The assistance of Steve Proctor, David Paul Allen and you with this inspection was gratefully appreciated. Please find the attached Basin Wide Information Management System (B1MS) inspection checklist summarizing each inspection. Below is a list of recommendations and findings developed from the inspection: Permit NCO020389 was reissued 03,15,2010 and became effective 04.01,2010. The permit will expire 04.30.2013. Permit renewal is required within180 days prior to expiration date. 2. The Basinwide Information Management System (BIMS) currently lists Martin Morris as grade WW-4 Operator in Responsible Charge (ORC) and Steve Proctor grade WW-4 back-up ORC (bORC). You stated that David Paul Allen will become the ORC when he completes the requirements for a grade WW-4 operator. If you have any questions concerning ORC or b-ORC designation, please contact Steve Reid in the Technical Assistance Certification Unit at 919.733.0026 extension 314. 3. The 1.9 MGD wastewater treatment facility consists of the following units: 2 concrete lined equalization basins, arc bar (mechanical bar screen), grit chamber, 2 manual bar screens, influent Parshall flume, influent ultrasonic flow meter, 3 screw pumps, 3 oxidation ditches (1 used for nutrient removal), 5 secondary clarifiers (2 @ 10' depth, 2 @ 11' depth, 1 @ 14' depth), 5 deep bed tertiary filters, dual chlorine contact chamber with gaseous chlorine, post aeration tank, effluent flow meter with ultrasonic sensor using a 2-feet rectangular weir with end contractions, sodium metabisuifite dechlorination, an effluent reuse system providing irrigation to three athletic fields at the South Johnston County High School, an effluent reclaim water spray irrigation system that can apply up to 1.97 MG per year on the 1.65 acres of land beside the oxidation ditch number 3, and a 3 MG concrete lined aerated sludge basin with 3 floating aerators. At &f 'time-of•inspectiorr;-one out of three screw pumps were being operated due to low inflow. The two screw pumps not being operated did function when manually turned on. All brush rotors in the three oxidation ditches were being operated on a time delay and at variable speed for biological nitrogen and phosphorus removal. The number 4 secondary clarifier was not in service due to low inflow. There was no effluent flowing over the weirs due to low flow in secondary clarifier numbers 1, 2 and 3. Effluent flowing off the weirs in secondary clarifier 5, appeared clear with no pin floc or solids. Very little algae were noted in all the secondary clarifiers. Sludge blanket in the # 2 clarifier (10 foot sidewall depth) was 1 foot. Sludge blanket in the #5 clarifier (14 foot sidewall depth) was 1 '/2 feet. The number 4 tertiary filter was out of service due to a broken hydrocone base. This was noted in last year's inspection. You stated that the hydrocone base will have to be handmade. All other tertiary filters were in service. Effluent in the chlorine contact tank, dechlorination post aeration tank, and at the outfall appeared clear with no solids. There was no discharge from the new effluent discharge outfall due to low flow: Freeboard level in the EQ basin was 1.90 feet. 5. The Town of Benson has a contract with McGill Environmental System of NC, Inc. to dispose of 25 dry tons sludge per year under McGill's 503 permit WQ0006816 at their Delway, NC facility. 1.4 MG of sludge was removed from the 3 MG aerated sludge basin in 2010. 1.5 MG of sludge is scheduled to be removed this year. 6. The laboratory was checked in a cursory manner. All instrument calibration logs are being maintained on computer. An operator's logbook was available and being maintained. Fecal coliform bath temperature was 44.6 degrees C, the drying oven was 104 degrees C, and the laboratory refrigerator was 3 degrees C. All meters were calibrated 12.21.2010 by Johnston Controls Inc. Influent sampler temperature was 13.5 degrees C and the effluent sampler temperature was 4.8 degrees C. I was informed by David Allen on July 15, 2011 that the influent refrigerator had been repaired and is cooling properly. 7. Discharge Monitoring Reports (DMR's) for the period September 2009 to April 2011 were reviewed for compliance with permit limits and monitoring requirements. Two violations were listed for the review period: exceeding weekly average BOD on October 30, 2010 and failing to monitor for mercury on April 30, 2011. You stated that data for mercury was inadvertently left off the DMR and that an amended DMR was submitted. The October 30, 2010 violation has been addressed. Stormwater Permit NCG110062 8. Inspection of the stormwater outfalls and plant site was conducted at the same time as the CEi for the wastewater treatment plant. Permit NCG110062 was reissued and became effective on June 01, 2008 and will expire on May 31, 2013. Permit renewal is required within180 days prior to expiration date. 9. There are two stormwater outfalls listed for the facility: a. Outfall 001 consists of drainage from the office/laboratory building and surrounding roadway drainage that is conveyed by concrete pipe to a drainage structure near the secondary clarifiers. This combined flow drains to a swampy area near the influent flow structure. There was no discharge at the time of inspection. b. Stormwater from roof drainage, the building housing the reuse water tank and sodium metabisulfate tank, and the shelter housing the chlorine gas tank is conveyed to the EQ basin via underground piping. Stormwater use to discharge onto the ground near the fence behind the building housing the reuse water tank and sodium metabisulfite tank, former Stormwater Discharge Outfall (SDO) #2. Outfall 003 consists of all other stormwater drains that flow to one common stormwater pipe that discharges to Hannah Creek. The outfall is next to the effluent discharge pipe. Page 3 It appeared that there was no discharge at the time of inspection though the pipe was submerged into the water. 10. Qualitative monitoring was last documented for SDO's 1 and 3 August 2010. There was no documentation of qualitative monitoring for SDO's 1 and 3 for Period 2, Year 3. Employee training on proper spill response and clean-up procedures and preventative maintenance activities is not being documented. Annual certification that all stormwater outfalls have been evaluated for the presence of non-stormwater discharges is not being completed. The last update to the site map was in 2003. The map does not include the new reclaimed water spray field adjacent to oxidation ditch # 3 and still lists 3 SDO's. Please note that failure to abide by the conditions and limitations contained in the permit may subject the Permittee to a civil penalty up to $25,000 per day per violation. The # 2 oxidation ditch is being operated as a de-ox zone. The #1 and #3 oxidation ditches were in service. There was very little effluent flowing over the weirs in the secondary clarifiers. All weirs were free of algae. All other tertiary filters were in service. Effluent flowing into the post aeration tank appeared clear. Effluent discharging into Hannah Creek / swamp appeared clear with no visible solids or foam. Level in the sludge storage lagoons was down approximately three feet. Level in the equalization lagoon was down approximately two feet. 4. Upgrade to increase the wastewater treatment plant permitted capacity to 1.97 MGD has been completed. The following items were constructed during the upgrade: ■ A new 14 feet sidewall depth, 15 feet in diameter secondary clarifier, mixed liquor effluent collection box and a new flow splitter box downstream of the existing oxidation ditches. ■ Sidewalls for the influent structure, grit chamber, mechanical bar screen, diversion box, and existing secondary clarifiers and the oxidation ditches sidewalls have been raised above the 100-year flood plain. ■ The existing 4-7.5 hp brush rotor drives in oxidation ditches # 1 and #3 were replaced with 4-15 hp drives. Oxidation ditches # 1 and 93 contain one new 15 hp rotor with new brushes. • All three -filter lift pumps have been upgraded and expanded to 30 hp each at 2300 gpm. ■ Two existing screw pumps have been replaced with two new 5 hp 1,750 gpm screw pumps. ■ A new 30" D.I. parallel effluent outfall line has been installed and is in use. The new outfall line is 5 feet offset from the existing 21" effluent outfall line. ■ A new yard hydrant and water meter for the reclaim water irrigation system has been installed and in use. The effluent reclaim water spray irrigation system will apply up to 1.97 MG per year on the 1.65 acres of land beside the oxidation ditch number 3. 5. The Town of Benson land application permit WQ0012514 expired August 31, 2007. Even thought the permit has expired, the permit is still considered to be available. A request to rescind the permit should be made to the close out the permit. Please mail your request to Land Application Unit, 1636 Mail Service Center, Raleigh, NC 27699-1636. According to Martin Morris, money has been budgeted for the 2008-2009 physical year to remove sludge in the 3 MG aerated sludge basin. Atlantic Dewatering Inc. will dewater the sludge and take the solids to McGill Environmental Systems Of N C in New Hill. All permits will be obtained from Atlantic Dewatering Inc. The aerated sludge basin was last pump out in 2003 by Atlantic Coast Contracting, Inc. 6. The laboratory was checked in a cursory manner. All instrument calibration logs are being maintained on computer. An operator's logbook was available and being maintained. Fecal coliform bath temperature was 44.5 degrees C, the drying oven temperature was 105 degrees, and laboratory refrigerator was 2.1 degrees C. The influent and effluent flow meters were calibrated October 10, 2008 by Johnston Inc. Influent sampler temperature was 1 degrees C and the effluent sampler temperature was 3 degrees C. 7. Discharge Monitoring Reports (DMR's) for the period October 2007 to September 2008 were reviewed for compliance with permit limits and monitoring requirements. Flow was exceeded for April 2008. The exceedence was less than 10% of the permit limit; no further action is needed. There were no other violations for the review period. 8. Commercial lab results, chain of custody records, and bench sheets were compared with data submitted on the DMR for the month of September 2008. No discrepancies were noted. Permit NCG110062 was reissued and became effective on June 01, 2008 and will expire on May 31, 2013. 10. Expansion to the wastewater treatment plant has been completed. The site plan that is dated 2003 should be updated to include the upgrades made to the treatment plant. 11. There are three outfalls listed for the facility: a) Outfall 001 consists of drainage from the office/laboratory building and surrounding roadway drainage that is conveyed by concrete pipe to a drainage structure near the secondary clarifiers. This combined flow drains to a swampy area near the influent flow structure. Water in the swamp area appeared clear. b) Stormwater from roof drainage, the building housing the reuse water tank and sodium metabisulfate tank, and the shelter housing the chlorine gas tank is conveyed to the stabilization lagoon through underground concrete pipe. This flow was thought to have discharged offsite and is currently designated Outfall 002. This outfall is not necessary and until a request is made to rescind Outfall 002 from the permit, all monitoring for Outfall 002 should continue. c) Outfall 003 consists of all other stormwater drains on the property that flow to Hannah Creek. The outfall is next to the effluent discharge pipe. It appeared that there was no discharge at the time of inspection though the pipe is half submerged into the water. 12. Qualitative monitoring for Outfalls 001 and 003 was conducted on February 01, 2008 and September 26, 2008. There is no vehicle maintenance done on site. 13. There have been two bypasses for the 2007-2008 year. One by-pass occurred on 05/31/2008 that resulted in 500 gallons released into Hanna Creek. The other by-pass occurred on 08/31/2008 that resulted in 48,000 gallons released into Hanna Creek. Both bypasses were caused by equipment failure. The assistance of Martin Morris grade WW-4 Operator in Responsible Charge (ORC) and Steve Proctor grade WW-4 back-up ORC was gratefully appreciated. Please find the attached Basin Wide Information Management System (BIMS) inspection checklist summarizing each inspection. Below is a list of recommendations and findings developed from the inspection: The subject permit expired April 30, 2008 and is currently under review. Please continue to operate under the last issued permit until a new permit is issued. 2. The 1.9 MGD wastewater treatment facility consists of the following units: 2 concrete lined equalization basins, arc bar (mechanical bar screen), grit chamber, 2 manual bar screens, influent Parshall flume, influent ultrasonic flow meter, 3 screw pumps, 3 oxidation ditches (1 used for nutrient removal), 5 secondary clarifiers (2 @ 10' depth, 2 @ 11' depth, 1 @ 14' depth), 5 deep bed tertiary filters, dual chlorine contact chamber with gaseous chlorine, post aeration tank, effluent flow meter with ultrasonic sensor using a 2-feet rectangular weir with end contractions, sodium metabisulfite dechlorination, an effluent reuse system providing irrigation to three athletic fields at the South Johnston County High School, an effluent reclaim water spray irrigation system that can apply up to 1.97 MG per year on the 1.65 acres of land beside the oxidation ditch number 3, and a 3 MG concrete lined aerated sludge basin with 3 floating aerators. 3. At the time of inspection, two out of three screw pumps were being operated. The third screw pump operates when there is a high inflow and did operate when the high water float was lifted up. All inflow flows to the #2 oxidation ditch for denitrification (anoxic zone). Flow moves from the middle of the anoxic zone to the outer 2 aerobic ditches for BOD removal and nitrification. All brush rotors, except one in oxidation ditch # 3, were in service. The bearings in the out of service rotor were being replaced and should be back in service within seven days. There was a moderate amount of algae build up on the weirs in the 4 peripheral feed clarifiers. The center feed clarifier had no algae growth on the weirs. This office highly recommends a more frequent cleaning schedule to prevent algae blockage on weirs. Sludge blanket in the # 1 clarifier (10 foot sidewall depth) was 1 foot. Sludge blanket in the #4 clarifier (11 foot sidewall depth) was one -quarter foot. Effluent appeared clear flowing off all weirs in all 5 clarifiers. The number 4 tertiary filter was out of service due to a broken hydrocone base. The number 2 tertiary filter was full of effluent due to plugged drain valves. The effluent is pumped to the other operating tertiary filters for clarification. Effluent in the chlorine contact tank, dechlorination post aeration tank, and at the outfall appeared clear with no solids. Freeboard level in the EQ basin was 2.85 feet. 4. The Town of Benson Land Application Permit WQ0012514 was rescinded June 3, 2009. Marty Morris stated that the Town will hire McGill Environmental Systems of NC, Inc. to compost the wastewater treatment plant's biosolids at McGill's Delway, NC composting facility under their W00006816 composting and distribution program. Atlantic Dewatering will dewater the sludge at the wastewater treatment plant in December 2009. Sludge was last removed from the 3 MG aerated sludge holding basin in 2003. 5. The laboratory was checked in a cursory manner. All instrument calibration logs are being maintained on computer. An operator's logbook was available and being maintained. Fecal coliform bath temperature was 44.3 degrees C and the laboratory refrigerator was'2.7 degrees C. The influent and effluent flow meters were calibrated October 10, 2008 by Johnston Inc. Influent sampler temperature was 2 degrees C and the effluent sampler temperature was 4 degrees C. All meters were calibrated by Johnston Inc. on 10/1312009. 6. Discharge Monitoring Reports (DMR's) for the period October 2008 to August 2009 were reviewed for compliance with permit limits and monitoring requirements. There were no violations listed for the review period. 7. Commercial lab results, chain of custody records, and bench sheets were compared with data submitted on the DMR for the month of August 2009. No discrepancies were noted. Stormwater Permit NCG110062 8. Inspection of the stormwater outfalls and plant site was conducted at the same time as the CEI wastewater treatment plant. Permit NCG110062 was reissued and became effective on June 01, 2008 and will- expire on May 31, 2013. 9. The Stormwater Pollution Prevention Plan (SWPPP) is reviewed annually. The SWPPP is not compliant with Part II, Section A, 1 (a) that require the site plan include a general location map (or alternatively the site map) that identify whether each receiving water is impaired (on the state's 303(d) list of impaired waters) or is located in a watershed for which a TMDL has been established, and what the parameter(s) of concern are. Outfall 001 consists of drainage from the office/laboratory building and surrounding roadway drainage that is conveyed by concrete pipe to a drainage structure near the secondary clarifiers. One-half of one oxidation is being operated as a de-ox zone. A moderate amount of algae growth was noted on the secondary clarifier weirs. This was causing some blockage of weirs and creating uneven flow over the weirs. Sludge judge reading for secondary clarifier # 1 was 1 foot with a sidewall depth of 10 feet. Sludge judge reading for secondary clarifier # 3 was 2 % feet with a sidewall depth of 12 feet. Effluent flowing into the post aeration tank appeared clear. Effluent discharging into Hannah Creek / swamp appeared clear with no visible solids or foam. 4. Upgrade to the increase the wastewater permitted capacity to 1.97 MGD has begun. Items noted during the inspection included the following: r7, Construction of new 14 feet sidewall depth, 15 feet in diameter secondary clarif er, mixed liquor. effluent collection box and a new flows litter box downstream of the existing .._. �.�.. , ... - oxidation ditches were under way. ■ Sidewalls for the influent structure, grit chamber, mechanical bar screen, and diversion box have been raised above the 100-year flood plain. The existing secondary clarifiers and the oxidation ditches sidewalls will be raised in 2008. ■ The existing 4-7.5 hp brush rotor drives in oxidation ditches # I and #3 will be replaced with 4-15 hp drives. Oxidation ditches # 1 and #3 will also contain one new 15 hp rotor with new brushes. As already noted, oxidation ditch #1 was out of service for the construction of the new rotor platform. ■ All three -filter lift pumps will be upgraded and expanded to 30 hp each 2300 gpm. ■ Two existing screw pumps will be replaced with two new 5 hp 1,750 gpm screw pumps. ■ A new 30" D.I. parallel effluent outfall Iine has been installed and is in use. The new outfall line is 5 feet offset from the existing 21" effluent outfall line. ■ A new yard hydrant and water meter for the reclaim water irrigation system has been installed. When in operation, the effluent reclaim water spray irrigation system will apply up to 1.97 MG per year on the 1.65 acres of land beside the oxidation ditch number 3. Sludge in the 3 MG aerated sludge basin is allowed to settle and the supernatant is withdrawn and pumped back into the headworks for reprocessing. The aerated sludge basin was last pump out in 2003 by Atlantic Coast Contracting, Inc. The Class B sludge is disposed of in accordance with permit WQ0012814. 6. The laboratory was checked in a cursory manner. All instrument calibration logs are being maintained on computer. An operator's logbook was available and being maintained. Fecal coliform bath temperature was 44.6 degrees C and the drying oven temperature was 106 degrees. The influent and effluent flow meters were calibrated October 07, 2007. Influent sampler temperature was 2 degrees C. and the effluent sampler temperature was 3 degrees C. 7. Discharge Monitoring Reports (DMR's) for the period August 2006 to September 2007 were reviewed for compliance with permit limits and monitoring requirements. Total residual chlorine permit limit was exceeded by 5% on 08/18/06. Flow was exceeded by 6% of the permit limit for November 2006. These issues have been resolved and no further action is needed A pH value of 4.89 was recorded on 08/16/2007. This low pH value was found to be in error and an amended DMR will be sent in with the correct value. S. Commercial lab results, chain of custody records, and bench sheets were compared with data submitted on the DMR for the month of September 2007. No discrepancies were noted. Qualitative monitoring for Outfalls 001 and 003 was conducted on March 16, 2007 and July 11, 2007. There is no vehicle maintenance done on site. There have been no spills for the 2006-2007 year. Below is a list of recommendations and findings developed from the inspection: 1. The subject permit was reissued and became effective March 01, 2006 and will expire April 30, 2008. This permit included a modification to waive instream monitoring in Iight of participation in the Lower Neuse Basin Association. 2. BIMS lists Steve Proctor; grade WW-4, as back-up Operator in Responsible Charge (b-ORC) for the Class 4 Wastewater Treatment Plant. BIMS does not list any ORC for the WWTP. BIMS lists Martin Morris; grade WW-4, as the b-ORC for Erin WWTP # 2. At the time of inspection, one screw pump was out of service due to a broken gearbox and the mechanical grit chamber was not operating. One-half of one oxidation is being operated as a de- ox zone. The stilling well for the influent flow meter had a large volume of floating scum and algae. This build up causes the flow meter to give an inaccurate reading of the influent flow. The stilling well needs to be cleaned on more frequent bases. There was a moderate amount of algae build up on the secondary clarifier weirs. This alga was causing uneven flow over the weirs and some pin flock was noted. Sludge judge reading in the 10' and 12' secondary clarifiers was one- half foot each. One deep bed tertiary filter was out of service to repair a broken air lateral. The filter should be repaired this year. The effluent sampler hose had algae build up inside. The hose needs to be cleaned or replaced. Paul Rawls suggested that the sampler hose could be covered with a garden hose to block sunlight. The two equalization basins had more than two feet of freeboard. 6. The aerated sludge basin was last pump out in 2003 by Atlantic Coast Contracting, Inc. The Class B sludge is disposed of in accordance with permit WQ0012814. 7. The laboratory was checked in a cursory manner. All instrument calibration logs are being maintained. All pH buffers were within expiration date. An operator's logbook was available and being maintained. The influent and effluent flow meters were calibrated October 04, 2006. Fecal coliform bath temperature was 44.5.degrees C and the drying oven temperature was 105 degrees. Discharge Monitoring Reports for the period July 2005 to July 2006 were reviewed for compliance with permit limits and monitoring requirements. Permit limit violations for mercury were recorded for June 21 and 30, 2006. These violations will be addressed in future correspondence. It is noted that mercury is being reported in nanograms per liter (ng/l). Permit requires that mercury be reported in micrograms per liter (ugll). Please begin reporting mercury in ug/1. There were no other violations recorded for the review period. 9. Commercial lab results, chain of custody records, and bench sheets were compared with data submitted on the Discharge Monitoring Report (DMR) for the month of July 2006. No discrepancies were noted. 10. Effluent entering Hannah Creek appeared clear with no solids, foam or odor. Stormwater Pen -nit NCG 110062 11, Inspection of the stormwater outfalls and plant site was conducted at the same time as the CEI wastewater treatment plant. Permit NCG 110062 was issued and became effective on June 27, 2003 and will expire on April 30, 2008, 12. There are three outfalls listed for the facility: a) Outfall 001 consists of drainage from the office/laboratory building and surrounding roadway drainage that is conveyed by concrete pipe to a drainage structure near the secondary clarifiers. This combined flow drains to a swampy area near the influent flow structure. Water in the swamp area appeared somewhat cloudy most likely from previous Benson W WTP C61 Page 3 rain events. Mr. Morris stated that during severe rain events, water from the swamp area floods into the drainage structure and floods the area surrounding the secondary clarifiers. This office recommends that measures be taken to prevent water from flooding into the drainage structure. b) Outfall 002 consists of roof drainage from the building housing the reuse water tank and sodium metabisulfate tank and the shelter housing the chlorine gas tank. This flow drains offsite just past the fence. Water on the ground appeared muddy most likely from previous rains. c) Outfall 003 consists of all other stormwater drains on the property and flows to Hannah Creek. The outfall is next to the effluent discharge pipe. Water in the Hannah Creek appeared clear. 13. Qualitative monitoring for Outfalls 001, 002, and 003 was conducted on March 21, 2006 and October 19, 2006. There is no vehicle maintenance done on site. This permit application is for Stormwater Permit NCO110095 that appears to be the same as Stormwater Permit NCG110062. A Compliance Evaluation Inspection of the Benson Wastewater Treatment Plant, NCO020389 and Stormwater Permit, NCGI10062 was performed July 20, 2005. The facility was found to be compliant with all stormwater permit requirements. The Raleigh Regional Office has no objection to the issuance of NCO110095 in accordance to the Neuse River Basin Buffer Rules. A copy of the NCG110062 inspection summary is listed below. Grass covers the pipe and the effluent could not be seen. Water does not appear to be adversely affected. Outfall 2 drains the roof runoff from the building housing the reuse water tank and sodium bisulfate tank. There is no containment area to contain a possible spill. A 1400-pound of chlorine gas cylinder is stored under shelter next to this building. The drainage area flows off site just past the fence. There was no stormwater discharge. Outall 3 combines the other stormwater road drains and discharges into Hanna Creek next to the wastewater discharge outfall. Hanna Creek at the point of discharge is a swamp. The stormwater discharging into the swamp had no color, foam or odor. All records for inspections, employee training, spill response qualitative monitoring are maintained. Drains for the screenings area, sludge loading station, and dumpster areas are piped to the influent pipe. Outfall 1 drains road drains near the office/laboratory and parking lot to swamp. Grass covers the pipe and the effluent could not be seen. water does not appear to be adversely affected. Outfall 2 drains the roof runoff from the building housing the reuse water tank and sodium bisulfate tank. There is no containment area to contain a possible spill. A 1400-pound of chlorine gas cylinder is stored under shelter next to this building. The drainage area flows off site just past the fenceo . There was no stormwater discharge. Outall 3 combines the other stormwater road drains and discharges int Hanna Creek next to the wastewater discharge outfall. Hanna Creek at the point of discharge is a swamp. The stormwater discharging into the swamp had no color, foam or odor. All records for inspections, employee training, spill response qualitative monitoring are maintained. SwPPP is complete and spill response plan is up to date.
