This permit is reissued pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between the state of North Carolina and the U.S. Environmental Protection Agency, dated October 15, 2007 (or as subsequently amended.) Box 2263 Smithfield, North Carolina 27577 Subject: Compliance Evaluation Inspection Central Johnston County Regional WWTF NPDES Permit NCO030716 Stormwater Permit NCG110077 Johnston County Dear Mr. Broome: Dee Freeman Secretary Mitch Hayes of the Raleigh Regional Office conducted an inspection of the subject facility on April 28, 2009. The time and assistance of Dan Wall, Operator in Responsible Charge (ORC), William Denton, back-up ORC, Jennifer Doll and Chad Thompson were greatly appreciated. Below is a list of findings and recommendation developed from the inspection: The current permit was reissued and became effective October 01, 2007 and will expire September 30, 2012. The 7.0 MGD Wastewater Treatment Facility consists of the of the following units: influent pump station; two mechanical bar screens with screenings compactor, Parshall flume with ultrasonic flow meter; influent pump station with 2 variable speed and 1 constant speed pump; 2 treatment trains with anoxic and aeration basins; five secondary clarifiers; four RAS pump stations; methanol feed facilities; 5 deep bed de -nitrification filters; filter influent pump station wet well; filter influent pump station; one clearwell; ultraviolet disinfection facility; cascade aeration structure; one effluent ultrasonic flow meter; two filter backwash reclaimed water tanks; one reclaimed water contact/storage tank; one reclaimed water pump station; one aerobic digester/solids storage tank; one liquid waste solids transfer pump station; back-up chlorination basins; three on site generators for back up power. Construction to the waste treatment facility to increase the permitted capacity and flow design to 9.5 MGD is approximately 60% complete with a completion date scheduled April 25, 2010. At the time of inspection, the new influent splitter box had been formed, aeration basin number 7 was approximately 80% complete, clarifier number 6 Had been poured, reinforcing steel had been erected for the denitrification filter walls, wiring for the UV disinfection had been relocated to the UV lamps, the lime silo has been relocated to dewatering facility, and the 1.5 MG aerated sludge holding tank located at the dewatering facility was in service. At the time of inspection, one aeration basin was not in use due to regularly scheduled cleaning, clarifier number 5 was not in use due to low inflow, and the influent anoxic basin was not being used. The influent flow sampler was out of service due to on -site contractor cutting electrical service. Influent samples were being collected hourly and composited. Influent in the aeration was a light brown with minimal foaming. Dissolved oxygen is kept around 5.0 mg/i to compensate for the higher outside temperature. Weirs in the secondary clarifiers all appeared level. There were some areas were the v- notches on the weir plate had detached from the effluent launder causing some short-circuiting. Most of the weir plates in secondary 1,2,3 and 4 were rusted and could become a problem if attention is not given in the near future. Otherwise, the effluent flowing off the weirs appeared clear. Sludge blanket in secondary clarifier number 1 was 4 feet with a sidewall depth of 12 feet. There was no algae build up in the effluent trough and weirs. Supernatant was being pumped back to the head of the plant. The denitrification filters were being backwashed at the time of inspection. Effluent appeared clear in the clear well with little foaming. Effluent entering the Neuse River appeared clear with no foam or odor. 4. The laboratory was checked in a cursory manner. All instrument calibration logs are being maintained in logbooks. An operator's logbook was available and being maintained. Fecal coliform bath temperature was 44.5 degrees C, the drying oven temperature was 105 degrees C. BOD incubator was 20.0 C, and the main refrigerator was 1 degree C. All meters including influent and effluent flow meters were last calibrated March 26, 2009. Effluent flow sampler temperature was 4 degrees C. 5. Discharge Monitoring Reports for the period July 2008 to February 2009 were reviewed for compliance with permit limits and monitoring requirements. One frequency violation was noted for 09/2008 for the parameter Zinc. It was determined that the value for Zinc was transposed in another column. No action is necessary. 6. Commercial lab results, chain of custody records, and bench sheets were compared with data submitted on the Discharge Monitoring Report (DMR) for the month of February 2009. No discrepancies were noted. Stormwater Permit NCG110077 7. Permit NCG110077 was issued and became effective on June 01, 2008 and will expire on May 31, 2013. A review of the permit requirements was conducted after the CEi for NC003017. A copy of the permit was on hand and contained all permit requirements with the exception that the general location map list the receiving water (UT to Neuse River) as impaired or not and what the parameter(s) of concern are. 8. Qualitative monitoring is done according to the permit schedule. The last two monitoring sets were completed March 20, 2009 and August 7, 2008. A qualitative monitoring report supplement has been developed as guidance for rating stormwater discharge. One distinct stormwater outfall is listed on the 2003 stormwater map of the facility. Stormwater drains at the screenings area and at the sludge press area are routed back to the head of the plant. A Secondary containment wall is provided for the methanol and sodium hypochlorite tanks. The alum and magnesium sulfite silos have secondary containment and are empty and have never been used. The methanol tank is double walled and sensors monitor the tank for any leaks All other stormwater is routed to one common discharge pipe and discharges next to the wastewater treatment facility's effluent discharge pipe to an unlined channel that drains to the Neuse River. There was no stormwater discharge at the time of inspection. 10. Stormwater Pollution Prevention Plan was available during inspection. Staff training on spill response and prevention was completed on November 20, 2008. The time and assistance of Dan Wall, Operator in Responsible Charge (ORC), William Denton, back-up. ORC, Jennifer Doll and Chad Thompson were greatly appreciated. Below is a list of findings and recommendation developed from the inspection: 1: The current permit was reissued and became effective October 01, 2007 and will expire September 30, 2012. An Engineer's Certification was received in the Central Office June 06, 2010 certifying that the upgrade and expansion to the Central Johnston County Water Reclamation Facility was complete. Effluent limits associated with the modifications to expand to 9.5 MGD are effective June 06, 2010. 2. The 9.5 MGD Wastewater Treatment Facility consists of the of the following units: influent pump station with 2 variable speed and 1 constant speed pumps and the addition of a 1,453 gpm 20 Hp influent pump; 2 mechanical bar screens with screenings compactor, a new influent splitter box with Parshall Flume with ultrasonic flow meter; 2 treatment trains with anoxic and aeration basins and the addition of a 1.3MG aerobic tank (Aeration Basin #7); the addition of two 3,200 scfm, 170 Hp aeration basin blowers; a 15 Hp NRCY recycle window pump; 6 secondary clarifiers includes one new 105-foot diameter clarifier (Clarifier #6) with 14-foot side -water depth; 4 RAS pump stations that includes the addition of a 2,250 gpm, 30 Hp pump to RAS pump station #3; methanol feed facilities; 8 deep bed de - nitrification filters that includes three additional deep -bed denitrification filter bays (9.5' x 48' each) with one filter bed operational with filter media; a additional 2,280 scfm,140 Hp air scour blower; filter influent pump station wet well; filter influent pump station; 1 clearwell; ultraviolet disinfection facility with the addition of 2 series UV banks with each having peak flow of 5.625 MGD; cascade aeration structure; one effluent ultrasonic flow meter; two filter backwash reclaimed water tanks; 1 reclaimed, water contact/storage tank; one reclaimed water pump station; one aerobic digester/solids storage tank; one liquid waste solids transfer pump station; back-up chlorination basins; 5 sludge drying beds; 3 on -site generators for back up power. 3. At the time of inspection, secondary clarifiers 1, 2, 3, and 4 were not in use due to scheduled cleaning and refurbishing, aeration basins 6 and 7 were not being used due to cleaning, 4 out of 5 tertiary filters were in use, one is down to repair a broken air cap, one UV channel was being used due to low inflow, and there was no sludge in any drying bed. Influent in the aeration was medium brown with minimal foaming. Weirs in the secondary clarifiers all appeared level. Effluent flowing off the weirs appeared clear. Sludge blanket in secondary clarifier number 5 was 3 feet with a sidewall depth of 15 feet. There was no algae build up in the effluent.trough and weirs. One violation for not reaching minimum Dissolved Oxygen, 6.0 mgll, was recorded on 06/29/2010, 5.8 mg/l. There were no other violations noted for the review period. 6, Commercial lab results, chain of custody records, and bench sheets were compared with data submitted on the Discharge Monitoring Report (DMR) for the month of August 2010. One discrepancy for pH was noted for 08/23/2010. An amended DMR will be sent in to correct the data point. Stormwater Permit NCG110077 7. Sullins, Director Division of Water Quality Mitch Hayes of the Raleigh Regional Office conducted an inspection of the subject facility on July 24, 2008. The assistance of Dan Wall, Operator in Responsible Charge (ORC) and the staff of the wastewater treatment facility were appreciated. Below is a list of findings and recommendation developed from the inspection. The current permit was reissued and became effective October 01, 2007 and will expire September 30, 2012. The new permit includes the following changes: Total Nitrogen discharge limits have been increased to 67,467 lb/yr effective January 01, 2008, The annual Total Nitrogen load limit must be less than or equal to said limit or the Permittee is a Co-Penmittee Member of a compliance association. Total Nitrogen Allocations, active verses reserve, have been assigned to the permit. Calculation of Total Nitrogen loading has been modified slightly. There are no other significant changes made to the reissued permit. 2. The 7.0 MGD Wastewater Treatment Facility consists of the of the following: influent pump station; two mechanical bar screens with screenings compactor, Parshall flume with ultrasonic flow meter; influent pump station with 2 variable speed and 1 constant speed pump; 2 treatment trains with anoxic and aeration basins; five secondary clarifiers; four RAS pump stations; methanol feed facilities; 5 deep bed de -nitrification filters; filter influent pump station wet well; filter influent pump station; one clearwell; ultraviolet disinfection facility; cascade aeration structure; one effluent ultrasonic flow meter; two filter backwash reclaimed water tanks; one reclaimed water contact/storage tank; one reclaimed water pump station; one aerobic digester/solids storage tank; one liquid waste solids transfer pump station; back-up chlorination basins; three on site generators for back up power. The plant is currently undergoing construction to increase permitted capacity and design to 9.5 MGD. Additions to the plant include: 1,453 gpm, 20 Hp pump at the influent pump station; a new influent splitter box with Parshall Flume flowmeter; a 0.3MG anoxic tank; 1.3 MG aerobic tank with fine bubble diffusers; a 105-foot diameter clarifier with 14-foot side -water depth; a 2,250 gpm, 30 Hp pump to RAS pump station #3; three additional deep -bed denitrification filter bays; two (2) series UV banks with each having a peak flow of 5.625 MGD; two 3,200 scfm, 170 Hp aeration basin blowers and one 2,280 scfm, 140 Hp air scour blower. The Biosolids Facilities located at the Johnston County Landfill are also undergoing modifications. These include the installation of two (2) 320 gpm rotary lobe sludge transfer pumps; a 1.5 MG aerated sludge holding tank; a 1.5 meter belt filter press system including a 120-gpm rotary lobe filter press feed pump and polymer feed system; and relocation of the existing WWTP lime silo including installation of a new pug mill and auger conveyor. At the time of inspection, 85% of the excavation for the aerobic tank had been completed. Approximately 70% of the demolition to the deep -bed denitrification filter building had been completed. The prestressed concrete tank structure for the 1.5 MG aerated sludge holding tank has been completed. Concrete has been demoed for the filter press. 4. At the time of inspection, one aeration basin and two secondary clarifiers were not in operation due to low flow and cleaning. Basins are cleaned every six months to remove grit. Two out of three influent pumps were being operated due to low flow. One out of two banks of UV was being operated due to low flow. The two banks of UV light cycle on according to flow. All other units were in operation at the time of inspection. The influent in the aeration basin appeared tan in color. Dissolved oxygen is kept at 3.8 mgll in the front of the basin and 3.3 in the back of the basin. All diffusers were operating normally. Sludge judge reading in the 85-foot diameter secondary clarifier was 3 'h with a sidewall depth of 12 feet. The weirs appeared level with no drafting of solids. There was no indication of algae growth on the weirs or in the trough. The effluent flowing off the weirs appeared clear. The deep bed de -nitrification filters are manually backwashed every 48 hours and 24 hours before an anticipated large rain event. Effluent in the clear well appeared clear. The aerated digester at the county landfill was approximately one-half full. All diffusers were operational. Belt filter presses were operating normally. Sludge was being collected to be land filled. The influent pumps were turned off to test SCADA and the audible/visual alarms. The audible and visual alarms functioned when the influent wet well was allowed to fill to the high water level. SCADA reported a high wet well and power out. Emergency generators are operated every day for peak shaving and before an approaching storm. 6. The effluent in the cascade aerator and entering the Neuse River appeared clear with no evidence of solids, foam, or odor. Solids in the digester/solids storage tank are conveyed via force main to the Johnston County Landfill for dewatering. The supernatant from the sludge dewatering at the county landfill is conveyed back to the wastewater treatment head works via force main for processing. The dewatered solids are landfilled in accordance with permit WQ0020632. Reclaimed water is distributed under permit WQ0019632. Reuse water is pumped to the Johnston County landfill for irrigation at the Johnston County Landfill under Permit WQ0019631. Reclaimed, reuse water permits and other non - discharge permits are regulated by the Aquifer Protection Section (APS). All correspondence pertaining to these permits should be directed to the APS. 9. The laboratory was checked in a cursory manner. All instrument calibration logs are being maintained. All pH buffers were within expiration date. An operator's logbook was available and being maintained. The influent and effluent flow meters were calibrated February 21, 2008. All meters and instruments are calibrated by Milco Calibrations, LLC. Fecal coliform bath temperature was 44.5 degrees C, drying oven temperature was 103 degrees C, and the BOD incubator temperatures were 20.0 and 19.0 degrees C. 10. The wastewater treatment plant staff analyzes pH, temperature, dissolved oxygen, alkalinity, BOD, total phosphorus, total suspended solids, settleable solids, ammonia, and conductivity. Environment I and Mentech performs all other analyses. 11. Discharge Monitoring Reports for the period February 2007 to June 2008 were reviewed for compliance with permit limits and monitoring requirements. There were no violations for the period. 12. Commercial lab results, chain of custody records, and bench sheets were compared with data submitted on the Discharge Monitoring Report (DMR) for the month of April 2008. No discrepancies were noted. Stormwater Permit NCG 110077 13. Permit NCG110077 was issued and became effective on June 01, 2008 and will expire on May 31, 2013. The reissued permit contains the following changes: The site plan must include a general location map (or alternatively the site map) that identify whether each receiving water is impaired (on the state's 303(d) list of impaired waters) or is located in a watershed for which a TMDL has been established, and what the parameter(s) of concern are. A copy of the permit was on hand and contained all permit requirements with the exception of the new requirements noted above. 14. Qualitative monitoring and first sampling analyses for the stormwater permit was conducted on May 17, 2007 and May 09, 2008. There is no vehicle maintenance done on site. 15. One distinct stormwater outfall is listed on the 2003 stormwater map of the facility. Stormwater drains at the screenings area and at the sludge press area are routed back to the head of the plant. A Secondary containment wall is provided for the methanol and sodium hypochlorite tanks. The alum and magnesium sulfite silos have secondary containment and are empty and have never been used. The methanol tank is double walled and sensors monitor the tank for any leaks All other stormwater is routed to one common discharge pipe and discharges next to the wastewater treatment facility's effluent discharge pipe to an unlined channel that drains to the Neuse River. There was no stormwater discharge at the time of inspection. 16. Stormwater Pollution Prevention Plan was available during inspection. The assistance of Dan Wall, Operator in Responsible Charge (ORC), was appreciated. Below is a list of findings and recommendation developed from the inspection. 1. The current permit was reissued and became effective November 1, 2002 and will expire August 31, 2007. 2. The Class 4 Wastewater Treatment Facility has undergone major upgrades to the treatment process and after completion will provide tertiary treatment for a permitted flow capacity of 7.0 MGD. Mr. Dan Wall stated in a phone conversation on August 3, 2005 that the Engineer's Certificates of completion were sent in on August 1, 2005. Please note that expansion at or above 5 MGD will require 24-hour operation. New effluent permit limits for N113-N will also take effect at 7.0 MGD. 3. This office request that a narrative description of installed wastewater treatment components at the facility that includes sizes and capacities for each component and a schematic of the wastewater flow that identifies specific treatment components be sent to this office. Dan Wall stated that the plant would begin construction next year to expand the plant to 9.5 MGD. 4. One on -site generator was out of service and being repaired. The influent pump station is connected to the SCADA system. The high water alarm sounded when pumps I and 2 were turned off. SCADA reported a power out and high flow. The three on site generators are exercised every day under load for peak shaving. The grit chamber, EQ basin, sludge press, and two older clarifiers are not being used. The effluent in the cascade aerator and entering the Neuse River was clear with no evidence of solids, foam, or odor. 5. Liquid waste solids are conveyed via new force main to the Johnston County Landfill for dewatering. Solids are dewatered using two 1.5-meter belt filter presses and are stared in one of two solids storage basins until landfilled in accordance with permit WQ0020632. 6. Reclaimed water is distributed under permit WQ0019632. Reuse water is pumped to the Johnston County landfill for irrigation at the Johnston County Landfill under permit WQ0019631. Reclaimed, reuse water permits and other non -discharge permits are regulated by the Aquifer Protection Section (APS). All correspondence pertaining to these permits should be directed to the APS. 7. The laboratory was checked in a cursory manner. All instrument calibration logs are being maintained. All pH buffers were within expiration date. An operator's logbook was available and being maintained. 8. The wastewater treatment plant staff analyzes pH, temperature, dissolved oxygen, alkalinity, BOD, total phosphorus, total suspended solids, settleable solids, ammonia, and conductivity. Environment land Tritest analyses all other parameters. 9. Discharge Monitoring Reports for the period May 2004 to May 2005 were reviewed for compliance with permit limits and monitoring requirements. One weekly limit violation for total mercury was noted on May 31, 2005. This will be addressed in future correspondence. There were no other limit or monitoring violations for the period. 10. Commercial lab results, chain of custody records, and bench sheets were compared with data submitted on the Discharge Monitoring Report (DMR) for the month of February 2005. No discrepancies were noted. Stormwater Permit NCG 110077 11. Permit NCG110077 was issued and became effective on January 01, 2004 and will expire on April 30, 2008. A review of the permit requirements was conducted after the CEI for NC003017. The stormwater permit was not available for the inspection. One of the major components of the stormwater permit is that a Stormwater Pollution Prevention Plan (SWPPP) be developed. A SWPPP has yet to be developed. Minimum requirements for the SWPPP can be found in the permit in Part II Section A, items 1 through 9. Qualitative monitoring is required for each stormwater outfall twice a year, once in the spring (April -- June), and once in the fall (September — November). Analytical monitoring is required for facilities that have any vehicle maintenance activity occurring on site which uses more than 55 gallons of new motor oil per month when averaged over the calendar year. Requirements for the monitoring are specified in Table 2, Section C of the stormwater permit. •`Centrai Johnston County WWTP CEl Page t. ' 12. There have been no records kept for the stormwater permit since issuance January 01, 2004. Dan Wall stated that Jennifer Willis, stormwater coordinator, will be developing the SWPPP and that qualitative monitoring will begin this year. 13. At the time of inspection, there is one distinct stormwater outfall. Stormwater drains at the screenings area and at the sludge press area are routed back to the head of the plant. A Secondary containment wall is provided for the methanol and sodium hypochlorite tanks. The methanol tank is double walled and sensors monitor the tank for any leaks All other stormwater is routed to one common discharge pipe and discharges next to the wastewater treatment facility's effluent discharge pipe to an unlined channel that drains to the Neuse River. 'there was no stormwater discharge at the time of inspection. This report is being sent as Notice of Violation for Stormwater Permit NCG 110077 for failure to develop and implement a SWPPP within twelve (12) months of the effective permit issuance date. Please notify this office by September 08, 2005 when a SWPPP is developed.
