Pollution Prevention Plan (SWPPP) BMP Summary requested during the inspection conducted on
December 11, 2012 and in your letter dated January 15, 2013 observation 4 item 1. A copy of this
summary has been added to the Towns SWPPP and my employees have been trained on the proper
implementation of these BMP's. Thank you for your help and suggestions during the 2012 inspection. If

Dear Mayor Pernell:
Enclosed please find a copy of the Compliance Evaluation Inspection form for the stormwater inspection
conducted by Mack Wiggins and of the Raleigh Regional Office on December 11, 2012. The assistance offered
by Mr. Jimmy Wayne Ellington was appreciated.
1. The following stormwater observations were noted:
The Stormwater General Permit Certificate of Coverage (NCG 1 10076) was issued to this facility on
June 1, 2008. The General Permit expires on May 31, 2013,
Louisburg Wastewater Treatment Plant has three stormwater outfalls. All outfalls were observed
during the inspection. Outfalls were assessable for sampling at the time of the inspection.
3. The facility does not perform vehicle maintenance activities on -site and does not use more than 55
gallons of new motor oil when averaged over a calendar year. Therefore, only qualitative monitoring
is required. Qualitative monitoring was conducted as required in NCG 110000. Sampling conducted
by the facility.
4. The Stormwater Pollution Prevention Plan (SPPP) has been implemented. The plan contained all
items required by Part 11 Section A with the exception of the following:
1. Include a BMP summary in your plan (SPIT).

I, Dave Parnell of the Raleigh Regional Office of the Division of Water Quality conducted two compliance
evaluation inspections (CEI) on December 1, 2011, consisting of the NPDES wastewater and stormwater
inspections. The assistance of Jimmy Ellington, Operator in Responsible Charge (ORC) was helpful in
facilitating the inspection. Please review the attached Basinwide Information Management System (BIMS)
inspection checklists summarizing the inspections.
Below is a list of findings and recommendations developed from the inspections:
1. NPDES Wastewater Inspection:
1. The current permit was reissued April 1, 2010 and will expire September 30, 2014.
2. The 1.37 MGD wastewater treatment plant consists of the following units: mechanical bar screen,
aerated grit/grease removal, anaerobic selector, dual oxidation ditches (BioDeniopho Process) duel
secondary clarifiers, three cell tertiary filter, ultraviolet disinfection, cascade aeration, polymer feed
system for sludge thickening, aerobic digesters and ultrasonic flow meter.

Town -of Louisburg Wastewater Treatment Plant NPDES Wastewater and Stormwater CEI
The inspection began in the treatment plant office. All paperwork was well organized and easily
accessible. Discharge Monitoring Reports were reviewed for compliance with permit limits and
monitoring requirements. Chain of custody records were available and found to be in order. Tile
laboratory is state certified and performs the majority of permit required parameters. A review of
field laboratory criteria was discussed with lab personnel. A contract lab, Tritest, performs the
remainder of the parameters required by the permit.
4. Overall maintenance was found to be in good condition. The mechanical bar screen was in good
condition. Grit removal system appeared to be operating properly. Debris is removed as needed and
taken to the landfill.
5. The Kruger anaerobic selector component was observed and discussed with Mr. Ellington. The
secondary clarifiers were functioning properly, were free of weir blockages and showed no sign of
short-circuiting. Coverage of the periphery of the clarifiers by metal sheets has cut down on algae
growth and continues to be an improvement to the clarifying process. The sludge blanket was
reported to be 2 feet with a sidewall depth of 15 feet.
6. Biological nutrient removal is conducted at the facility. The Louisburg Wastewater Treatment Plant
is a member of the Tar -Pam Association and is compliant with its nutrient requirement.
7. The tertiary filters and ultraviolent disinfection appeared to be functioning properly. Additional
bulbs are on hand. The need for a backup to the disinfection system was discussed with Mr.
Ellington.
S. The aerobic digester have adequate capacity with no objectionable odor. The use of an air jammer
has been implemented in one digester and a second one will be installed in the future. One of the
two digesters had been emptied and cleaned out. There were no objectionable conditions found with
the digesters.
9. Standby power, consisting of three generators, is available and is tested on a monthly basis. The
generators have the capacity to run the entire wastewater treatment plant.
10. The effluent pipe is located adjacent to the plant. The effluent discharging into Tar River was
observed during the inspection, and found to be clear with no foam present in the River

II. [NPDES Stormwater Inspection:
1. The Stormwater pollution prevention plan (SPPP) was reviewed and found to be in compliance with
the permit. All regular inspections and duties, including annual review, qualitative monitoring,
knowledge and inspections of secondary containment are being accomplished as required by the
stormwater permit.
The Town of Louisburg Wastewater Treatment Plant is considered to be in compliance with the NPDES
wastewater permit, as well as the NPDES general stormwater permit, at the time of these inspections.
Please continue your diligence in the proper operation and maintenance of the facility. If you have

knowledge and inspections of secondary containment are being accomplished as required by the
stormwater permit.

On November 18, 2008, Vicki Webb of the Raleigh Regional Office conducted a compliance evaluation
inspection of the subject facility. The assistance of Jimmy Ellington, superintendent and operator in responsible
charge (ORC), was appreciated as it facilitated the inspection process.
The following observations were made:
I . The Town of Louisburg WRF is permitted to discharge at a rate of 1.37 MGD and consists of: manual
bar screen, aerated grit and grease removal, flow measurement, anaerobic selector, dual oxidation
ditches, dual secondary clarifiers, three cell tertiary filter, ultraviolet disinfection, cascade aeration,
sludge thickener, two aerobic sludge digesters and a backup power generator. The facility discharges to
the receiving waters designated as the Tar River, a class V water supply (WS-V), nutrient sensitive
water (NSW), in the Tar Pamlico River Basin.
2. The facility's permit and authorization to discharge shall expire at midnight on September 30, 2009,
Please refer to your permit in Part If, Section B, paragraph 10 in reference to your permit renewal.
3. Please ensure the plant description for the permit is updated to include the cascade aeration in your
permit renewal.
4. It was noted during the inspection that the facility is working on installing some new equipment. The
new equipment consist of: mechanical bar screen, septic tank hauler receiving station, spare digester
ejector pump, portable self priming pump, influent and effluent refrigerated samplers, sludge thickener
pumps, clarifier effluent trough cover plates, and replace the mixers in the oxidation ditch. Keep this
office informed of the progress of the installation of this equipment.
5. A review of the Discharge Monitoring Report (DMR) data for the 12-month period from August 2007
through July 2008 was performed prior to this inspection. The staff of this facility should be commended
for not having any permit limit or monitoring violations in this 12-month time period. ; Icto arolina

6. DMRs for August 2008 was reviewed and compared to lab reports. No mistakes or inconsistencies were
noted. Chains of custody were complete and kept on file as required. The annual compliance report was
reviewed and found to be complete and acceptable.
7. The plant and laboratory was very clean and well maintained. No operational or maintenance issues
were noted.
8. Visitation and operations logs were reviewed and found to be complete. It is recommended that the ORC
start keeping a separate operator logbook. Where they document certain functions related to the
operation of the wastewater system. The logbook is not only for documenting visitation requirements
but is also a place to note any unusual circumstances, preventative maintenance, etc. This was discussed
with the ORC and agreed upon.
9. The plant effluent was free and clear of solids or any other pollutant indicators. The receiving stream
looked healthy and showed no adverse conditions and right-of-way (ROW) to the outfall was well
maintained.
10. A compliance evaluation inspection was also conducted of the facility's Stormwater permit NCG110076
during the November 181h visit. All of the facility's stormwater outfalls were observed during the
inspection. The ROW to the outfalls was well maintained.
11. The stormwater pollution prevention plan (SP3) was reviewed. It contained all the required components
and met all requirements. Qualitative and quantitative inspection records were available for review and
found to be complete and acceptable.
The overall conditions of the.Town of Louisburg WRF are compliant with Division standards. If you have

I conducted a compliance evaluation inspection of the Louisburg water reclamation facility (WRF) on 20
June 2007. Jimmy Ellington, Operator in Responsible Charge (ORC) assisted me with the inspection.
His assistance and cooperation during the inspection were greatly appreciated. Inspection findings are
summarized below and an inspection checklist is attached for your records.
Louisburg's WRF discharges into receiving waters known as the Tar River, this section of which is
currently classified as a class V water supply (WS-V), nutrient sensitive water (NSW), in the Tar
Pamlico river basin. The plant consists of a manual bar screen, aerated grit and grease removal, flow
measurement, anaerobic selector, dual oxidation ditches, dual secondary clarifiers, three cell tertiary
filter, ultraviolet disinfection, cascade aeration, sludge thickener, two aerobic sludge digesters and a
backup power generator. Please ensure the plant description for the permit is updated to include the
cascade aeration in the next permit renewal cycle.
The plant was generally very clean and well maintained. All units were operational except one aerator
that had been removed from the western oxidation ditch for bearing replacement. This did not adversely
affect plant operation however.
It is noted that the plant still uses a manual bar screen. The town should seriously consider replacing this
unit with a mechanical bar screen to save man hours, remove safety hazards and prevent possible sewer
overflows at the plant caused by a clogged bar screen. The long-term benefits of replacing the manual
screen with a mechanical unit far outweigh the capital costs of installing the mechanical unit. This has
been an issue at the plant for many years now.
Plant records were reviewed. Discharge monitoring reports (DMR) were compared with lab records and
no inconsistencies were noted. Lab records were complete and met all requirements. The coliform and
BOD incubators were set at the correct temperatures, although a contract lab now runs the BODs for the
plant. DMRs were reviewed at the Raleigh regional office and there have been no limit violations over
the last 12 months. The composite samplers are set up for flow proportional sampling and the units were
set to the correct temperatures. Flow meters were calibrated as required.

The plant operations logs and operator's visitation logs were up to date and met requirements. The town
has not completed an Annual Performance Report (APR) as required in Part 11, Section E, paragraph 12
of your permit. Please' ensure this requirement is completed as soon as possible and that it is
accomplished annually from this point forward. Operations records are kept at the plant for at least three
years and lab data for at least five years.
It was noted that a polymer feed system for a sludge composting program was being installed at the plant
at the time of the inspection. Although we have decided that an authorization to construct (AtQ permit
was not -required to authorize this work, please include in your reply to this letter, a brief description of
the system being installed and'its purpose, as well as ensure that this new component is added to the plant
description in the next permit renewal cycle.
There were no other issues noted during the inspection. The effluent of the plant was clear and free of
foam and other debris. The plant is operated and maintained in a very professional manner.
Inspection of the town's stormwater management program associated with stormwater permit number
NCG110076 was also conducted on 20 June. The stormwater pollution prevention plan (SP3) contained
all the required components and all qualitative monitoring is being accomplished and recorded as
required. The town is not required to perform any analytical testing of the stormwater discharge(s).
However, the town is not reviewing the SP3 and making necessary changes or annotating annual
stormwater training as required by the stormwater permit. The town must begin reviewing the SP3
annually and making any changes necessary to keep the plan accurate and current. These reviews and
the results of the reviews, i.e. whether any changes were necessary, must be annotated in the SP3.
Additionally, NCG110076 also requires the town to annually train its employees at the plant on
stormwater management, Please begin annotating this training in the SP3 immediately.

are forwarding herewith the subject certificate of coverage to discharge under the subject state - NPDES
general permit. This permit is issued pursuant to the requirements of North Carolina General Statute
143-215.1 and the Memorandum of Agreement between North Carolina and the US Environmental
Protection agency dated May 9, 1994 (or as subsequently amended).
If any parts, measurement frequencies, or sampling requirements contained in this permit are
unacceptable to you, you have the right to request an individual permit by submitting an individual
permit application. Unless such demand is made, this certificate of coverage shall be final and binding.
Please take notice that this certificate of coverage is not transferable except after notice to the
Division of Water Quality. The Division of Water Quality may require modification or revocation and
reissuance of the certificate of coverage.
This permit does not affect the legal requirements to obtain other permits which may be required
by the Division of Water Quality or permits required by the Division of Land Resources, Coastal Area
