On June 11, 2012 Mitch Hayes of the Raleigh Regional Office conducted a compliance evaluation
inspection of the subject facility. The assistance of Darrell Highsmith, Amanda James, Angie Boswell,
and you were gratefully appreciated. Please find the attached Basin Wide Information Management
System (BIMS) inspection checklist summarizing each inspection. Below is a list of recommendations
and findings developed from the inspection:
The reissued NPDES Permit NCO030317 effective July 01, 2010 and will expire October 31,
2014. Permit renewal is required within 6 months of the permit expiration date.
2, BIMS lists L. Kirk Bass as the Operator in Responsible Charge (ORC) wastewater grade 4
certified (WW-4), Darrell Highsmith as back-up ORC (bORC) WW-4, and David J. Gardner as
bORC WW-4.
3. The 21.0 MGD wastewater treatment facility consists of the following units: influent screw pump
station with 4 screws; 4 automatic bar screens with grit channels; 4 parallel biological nutrient
reduction (BNR) trains utilizing the Anaerobic — Anoxic — Oxic (A20) process with a total of 8
aeration basins, 8 secondary clarifiers and 8 tertiary filters; 1 chlorine contact basin providing 30
minute detention time; 1 dechlorination contact basin; post aeration basin; 4 — 1.5 million gallon
aerobic digesters with pump station; 2 dissolved air floatation (DAF) thickeners with pump
station; 1 sludge truck loading station; an oxygen, generation plant; 3 on site generators capable
of operating the entire plant; 2 reclaim secondary clarifiers with pump station; in house plant
process water distribution system and 1 sodium hydroxide feed system for pH control.
4. At the time of inspection, automatic and manual bar screens and grit removal system were
functioning normally. The following were not in service due to low inflow: 2 out of 4 influent
screw pumps; the "C° automatic bar screen; aeration basin B and the 2 associated rectangle
secondary clarifiers and the # 4 tertiary clarifier; the DAF thickeners and associated pump
station and the #3 digester. The oxygen generation plant was offline and is considered a back-
up source for oxygen. Compressed liquid oxygen is used as an oxygen source in the A20
process. Sludge blanket in the C1 seconda clarifier was 3 feet and 5 feet in secondary clarifier

D2. Both clarifiers have 15 foot sidewall depths. Effluent in the wet well at the tertiary filters
appeared clear with no foam. Effluent appeared cleared flowing over the weirs in clarifiers C1
and D2. Effluent flowing into the Tar River appeared clear with little foam, dissipating within 30
feet.
5. Biosolids are land applied under the City of Rocky Mount Land Application Permit WQC005568.
The permit will expire October 31, 2016. Granville Farms Inc. is contracted for transportation,
application, and overseeing of the land application program.
6. The laboratory was checked in a cursory manner. All instrument calibration logs are maintained
in a log book. Fecal coliform bath temperature was 44.3 degrees C and the laboratory
refrigerator was 1.5 degrees C. BOD incubator temperature was 19.8 C. All thermometers are
checked against a certified thermometer. The influent and effluent flow meters were calibrated
June 05, 2012 by plant staff. The influent sampler temperature was 0.5 degrees C and the
effluent sampler temperature was 1 degree C.
7. Discharge Monitoring Reports (DMR's) for the period December 2010 through April 2012 were
reviewed for compliance with permit limits and monitoring requirements. A Notice of Violation
was issued for not meeting daily minimum pH on August 31, 2011. This violation has been
addressed. There were no other violations listed for the review period.
8. The Permittee is a member of the Tar -Pamlico River Basin Association. The Phase III Nutrient
caps for the Tar -Pamlico River Basin Association for total nitrogen is 891,272 pounds per year
(lblyr) and 161,070 Ib/yr for total phosphorus. Based upon the 2011 annual report, the Tar -River
Basin Association was well under the nutrient cap for total nitrogen and total phosphorus.

9. Inspection of the stormwater outfalls and plant site were conducted at the same time as the CEI
for the Wastewater Treatment Facility. Permit NCG110030 was reissued and became effective
on June 01, 2008 and will expire on May 31, 2013. Permit renewal is required within 6 months
prior to permit expiration date.
10. There are 2 stormwater outfalls listed for the facility. The last completed stormwater qualitative
monitoring report for both outfalls was December 22, 2011. There is no vehicle maintenance
done on site. The stormwater pollution prevention control plan is reviewed and amended if
needed, annually. Please be sure to document all spill and clean up efforts that occur in keeping
with requirements of your stormwater pollution prevention plan. Annual employee training on
spill prevention and response occurred on May 07, 2012,

1, Dave Parnell of the Raleigh Regional Office of the Division of Water Quality conducted two compliance
evaluation inspections (CEI) on March 15, 2010, consisting of the NPDES wastewater and stormwater
inspections. The assistance of L. Kirk Bass, Operator in Responsible Charge (ORC), Darrell Highsmith,
Backup ORC, Angela Boswell, Compliance Administrator and Brent Bent, Laboratory Supervisor, was
appreciated. Please review the attached Basinwide Information Management System (BIMS) inspection
checklists summarizing the inspections.
Below is a list of findings and recommendations developed from the inspections:
1. NPDES Wastewater Inspection,
1. The current permit re -issuance is pending. The application timeframe was adhered to by the facility.
2. The 21.0 MGD wastewater treatment plant consists of the following units: influent pump station,
mechanical bar screens, grit chamber, parshall flume, pure oxygen A/O process aeration basin,
secondary clarifiers, tertiary filters, chlorination basins, de -chlorination basins, post aeration basins,
and aerobic sludge digesters.

3. The inspection began in the treatment plant office. All paperwork was well organized and easily
accessible. Discharge Monitoring Reports were reviewed for compliance with permit limits and
monitoring requirements. Chain of custody records were available and found to be in order. The
laboratory is state certified and performs the majority of permit required parameters. A contract lab,
Tritest, performs the remainder of the parameters required by the permit.
4. Overall maintenance was found to be in good condition. The mechanical bar screen was in
acceptable condition. Grit removal was free of organic matter and excessive odor. Debris is
removed as needed and taken to the landfill.
5. The clarifiers were functioning properly and were free of weir blockages and showed no sign of
short-circuiting. The sludge blanket was reported to be 3 feet, which is within the acceptable range
of sidewall depth.
6. Biological nutrient removal is conducted at the facility. The plant meets its obligation to the Tar -
Pam Association.
7. The tertiary filters, oxygen production system, chlorination and de -chlorination systems, all,
appeared to be functioning properly.
8. The aerobic digester appears to have adequate capacity with no objectionable odor.
9. Standby power, consisting of three generators, is available and is tested on a monthly basis. The
generators have the capacity to run the entire wastewater treatment site.
10. The effluent pipe is located across from the treatment plant entrance on NC 97. The effluent
discharging into Tar River appeared to be clear. The receiving water is free of foam and other
debris.
II. NPDES Stormwater Inspection:
1. The stormwater pollution prevention plan (SP3) was reviewed and found to be compliant with the
permit. All regular inspections and duties, including training, annual review, qualitative monitoring,
knowledge and inspections of secondary containment are being accomplished as required by the
stormwater permit.

Dear Mr. Hollowell:
I, Ron Boone, of the NC Division of Water Quality (DWQ), Raleigh Regional Office (RRO), Surface
Water Protection (SWP) section, conducted wastewater and stormwater compliance evaluation inspections
(CEI) of the subject facility on 8 January 2008. The cooperation and assistance of the Operator in
Responsible Charge (ORC), Leon K. Bass, and the backup ORCs, Darrell Highsmith and Angela Boswell,
were greatly appreciated. The results of the CEI are summarized below and in the attached inspection
reports.
2. This is a Grade IV facility and it is located at 3031 Water Treatment Plant Road in Rocky Mount,
Edgecombe County, NC. It is just north of NC Hwy 97, between Deer Run Road and Cool Springs Road.
The NCO030317 permit allows the discharge of treated wastewater into the Tar River from the Tar River
Regional Wastewater Treatment Plant (WWTP) and the NCGI10030 permit/Certificate of Coverage
(COC) allows the discharge of stormwater to the Tar River from stormwater point source discharges
associated with activities classified as treatment works treating primarily domestic sewage. This section
of the Tar River is currently classified as a Class C, nutrient sensitive water.
3. The plant description in the permit is accurate except for the sludge thickener. This unit can be added to
the plant description in the next permit renewal cycle.
4. The plant is very well operated and maintained. The grounds were well kept and all equipment was in
very good condition. Some minor maintenance work was being done to the grit removal units that were
inactive at the time of the inspection.
5. The flow meters are calibrated monthly and the composite samplers are set up to draw flow proportional
samples over a 24-hour period. Sample coolers were both at 4 degrees Celsius during the inspection.
6. The clarifiers, tertiary filters, chlorination and dechlorination systems and final aeration systems were all
operating normally. No discrepancies were noted.

Two of the four 1.5 million -gallon aerobic digesters were full and in operation during the inspection. Mr.
Highsmith stated that they would likely place another in operation soon to increase capacity and help
avoid foaming problems.
The operator's visitation and operations and maintenance logs were complete and up-to-date. The
laboratory records were also found to be complete and accurate. A review of discharge monitoring reports
for the months-,of"July and October 2007 yielded no errors or inconsistencies with laboratory bench sheets.
The lab is state certified and performs the majority of the permit -required parameters except for the
metals, toxicity'and-pnority pollutants scans.
9. The stormwater pollution prevention plan required by the NCG110030 permit was reviewed and found to
be complete and up-to-date. All regular checks and duties, including training, annual review, qualitative
monitoring, inspection of secondary containments, etc, are being accomplished as required.
10. Please continue your diligence in the proper operation and maintenance of the Tar River WWTP. Should

On 8 November 2006, I, Ron Boone, of the NC Division of Water Quality (DWQ), Raleigh Regional
Office (RRO), conducted a compliance evaluation inspection at the Tar River Regional Wastewater
Treatment Plant (WWTP) for both of the subject permits. The assistance and cooperation of Mr.
Wayne Hollowell, Plant Superintendent, Darrell Highsmith, Chief Operator, Leon Bass, Electrical
Supervisor, and Dennis Carpenter, Maintenance Supervisor, was helpful and appreciated. Inspection
reports are attached for your records. Inspection findings are listed below.
2. This plant discharges to the Tar River, this segment of which is classified as a class C, nutrient
sensitive water (NSW) in the Tar Pamlico river basin. The plant is as described in the permit. There
is ongoing construction to install additional aerobic digesters.
3. All plant components were inspected. In general the plant was very clean and well maintained. No
components were noted as inoperable or poorly maintained.
4. The plant experienced a marked increased in flow on the 7th/8th of November as a result of the rain
event of 7 November. According to the operators, just over an inch of rain fell during this rain event.
The marked increase in plant flow strongly indicates the presence of heavy inflow/infiltration (1&1) in
the collection system. The City must undertake measures to identify and correct all sources of I&I in
a timely fashion. Such I&I not only makes the City vulnerable to violations related to the collection
system bui also jeopardizes the plant's ability to treat the wastewater and reduces its ability to remain
compliant with permit limits. Efforts to address this problem should be initiated immediately as
rehabilitation of the collection system can be a very time consuming process.
5. Our records indicate that Mr. Bass is the Operator in Responsible Charge (ORC) of the plant and that
Mr. Hollowell and Mr. Highsmith are backup ORCs. Please submit the necessary paperwork to the
Division to ensure the ORCs are properly reflected in our records.

6. All plant records were reviewed including the operator's -visitation -log, operations and maintenance
logs, discharge monitoring reports (DMR) for 2006, laboratory calibration records and chains of
custody. All were up to date and compliant. Additionally, January and March 2006 DMRs were
compared to laboratory bench sheets and no transcription errors were noted.
7. The influent and effluent samplers were inspected. The thermometer in the influent sampler read 0.7
degrees Celsius at the time of the inspection. Please have your laboratory personnel check the
accuracy of this thermometer and then adjust the temperature setting for the sampler to maintain the
temperature between 1.0 and 4.4 degrees Celsius.
8. The discharge from the plant was observed. No excessive foam, solids or other pollutant indicators
were noted. The right-of-way to the outfall is well maintained.
9. The stormwater program was also reviewed. All records of qualitative monitoring, facility
inspections, secondary containment inspections and employee training, as well as the site map and
general location map were reviewed and found to be compliant. Stormwater discharge outfalls were
inspected and no excessive foams, solids, sheens, or other pollutant indicators were noted.
