Box 1307 Oxford, NC 27565-1307 Dear Mr. Donham: Director November 13, 2012 SUBJECT: Compliance Evaluation Inspection Oxford Wastewater Treatment Plant NPDES Permit NCO025054 NPDES Stormwater Permit NCG110078 Granville County Secretary I, Mitch Hayes, of the Raleigh Region Office conducted the compliance evaluation inspection on November 06, 2012. The assistance of Dennis Wilson, Antoine Lewis and Cindy Marks with this inspection was greatly appreciated. Please find the attached Basinwide Information Management System (BIMS) checklist summarizing the inspection. Below is a list of findings and recommendations developed from the inspection: The subject permit was reissued and became effective April 1, 2010 and will expire January 31, 2014. Permit renewal is required 180 days before permit expiration. Please ensure the following items are corrected for the next permit renewal: the receiving stream should be listed as an Unnamed Tributary (UT) to Fishing Creek. The following should be added to the facility description: dual traveling bridge type sandfilters, 2- Return Activated Sludge (RAS) pump stations, 2 — thickened sludge storage lagoons at 2 million gallons (MG) each; and on site generator for power generation. 2. The 3.5 MGD facility consists of the following units: manual and automatic bar screens; mechanical aerated grit collector; influent Parshall flume; influent flow meter; influent pump station with 3 submersible pumps; influent selector featuring anaerobic, anoxic, and aerobic zones; 2 — oxidation ditches with each oxidation ditch equipped with two surface aerators andtwo submersible mixers providing nutrient reduction using BioDenipho process; 3 — secondary clarifiers; dual traveling bridge type sandfilters; dual ultra violet disinfection; post aeration; 2 — Return Activated Sludge (RAS) pump stations; 3 — aerated digesters at 400,000 gallons each; 2 — thickened sludge storage lagoons at 2 MG each; on -site generator for power generation. 3. At the time of inspection, one aerated digester was not being used. All other units were operational. Influent in the oxidation ditch appeared medium brown with foam covering more than 90% of the surface area, which is normal for this type of process. Total suspended solids in the ditch are being maintained around 3,000 mgll with a dissolved oxygen content of 1.2 mgll. Effluent flowing off the weirs of the secondary clarifiers appeared clear with no algae blockage. Effluent flowing into the clear well from the traveling bridge sandfilters appeared clear. Effluent flowing from the reareation contact chamber into the two discharge pipes appeared clear. Effluent flowing into the UT to Fishing Creek appeared clear. The digerters were functioning properly at both discharge outfalls. Thickened sludge is then pumped to one of two storage lagoons. Level in the storage lagoons was approximately three-quarters full. The on -site generator is operated under load once a week. The entire waste watertreatment operation is controlled by SCADA. This includes all alarms associated with high wet well in the RAS pump stations and influent pump station. 4. Thickened sludge is dewatered by Synagro, a contract sludge hauler, and land applied according to permit WQ0011244. For the year 2011, 1,555,000 gallons were land applied. 5. Discharge Monitoring Reports for the period September 2011 to September 2012 were reviewed for compliance with permit limits and monitoring requirements. The following violations were noted: frequency violation for failing to monitor for total selenium for the week ending 11.22.2011; frequency violations for failing to monitor total mercury for the weeks ending 03.10.2012, 03.17.2012, 03.24.2012, and 03.31.2012; frequency violation for failing to monitor for total suspended solids for the week ending 04.21.2012. All of the violations listed have been addressed. 6. Commercial lab results, chain of custody records, and bench sheets were compared with data submitted on the DMR for the month of September 2012. No discrepancies were noted. Oxford WWTP laboratory analyses fecal coliform, total phosphorus and BOD. All other permit parameters are analyzed by Merritech Environmental Laboratories Inc NPDES Stormwater Permit NCG110078 7. Stormwater Permit NCG110078 was issued June 06, 2008 and will expire May 31, 2013. Permit renewal is required within 180 days permit expiration date. A review of the permit requirements was conducted after the CEI for NC0025054. A copy of the permit was on hand and contained all permit requirements. There are 6 stormwater discharge outfalls. There was no discharge at the time of inspection. There have been no changes to the facilitywithin the past two years. All elements of the Stormwater Pollution Prevention Plan (SWPPP) are being maintained. Qualitative monitoring was conducted 05.15.2012 and 10.25.2012, Employee training was last conducted on 10.29.2012. Permit No. NCO025054 and NCG 110078 Granville County Dear Mr, Donham: On December 1, 2011, 1, Cheng Zhang of the Raleigh Regional Office (RRO) conducted a compliance evaluation inspection of the subject facility. The assistance provided by Mr. Dennis Wilson, the operator in responsible charge (ORC), and Mr. Antoine Lewis, the backup ORC of the facility, was appreciated as it facilitated the inspection process. The inspection report is attached. The following observations were made: Oxford Wastewater Treatment Plant is permitted to discharge at a rate of 3.5 MGD and consist of: bar screens and grit chamber, influent pump station with three pumps, two three - zone oxidation ditches with biological nutrient removal, three secondary clarifiers, ultra - violate disinfection, traveling bridge tertiary filter, post aeration basin with floating aerator, sludge thickener, and aerobic sludge digester. The plant discharges to Fishing Creek, a stream classified as C-NSW waters within the Tar -Pamlico River basin. 1. All primary, secondary, and tertiary treatment and post -treatment processes, especially the BNR system, were in good operational condition. Both BNR trains and all three clarifiers were in use 2. Waste activated sludge is pumped to the three aerobic sludge digesters. The digested sludge is pumped to the two sludge lagoons (Synagro Company is the sludge hauler). �. The standby power generator is tested under load weekly. The generator will be activated automatically when primary power is out. The generator has capacity to operate the entire facility during power outrage. 4. The ORC maintains a daily logbook. Lab results, chain -of -custody forms, and DMRs were complete and current. kept in good order and ready for review. The facility is certified to analyze the following field parameters: pH, temperature, and DO (Field Certification # 621), calibration/analysis records were checked and met the Division's requirements. 5. The right of way to the outfall was well maintained. The effluent was observed to be clear and free of solids. No visible impacts were noted immediately downstream the effluent pipe. 6. 1 also conducted a compliance evaluation inspection for the NPDES stormwater permit (No. NCG110078). The facility has developed and implemented a Stormwater Pollution Prevention Plan (SPPP), which includes all required components except that the employee training was not conducted in 2010. The facility also did not conduct qualitative monitoring during the first half of 2011. The facility must conduct and document employee training by the end of 2011. I observed all stormwater discharge outfalls, these outfalls and the right-of-way were well maintained. Please address Item 6 and respond in writing to RRO within 30 days of receipt of this letter, by indicating the completion of employee training and proper documentation of the qualitative monitoring. The overall condition of Oxford WWTP is compliant with Division standards. The assistance of Mr. Dennis Wilson, the operator in responsible charge (ORC) of the facility, was appreciated as it facilitated the inspection process. The inspection report is attached. The following observations were made: Oxford Wastewater Treatment Plant is permitted to discharge at a rate of 2.17 MGD and consist of: bar screens and grit chamber, influent pump station, activated sludge basin with floating aerator, two secondary clarifiers, chlorinating facility, post aeration basin with floating aerator, sludge thickener, and aerobic sludge digester. The plant discharges to Fishing Creek, a stream classified as C-NSW waters within the Roanoke River basin. Plant expansion was completed in September 2007, new treatment units include mechanic bar screen and grit chamber, new influent pump station with three pumps, two three -zone oxidation ditches with biological nutrient removal, active sludge return pump station, three secondary clarifiers, traveling bridge tertiary filter, ultra -violate disinfection, post aeration basin, and new effluent outfall. The expansion raised the treatment capacity to 3.5 MGD. 1. The current permit expired on January 31, 2009. The facility submitted permit renewal application to the Division on August 4, 2008. A new permit has been drafted and is still being reviewed by the Division. 2. All treatment units were in good operational condition. Both oxidation ditches and all three secondary clarifiers were in use at the time of inspection. The generator is tested weekly (monthly under load). 4. Wasted sludge is digested in the aerobic digester and thickened in the thickener. Final sludge is stored in two sludge lagoons before being hauled for land application. The ORC maintains a daily logbook. Lab results, chain -of -custody forms, and DMRs were complete and current, kept in good order and ready for review. October 2009 DMR data were compared to lab bench sheets; No data transcription errors were noted during the inspection. 6. The right of way to the outfall was well maintained. The effluent was observed to be clear and free of solids. No visible impacts were noted immediately downstream the effluent pipe. A compliance evaluation inspection was also conducted of the facility's Stormwater permit NCG110078. The facility has been implementing its Stormwater Pollution Prevention Plan (SPPP) as required. All six stormwater outfalls were observed during the inspection and were in good condition. Please update outfall locations in your SPPP. It is noted that the facility had been conducting qualitative monitoring annually, and must immediately start conducting and recording qualitative monitoring at all stormwater outfalls semi-annually, as required by the general permit. The SPPP should be reviewed and updated (if necessary) annually. The annual review or update must also be documented. The overall condition of Oxford WWTP is compliant with Division standards. The site drains to Fishing Creek, Class C NSW waters located in the Tar - Pamlico Basin. The site visit and file review revealed that the subject project is covered by NPDES Stormwater General Permit NCG110078 issued in 2004. On March 27, 2006, the Division of Water Quality conducted a site inspection and sent a Notice of Violation, NOV-2006-PC-0088, dated March 31, 2006 (copy attached). To date, DWQ has not received an adequate response to the issues noted in the March 31, 2006 Notice of Violation regarding your NPDES Stormwater General Permit NCG110078. From the recent site visit and file review, the following observations and/or permit conditions violations are noted. The NPDES permitting program for stormwater discharges was established in 1990. Phase I focuses on site and operations planning to reduce pollutant sources. Through the NPDES permitting program industrial facilities that fall into one of the subject ten categories are required to obtain permit coverage under a general permit or an individual permit, depending upon the facilities SIC code and the industrial activity occurring at the facility. There are currently 19 active general stormwater permits in North Carolina. One condition that is applicable to both the general permits and individual stormwater permits is the requirement to develop and implement site -specific comprehensive stormwater pollution prevention plans (SPPP). These plans are required to include a comprehensive evaluation of the site and operations to reduce pollutant sources and prevent pollutant discharge. Copies of the general permits and accompanying documents can be accessed from the following webpage: http://h2o.enr.state.nc.us/su/Forms_Documents.htm Permit text, technical bulletins, discharge monitoring forms, and a Notice of Intent [which is the application of for coverage under a NPDES General Stormwater Permit] can be found at this web site. The following items are a subset of permit requirements and are listed to assist you and help bring your attention to certain permitting issues, process and condition requirements of your permit. That said, please see the above mentioned web site in order to obtain a copy of the permit such that you may fully comply with all the conditions therein. Failure to properly comply with conditions of the permit, like those bulleted below, constitutes violations and are subject civil penalty assessment of up to $25,600.00 per day. • Industrial facilities that fall into one of the subject ten categories are required to obtain permit coverage under a general NPDES stormwater permit or an individual NPDES stormwater permit • All persons desiring to have facilities covered by a General Permit must register with the Division of Water Quality (DWQ) by the filing of a Notice of Intent (NOI) and Applicable Fees. • This Notice of Intent (NOI) must be submitted and a Certificate of Coverage issued prior to any discharge of stormwater associated with the industrial activity, process wastewater, and/or mine dewatering. • Once the Notice of Intent is received, DWQ will review and determine whether a Certificate of Coverage (COC) may be issued. If this Certificate of Coverage is issued the facility is covered by the General Permit. That is, the permittee is authorized to discharge stormwater, process wastewater, and/or mine dewatering pursuant to the type of General Permit until the permit expires or is modified or revoked. • Any other point source discharge to surface water of the state is prohibited unless it is an allowable non-stormwater discharge'or is covered by another permit, authorization or approval. Permit conditions vary accordingly; however, most of the permits have following condition requirements (you must read your entire permit to ensure full compliance): o Development and compliance with the Stormwater Pollution Prevention Plan. o Collection, reporting, and recording of Analytical Monitoring o Collection, reporting and recording of Qualitative Monitoring o Collection, reporting and recording of Vehicle Maintenance Monitoring o Records — Visual monitoring shall be documented and records maintained at the facility along with the Stormwater Pollution Prevention Plan. Copies of the analytical monitoring results shall also be maintained on -site. The Permittee shall retain records of all monitoring information, including all calibration and maintenance records and all original strip chart recordings for continuous monitoring instrumentation, and copies of all reports required by this general permit for a period of least 5 years for the date of the sample measurement, report or application. o Expiration — The permittee is not authorized to discharge after the expiration date of the general permit. In order to receive automatic authorization to discharge beyond the expiration date, the permittee shall submit forms and fees as are required by the agency authorized to issue permits no later than 180-days prior to expiration date. Any permittee that has not requested renewal at least 180 days prior to expiration, or any permittee that does not have a permit after the expiration and has not requested renewal at least 180 days prior to expiration, will be subject to enforcement. Failure to renew your permit is a violation. o Transfer — The Certificate of Coverage (COC) issued pursuant to this general permit is not transferable to any person expect after notice to and approval by the Director. The assistance and cooperation' of your Operator in Responsible Charge (ORC), Mr. Dennis Wilson and Backup ORC, Mr. Larry Thomas, was very helpful and appreciated. Inspection checklists are attached for your review. During the inspection the following items were noted: I. NPDES Wastewater CEI Permit NCO025054 A.) Our records indicate that Ms. Amanda T. Hall is the backup ORC. Please submit appropriate paperwork to the Division of Water Quality (DWQ) to initiate the change to our records to reflect the current backup ORC, Mr. Larry Thomas. B.) The influent headworks, consisting of a bar screen, a bar rack, a flow meter, a composite sampler, a wet well with three submersible pumps, level sensing and alarm systems and a grit removal system, was inspected. The bar screen was in use and operational. Screenings are sent to a landfill. The influent wet well seemed to be in good repair and all three submersible pumps, along with the level sensing and alarm systems, were installed and operational. According to the ORC the influent flow sampler is set to sample on a constant time/constant volume basis, as defined in Part 11, Section A of permit NC0025054. This particular sampling regime requires approval by the Director prior to use. We currently do not possess copies of such approval in our records for your plant. Please forward to DWQ all paperwork showing that you have approval. If you do not have the requested paperwork then please request approval from this office in writing. Alternatively, you could change the sampling regime of the influent composite sampler to one of the other three acceptable regimes that do not require pre -approval. The temperature of the influent sampler was acceptable and the tubing was clean. The grit removal system was not in full operation. Wastewater passts through the grit removal -hamber but the grit pump is not used to remove the grit on a regular basis. The ORC indicated that the system was not fully effective and that the grit is manually removed from the system once per year. The grit is sent to a landfill. C.) The aeration basin was observed. There appeared to be good mixing and aeration but some dead spots were noted. Foam coverage of the aeration basin was heavy. D.) The two secondary clarifiers were observed and seemed to be operating effectively. There was some foam/solids buildup in the stilling well of the eastern most clarifier. The weirs were level and no short-circuiting was noted. Very little solids carryover was observed and the sludge blankets were at an acceptable depth. E.) The chlorination/dechlorination and post -aeration systems were inspected; all systems were operating properly. Sodium Bisulfate is used to dechlorinate the plant effluent and you currently have one 500-gallon tank of the chemical on -site. This chemical is not protected from spillage with a double -walled tank or secondary containment. Please ensure you correct this as soon as possible by installing a secondary containment vessel or replacing the existing tank with one that is double -walled. Please note that if you install secondary containment that has a drain line, the drain line must have a valve installed on it and the valve must be lockable. Also, maintenance and drainage of any secondary containment device must be done in accordance with the requirements of your general stormwater permit and certificated of coverage, NCG110078. The effluent sampler was set to sample on a variable time/constant volume basis. The unit seemed to operate properly. The temperature was acceptable and the sample tubing was clean. The effluent flow meter was just recently calibrated. At 11:30 A.M., it read 1.12 million gallons per day (MGD). Cross-referencing this reading with the recorder in the operations building revealed that the recorder is reading correctly. F.) The sludge digesters and storage units were well operated and the appearance and odor of the sludge was acceptable. The air supply to the digester was sufficient. Waste sludge is land - applied at several different farms in Granville County. G.) The plant outfall was visited. The right-of-way to the outfall was well maintained. Minimal solids and foaming were noted in the receiving stream at the outfall. H.) The laboratory was clean and well operated/maintained. The sample refrigerator, Coliform incubator, and drying oven were all set to the correct temperatures and calibration methods and records were in order. 1.) A records review of discharge monitoring reports (DMR) and laboratory bench sheets from July, 2005 and October, 2005, revealed no transcription errors. Operations and ORC logs were available and in order. II. NPDES Stormwater CEL Permlt/COC NCG110078 Notice of Violation NOV-2006-PC-0088 A.) Neither Mr. Wilson nor Mr. Thomas was aware of the requirements of stormwater permit NCG110000. As such, there is essentially no stormwater management program for the plant and no Stormwater Pollution Prevention Plan (SP3) has been developed and implemented. All ,,fwWTP CEI ,i'/2006 requirements for this program and the SP3 can be found in the text of the permit. This permit was originally issued in 2004. Please ensure this program and plan are developed and implemented as soon as possible. Please provide a response to items number IA, IB and IE within 10 days of your receipt of this letter. The assistance and cooperation of your Operator in Responsible Charge (ORC), Mr. Dennis Wilson and Backup ORC, Mr. Larry Thomas, was very helpful and appreciated. Inspection checklists are attached for your review. During the inspection the following items were noted: I. NPDES Wastewater CEI, Permit NCO025054 A.) Our records indicate that Ms. Amanda T. Hall is the backup ORC. Please submit appropriate paperwork to the Division of Water Quality (DWQ) to initiate the change to our records to reflect the current backup ORC, Mr. Larry Thomas. B.) The influent headworks, consisting of a bar screen, a bar rack, a flow meter, a composite sampler, a wet well with three submersible pumps, level sensing and alarm systems and a grit removal system, was inspected. The,bar screen was in use and operational. Screenings are sent to a landfill. The influent wet well seemed to be in good repair and all three submersible pumps, along with the level sensing and alarm systems, were installed and operational. According to the ORC the influent flow sampler is set to sample on a constant time/constant volume basis, as defined in Part 1I, Section A of permit NC0025054. This particular sampling regime requires approval by the Director prior to use. We currently do not possess copies of such approval in our records for your plant. Please forward to DWQ all paperwork showing that you have approval. If you do not have the requested paperwork then please request approval from this office in writing. Alternatively, you could change the sampling regime of the influent composite sampler to one of the other three acceptable regimes that do not require pre -approval. The temperature of the influent sampler was acceptable and the tubing was clean. The grit removal system was not in full operation. Wastewater passes through the grit removal chamber but the grit pump is not used to remove the grit on a regular basis. The ORC indicated that the system was not fully effective and that the grit is manually removed from the system once per year. The grit is sent to a landfill. C.) The aeration basin was observed. There appeared to be good mixing and aeration but some dead spots were noted. Foam coverage of the aeration basin was heavy. D.) The two secondary clarifiers were observed and seemed to be operating effectively. There was some foam/solids buildup in the stilling well of the eastern most clarifier. The weirs were level and no short-circuiting was noted. Very little solids carryover was observed and the sludge blankets were at an acceptable depth. E.) The chlorination/dechlorination and post -aeration systems were inspected; all systems were operating properly. Sodium Bisulfate is used to dechlorinate the plant effluent and you currently have one 500-gallon tank of the chemical on -site. This chemical is not protected from spillage with a double -walled tank or secondary containment. Please ensure you correct this as soon as possible by installing a secondary containment vessel or replacing the existing tank with one that is double -walled. Please note that if you install secondary containment that has a drain line, the drain line must have a valve installed on it and the valve must be lockable. Also, maintenance and drainage of any secondary containment device must be done in accordance with the requirements of your general stormwater permit and certificated of coverage, NCG 110078. The effluent sampler was set to sample on a variable time/constant volume basis. The unit seemed to operate properly. The temperature was acceptable and the sample tubing was clean. The effluent flow meter was just recently calibrated. At 11:30 A.M., it read 1.12 million gallons per day (MGD). Cross-referencing this reading with the recorder in the operations building revealed that the recorder is reading correctly. F.) The sludge digesters and storage units were well operated and the appearance and odor of the sludge was acceptable. The air supply to the digester was sufficient. Waste sludge is land - applied at several different farms in Granville County. G.) The plant outfall was visited. The right-of-way to the outfall was well maintained. Minimal solids and foaming were noted in the receiving stream at the outfall. H.) The laboratory was clean and well operated/maintained. The sample refrigerator, Cohform incubator, and drying oven were all set to the correct temperatures and calibration methods and records were in order. 1.) A records review of discharge monitoring reports (DMR) and laboratory bench sheets from July, 2005 and October, 2005, revealed no transcription errors. Operations and ORC logs were available and in order. 11. NPDES Stormwater CEI, Permit/COC NCG110078 Notice of Violation NOV-2006-PC-0088 A.) Neither Mr. Wilson nor Mr. Thomas was aware of the requirements of stormwater permit NCG] 10000. As such, there is essentially no stormwater management program for the plant and no Stormwater Pollution Prevention Plan (SP3) has been developed and implemented. All Oxford WWTP CEI 03/31 /2006 r requirements for this program and the SP3 can be found in the text of the permit. This permit was originally issued in 2004. Please ensure this program and plan are developed and implemented as soon as possible. Please provide a response to items number IA, IB and IE within 10 days of your receipt of this letter. Also, please provide a narrative of your plan to achieve full compliance with your stormwater permit and your schedule for doing so within 30 days of your receipt of this letter. Please be aware that the findings concerning your stormwater management program (IIA above) constitute violations of North Carolina General Statute (NCGS) 143-215.1 et seq. and could result in the assessment of civil penalties in an amount of up to $25,000 per day per violation. The purpose of the SWPPP is to identify operations at the facility that may impact storm water runoff and to establish procedures to minimize any such impacts. The practices and procedures outlined in the SWPPP are designed to be implemented on a continuous basis to minimize potential impacts to storm water runoff at the facility. The plan is designed to be reviewed and updated in response to changes in operations at the facility. Amendments to the Clean Water Act in 1987 required EPA to develop regulations to control storm water discharges under the National Pollution Discharge Elimination System NPDES). EPA published regulations on November 16, 1990, which incorporated the permitting of certain storm water discharges into existing NPDES regulations. These regulations are generally incorporated into the North Carolina Department of Environment and Natural Resources (DENR) Division of Water Quality (DWQ) regulations. The discharge from City of Oxford WWTP is regulated under General Permit No. NCG 110000 for discharges of storm water. • 2.0 STORM WATER POLLUTION PREVENTION TEAM The City of Oxford WWTP has a Storm Water Pollution Prevention Team to ensure that the SWPPP is implemented and maintained in accordance with good engineering and management practices. The Team is comprised of various facility personnel. The team will meet to evaluate and discuss the status of storm water control efforts and address any deficiencies or additional requirements in the SWPPP. Specific issues for the team to address include the following: • Provide assistance for developing and maintaining the SWPPP; 0 Update significant materials list; • Update SWPPP incident reporting, inspection, and record keeping procedures; • Review environmental incidents; • Continue and improve SWPPP training for facility personnel; and a Evaluate the overall effectiveness of the SWPPP Plan. The Storm Water Pollution Prevention Team shall meet at least once a year to perform a comprehensive review of the SWPPP and facility operations. • 3.0 FACILITY DESCRIPTION AND OPERATIONS The City of Oxford WWTP is located South of Oxford NC between Hwy 96 and I-85 off SR 1649 in Granville County. Stormwater run-off from the property will drain into an unnamed tributary of Fishing Creek. No vehicle maintenance is done at this facility. Materials handled at the facility include the following: • Lubricating oil used for equipment maintenance; • Waste oils generated from equipment maintenance; • Fuel for an emergency generator; • Laboratory chemicals. No reportable material spills have occurred at the facility. The land around the property and between the property and the creek is heavily vegetated. The majority of the property is a pervious grassed surface,Run-off will occur during high intensity rain events, but during light events storm water will permeate the soil. The few impervious surfaces consist of an office building, storage buildings, • treatment structures and an internal roadway. Outfall 001 Storm water runoff from the area around the plant headworks. Outfall 002 & Outfall 003 Storm water runoff from the former aeration basin, a portion of the roadway section, a portion of the sludge storage basins and the clarifiers. Outfall 004 Storm water runoff from the filters and digester # I . Outfall 005 Storm water runoff from the area around aerobic digester # 1. Outfall 006 Storm water runoff from the area around clarifiers #2 and #3. Drainage from all other than those leading to the described outfalls are natural features, and currently vegetated. Therefore, at this time, no additional outfall monitoring locations are established. 0 The City of Oxford has evaluated the drainage from its facilities and has determined that there is no non -storm water discharges connected to the storm drainage system. 4.0 STORM WATER MANAGEMENT CONTROLS • Good Housekeeping; • Preventive Maintenance; • Spill Prevention and Response Procedures; • Inspections; • Employee Training; • Record Keeping and Internal Reporting Procedures; • General Material Management Practices. Good housekeeping practices are an effective first step towards preventing the accidental exposure of materials that may have an adverse impact on storm water quality. Clean and orderly work areas reduce the possibility of accidental spills caused by mishandling of materials and equipment, and reduce safety hazards to facility personnel. Good housekeeping practices are described below. Floor and ground surfaces at all storage, shop, and work areas are maintained as appropriate. Floors should be swept clean, garbage and waste material disposed of on a regular basis. Spills will be cleaned up immediately, using necessary and appropriate equipment. If deemed necessary, contaminated soil should be excavated and disposed of properly. In the event of a spill, notify a member of the responsible party who will investigate and assess the spill. An evaluation should be conducted to determine the cause of the spill, if it could have been prevented, and how to prevent a similar spill in the future. Internal records should be kept for any spill that occurs at the facility. If a release reaches a surface water body or otherwise exceeds a reportable quantity, the North Carolina Division of Water Quality should be notified within 24 hours. The information to be reported to the regulatory agency must include the following: • The date and time of the spill; • An estimate of the quantity of material released and the time or duration of the event; • The exact location of the spill, including the name of the waters involved or threatened and/or other medium or media affected by the release or spill; • The source of the release; . • The name, address, and telephone number of the party in charge of, or responsible for, the facility or activity associated with the release; • The extent of actual and potential water pollution; • The party at the release site who is in charge of operations at the site and the telephone number of this party; and • The steps being taken or proposed to contain and clean up the spill and any precautions taken to minimize impacts. All personnel at the facility shall receive annual training as part of the SWPPP implementation. This training will be incorporated as a part of periodic environmental and safety meetings held at the facility. This training will consist of an overview of the SWPPP with an emphasis placed on spill response, good housekeeping and other storm water management controls. Specific areas to be addressed in this training include: • Identification of significant materials; • Good housekeeping; • Spill response and reporting; • Inspection procedures; • Sediment and erosion prevention; and • General material management practices. 5.0 Storm water Monitoring This section describes both visual and chemical monitoring of storm water discharges that must be performed in accordance with the general permit. Under this general permit, storm water runoff monitoring will be conducted at Outfall 001, 002, 003, 004, 005, and 006. Visual (qualitative) monitoring of storm water must be performed on a biannual basis for Outfalls 001, 002, 003, 004, 005, and 006. Monitoring must be performed once between April and June and once between September and November. The sampling must be visually observed for the following: • Color; • Odor; • Clarity; • Floating solids; • Suspended solids; • Foam; • Oil sheen; and 0 Other obvious indictors of storm water pollution. • 6.0 COMPREHENSIVE SITE COMPLIANCE EVALUATION As part of the SWPPP requirements, the facility will conduct semiannual Comprehensive Site Compliance Evaluations. These evaluations will assure that the facility is complying with the requirements and guidelines prescribed in the SWPPP and that the SWPPP reflects current conditions and operations at the facility. The site compliance evaluation consists of three main tasks: a semiannual inspection, correction of any deficiencies, and preparation of a summary report. Inspections are to be done once in the fall (September — November) and once during the spring (April — June). The Pollution Prevention Team shall designate an individual to conduct a semiannual inspection to assure the effectiveness of the SWPPP. The "Semiannual Site Compliance Evaluation" form summarizes the actions to be taken as part of this inspection and may be used as a checklist to complete the evaluation activities including the following: • The description of potential pollutant sources is up to date and accurate; • Areas of activity are inspected for signs of pollutants entering the storm drainage system. • The drainage map reflects current conditions; O The storm water management and controls listed in the SWPPP are implemented and effective; and Any changes to the list of potential pollution sources or to pollution prevention measures shall be incorporated into the SWPPP within 2 weeks of the evaluation. Any required changes shall be implemented within 12 weeks of the compliance evaluation. A semiannual site compliance report shall be prepared summarizing the results of the compliance evaluation. The information to be contained in the report should contain at least the following: • Personnel making the inspection; • Date(s) of the inspection; a Outline of inspection procedures; • Major observations relating to the implementation of the SWPPP; • Any non-compliance items; and Actions taken. This report shall be retained as a portion of the SWPPP for a period of at least three years. If the report finds there are no non-compliance issues, the report shall contain a certification that the facility is in compliance with the SWPPP and the report shall be signed by a member of the Stormwater Pollution Prevention Team. • The SWPPP will be reviewed and revised by the Pollution Prevention Team on a continuous basis, at a minimum annually following a site compliance evaluation. The SWPPP shall be revised immediately upon recommendations in the semiannual compliance report or under other circumstances listed above.
