In response to your renewal application for continued coverage under Stormwater General Permit
NCG 110000, the Division of Water Quality (DWQ) is forwarding herewith the reissued Certificate of Coverage
and stormwater General Permit. This permit is reissued pursuant to the requirements of North Carolina General
Statute 143-215.1 and the Memorandum of Agreement between the state of North Carolina and the U.S.
Environmental Protection Agency, dated October 15, 2007 (or as subsequently amended.) The following
information is included with your permit package:
• A new Certificate of Coverage
+ A copy of the stormwater General Permit
+ A copy of a Technical Bulletin for the General Permit
• Copies of the Discharge Monitoring Report (DMR) Form
• Copies of the Annual Discharge Monitoring Report Form (if applicable)
• Copies of the Qualitative Monitoring Report Form
The General Permit authorizes discharges of stormwater only, and it specifies your obligations with respect to
stormwater discharge controls, management, monitoring, and record keeping. Please review the new permit to
familiarize yourself with all changes in the reissued permit. Significant changes to the General Permit are
outlined in the attached Technical Bulletin.
Your coverage under the General Permit is transferable only through the specific action of DWQ. This permit
does not affect the legal requirements to obtain other permits which may be required by DENR, nor does it relieve
the perrnittee from responsibility for compliance with any other applicable federal, state, or local law, rule,
standard, ordinance, order, judgment, or decree.

Dear Mr. Pearce:
On November 27, 2012, Cheng Zhang of the Raleigh Regional Office (RRO) conducted a
compliance evaluation inspection of the subject facility. The assistance provided by you and Mr.
Bob Stewart, the wastewater superintendent and operator in responsible charge (ORC) of the
facility, was appreciated as it facilitated the inspection process. The inspection report is
attached. The following observations were made:
Triangle WWTP is permitted to discharge at a rate of 12.0 MGD and consist of: mechanical
fine screens, mechanical grit removal, Parshall flume, three 5-stage biological nutrient
removal (BNR) trains, four secondary clarifiers, five tertiary sand filters, post aeration
ultraviolet disinfection, sludge lagoons, waste and return activated sludge pumping system,
methanol, potassium hydroxide, and sodium hypochlorite storage and feed systems. The
plant discharges to Northeast Creek, a Class WS-IV NSW water in the Cape Fear River
basin.
All primary, secondary, and tertiary treatment and post -treatment processes, especially
the BNR system, were in good operational condition. One of the three BNR trains and
two of the four clarifiers and was not in use because of low flow. The chemical feed
systems for methanol, sodium aluminate, potassium hydroxide were all functioning well.
2. Sludge from the lagoon was removed recently. Construction of the sludge storage,
dewatering, and loading facility was slightly behind schedule at the time of inspection.
Once the construction is completed, the lagoon will serve as backup storage for wasted
sludge.
I Three standby power generators are tested under load monthly. The generators will be
activated automatically when primary power is out. The generators have capacity to
operate th�e0tire facility during nower outrage.

4, The ORC maintains a daily logbook. Lab results, chain -of -custody forms, and DMRs
were complete and current, kept in good order and ready for review. August 2012 DMR
data were compared to lab bench sheets; no data transcription errors were noted during
the inspection.
5. The right of way to the outfall was well maintained. The effluent was observed to be
clear and free of solids. No visible impacts were noted immediately downstream the
effluent pipe.
6. Field lab parameter (Certificate No. 671) calibration and analysis records were reviewed
during the inspection.
7. A compliance evaluation inspection for the NPDES stormwater permit (No.
NCGI 10054) was also conducted. The facility has developed and implemented a
Stormwater Pollution Prevention Plan (SPPP), which includes all required components.
All stormwater discharge outfalls were observed; these outfalls and the right-of-way were
well maintained. Please be reminded to conduct annual employee training and
second stormwater qualitative monitoring before the end of 2012.
The overall condition of Triangle WWTP is compliant with Division standards. If you have any

compliance evaluation inspection of the subject facility. The assistance provided by you and Mr.

facility, was appreciated as it facilitated the inspection process. The inspection report is
attached.. The following observations were made:

On January 7, 2010, Mrs. Mandy Hall and Ms. Vicki Webb of the Raleigh Regional Office conducted a
compliance evaluation inspection of the Triangle WWTP with the assistance of Mrs. Stephanie Brixey,
compliance manager, Mr. Tony Mencome, superintendent of the facility, Mr. Rob Stewart, operator in
responsible charge (ORC), and Mr, Joe Pearce, Utility Division Manager. Their help was appreciated as
it facilitated the inspection process.
The following observations were made:
I. The Durham County is authorized to discharge wastewater from the facility located at Durham
County Triangle WWTP, 5926 NC Highway 55, in Durham County. The permit expires April
30, 2011. The facility discharges into the receiving waters designated as Northeast Creek, a
stream classified as WS-IV, nutrient sensitive water (NSW) waters within the Cape Fear River
Basin.
2. The Triangle WWTP is permitted to discharge at a rate of 12 MGD and consists of: mechanical
fine screens, mechanical grit collection unit, influent pump station, parshall flume, three 5-stage
BNR trains, four secondary clarifiers, five tertiary filters, UItraviolet disinfection, post aeration,
sludge lagoons, waste and return activated sludge pumping system, and methanol, potassium
hydroxide, and sodium hypochlorite storage and feed systems.
3. Chlorine, Dissolved Oxygen, pH, and Temperature are analyzed in-house. All other parameters
are analyzed by Meritech Laboratories.
4, A file review of the plant records indicated that all records are being kept as required. A
comparison of the April 2009 and August 2009 DMRs indicated no discrepancies.

5. " The plant and laboratory was very clean and well maintained. No operational or maintenance
issues were noted. Visitation and operations logs were reviewed and found to be complete and
satisfactory.
6. A compliance evaluation inspection was also conducted of the facility's Stormwater Permit
NCG110054 during the January 71h visit. All of the facility's stormwater outfalls were observed
during the inspection. The facility has installed several berms at the Headworks to prevent
overspray and excess washdown water from entering Outfall 005. This has complied with the
feasible alternative practices in the SW Permit. In addition, the area downstream of Outfall 002
has become inundated due to beaver dams. This property is located on the periphery of the Army
Corps of Engineers property. Contact Eric Alsmeyer with the Corps at 919-554-4884 regarding
removal of the beavers. All outfalls were in excellent condition.
7. The stormwater pollution prevention plan (SP3) was reviewed. It contained all the required
components and met all requirements. Qualitative and quantitative inspection records were
available for review and found to be complete and acceptable.
The overall conditions of the Durham County Triangle WWTP are compliant with Division standards.

On June 18, 2009, Mack Wiggins and Mandy Hall of the North Carolina Division of Water Quality,
Surface Water Protection Section, Raleigh Regional Office, conducted a stormwater compliance
evaluation inspection of the Durham Triangle, at 5926 NC Highway 55 E. The assistance offered by Mr.
Joseph Pearce, Utility Division Manager, Mr. Tony Mencome, Wastewater Superintendent and Back-up ORC,
Mr. Andy Lambeth, Project Manager and Mr. Boyd McKenzie, Collection ORC and Maintenance Manager was
appreciated. The facility was found to be in Compliance with General Permit NCI 10000 at the time of this
inspection.
I. The following observations were made during the stormwater inspection:
1. The Stormwater Pollution Prevention Plan for this facility contained all the components as
required by NCGI 10000. The facility must continue to aggressively implement the plan.
2. The facility has seven stormwater outfalls. Qualitative monitoring was performed at the
stormwater outfalls as required by NCG 110000. Continue to monitor your outfaIls.
Qualitative monitoring must be conducted during the fall months (September - November),
and during the spring months (April -June) and recorded on the attached forms provided by
DWQ.
3. Analytical monitoring is not required at this facility. The facility does not have any vehicle
maintenance activity and does not use more than 55 gallons of new motor oil per month
when averaged over a calendar year.
4. All chemicals are stored in the secondary containment as required.
5. The facility had improved their mapping to include aerial photographs with location
overlays.

On August 2, 2008, Mr. Mack Wiggins of the North Carolina Division of Water Quality, Surface
Water Protection Section, Raleigh Regional Office, conducted a stormwater compliance
evaluation inspection of our facility. As a result of the SWP3 compliance inspection there were
several observations made that needed to be addressed within our Stormwater Pollution
Prevention Plan. Please note our response for each one of the observations made. "These were:
• Observation #I: Documentation of any spill and corrective actions taken to mitigate spill
impacts when they occur.
Response: Section 4.2 of our Plan addressees this observation and is updated regularly.
• Observation 42: The Stormwater Pollution Prevention Plan must be reviewed and
updated annually.
Response: Section 6.2 of our Plan addresses this observation, however, the word annually
has been inserted into the language to further clarify our current practice of annual
updates, see attachment # 1.
• Observation #3: The plan must addresslincltide a feasibility study, whereby the possible
elimination or reduction of exposure of pollutants to stormwater is assessed.
Response: Roadway weep holes for stormwater collection system have had plugs placed
within them. This will capture any small spill and can be opened after cleanup.
• Observation 44: The plan must show employee signatures. Employee signature document
that the employee has received training for spill response, cleanup procedures and
preventive maintenance.
Response: Section 5.3 of our Plan addressees this observation, however, at the time of
this inspection the latest documentation was not copied into the Plan. The original
training documents are kept in a separate file near the operations area of the
administrative building. This oversight has been corrected and moving forward greater
emphasis will be placed on keeping the documents in both places, see attachment #2.

6.0 ANNUAL SITE COMPLIANCE EVALUATION & PLAN REVISIONS
ATTACHMENT 01
6.1 ANNUAL SITE COMPLIANCE EVALUATION
Triangle WWTP must conduct an Annual Site Compliance Evaluation of the storm water
management practices in use. The objective of the evaluation is to assess the .overall
effectiveness of this SWP3 and to modify and improve the SWP3, as appropriate. This annual
evaluation is conducted by Triangle Wastewater Treatment Facility's SWP3 Team and typically
occurs during the fourth quarter of each year. The evaluation includes the following elements:
• Inspect storm water drainage areas for evidence of pollutants entering the drainage
system;
• Ensure the drainage area delineations remain accurate and the figures are updated, as
necessary;
• Assess whether improvements to BMPs or additional control measures are needed;
and
• Revise the list of significant materials, industrial activities and potential pollutant
sources, as necessary.
The Annual Site Compliance Inspection form contained in Appendix G will be used to record the
findings of the evaluation. Completed inspection forms will be maintained in Appendix G of this
SWP3.
6.2 PLAN REVISIONS
Based on the findings of the Annual Site Compliance Evaluation, the SWP3 must be revised
annually to reflect current conditions. Necessary facility changes will be implemented as soon as
practical. Facility changes in design, construction, operations or maintenance that may impact
the potential for storm water pollutants will also require the SWP3 to be amended. Once the
SWP3 is revised, it must be signed by the Operations Manager. Section 1.3 Facility Certification
will be re -certified with each SWP3 revision.
This SWP3 is retained with the storm water records in the Triangle WWTP central files.

Please note that by our acceptance of your no -exposure certification, you are obligated to maintain no -
exposure conditions at your facility. If conditions change such that your facility can no longer qualify for a no -
exposure exclusion, you are obligated to immediately obtain NPDES permit coverage for your stormwater
discharge. Otherwise, the discharge becomes subject to enforcement as an un-permitted discharge. Your
conditional no -exposure exclusion expires in five years (February 28, 2013). At that time you must re -certify
with the Division, or obtain NPDES permit coverage for any stormwater discharges from your facility.
Your conditional exclusion from permitting does. not affect your facility's legal requirements to obtain
environmental permits that may be required under other federal, state, or local regulations or ordinances.

In accordance with your application for a discharge permit received on March 11, 2003 we are
forwarding herewith the subject certificate of coverage to discharge under the subject state — NPDES general
permit. This permit is issued pursuant to the requirements of North Carolina General Statute 143-215 .1 and the
Memorandum of Agreement between North Carolina and the US Environmental Protection agency dated May
9, 1994 (or as subsequently amended).
Please take notice that this certificate of coverage is not transferable except after notice to the Division
of Water Quality. The Division of Water Quality may require modification or revocation and reissuance of the
certificate of coverage.
This permit does not affect the legal requirements to obtain other permits which may be required by the
Division of Water Quality or permits required by the Division of Land Resources, Coastal Area Management
Act or any other Federal or Local governmental permit that may be required.
