.
6. All chemicals are stored in the secondary containment as required.

Mack Wiggins of the North Carolina Division of Water Quality, Surface Water Protection Section,
Raleigh Regional Office. conducted a stormwater compliance evaluation inspection of the South
Durham WRF, at 6605 Farrington Road on March 30, 2010. The assistance offered by Mr. Robert
Dodson was very helpful and appreciated during the inspection process.
The following observations were made during the stormwater inspection:
1. The Stormwater Pollution Prevention Plan for this facility contained all the components as
required by NCG110000. The facility must continue to aggressively implement the plan.
2. Qualitative monitoring was performed, at the stormwater outfalls as required by NCGI 10000.
Continue to monitor your outfalls. Qualitative monitoring must be conducted during the fall
months (September -November), and during the spring months (April -June). '
3. Analytical monitoring is not required at this facility. The facility does not do any vehicle
maintenance.
4. A sanitary line was connected in October, 2008 to the trench drain to re-route any spillage
from the scum concentrator building to the sanitary sewer. This eliminates contamination of
stormwater from scum building area.
5. In November 2008 a berm was installed at the septage dump site to contain any spillage. The
drain at this containment routes any spillage back to the headworks of the plant.

8. In October 2007 all stormwater lines were cleaned by the Water & Sewer Maintenance
Division.
9. All six outfalls were visited and were easily accessable during the inspection. The site visit
revealed 4 & S are internal outfalls. Sampling may be discontinued at these locations
provided the SP3 plan is modified accordingly.
10. All chemicals are stored in the secondary containment as required.

Mack Wiggins of the North Carolina Division of Water Quality, Surface Water Protection Section,
Raleigh Regional Office, conducted a stormwater compliance evaluation inspection of the South
Durham WRF, at 6605 Farrington Road on August 22, 2008. Your assistance and cooperation Mr.
Dodson was very helpful and appreciated during the inspection process.
The following observations were made during the stormwater inspection:
1. The Stormwater Pollution Prevention Plan for this facility contained all the components
as required by NCGI 10000. The facility must continue to aggressively implement the
plan.
2. Qualitative monitoring was performed at the stormwater outfalls as required by
NCG110000, Continue to monitor your outfalls. Qualitative monitoring must be
conducted during the fall months (September -November), and during the spring months
(April -June). '
3. Analytical monitoring is not required at this facility. The facility does not do any
vehicle maintenance.
4. All chemicals are stored in the secondary containment as required.
5. One non-stormwater discharge was found as a result of implementation of their SPPP
Plan. The trench drain outlet is currently being monitored. Plans are underway to
connect the discharge to an on -site manhole that will route the discharge to the head of
the Wastewater Treatment Plant. As per the ORC connection will be complete by end
of year 2008. 9

Observations were as follows:

Monitoring had been done but not submitted as required by stormwater permit NCG 110000.
We strongly recommend Mr. Roland that you implement your stormwater pollution
prevention plan and stormwater monitoring as required by your stormwater permit
NCG 110082 (NCG 110000).
Within 60 days of receipt of this letter please respond in writing as to how you have met all the
requirements, including the stormwater pollution prevention plan, of the General Permit No.

permit. This permit is issued pursuant to the requirements of North Carolina Gdneral`Statute 143-215 .1 and the
Memorandum of Agreement between North Carolina and the US Environmental Protection Agency dated May
9, 1994 (or as subsequently amended).,
Please take notice that this certificate of coverage is not transferable except after notice to the Division
of Water Quality. The Division of Water Quality may require modification or revocation and reissuance of the
certificate of coverage.
This permit does not affect the legal requirements to obtain other permits which may be required by the
