are forwarding herewith the subject certificate of coverage (COC) to discharge under the subject state
- NPDES general permit. This permit is issued pursuant to the requirements of North Carolina General
Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the US
Environmental Protection Agency dated October 15, 2007 (or as subsequently amended).
If any parts, measurement frequencies, or sampling requirements contained in this permit are
unacceptable to you, you have the right to request an individual permit by submitting an individual
permit application. Unless such demand is made, this certificate of coverage shall be final and binding.
Please take notice that this certificate of coverage is not transferable except after notice to the
Division of Energy, Mineral and Land Resources (Division). The Division may require modification or
revocation and reissuance of the Certificate of Coverage. This permit does not affect the legal
requirements to obtain other permits which may be required by the Division or permits required by
the Division of Water Resources, Coastal Area Management Act or any other federal, state or local
governmental permit that may be required.

On February 6, 2013, a compliance evaluation inspection (CEI) of your auto salvage facility, was
performed by Dave Parnell of the Division of Water Quality (DWQ) Raleigh Regional Office. The CEI
encompassed the stormwater permit requirements. Your assistance was appreciated.
NPDES NCGIO0210 - Stormwater Permit Inspection:
1. The current general permit was issued on November 1, 2012, and it expires on October 31,
2017.
2. The Stormwater Pollution Prevention Plan (SPPP) was requested for review during the
inspection, and was available. There were deficiencies note at that time. Those noted
were;
• A list of significant spills must be retained within the SPPP for three years.
• Documentation of annual employee training.
• Documentation of annual review and update of the SPPP.

3. During the CEI, it was noted that Qualitative Monitoring was not being conducted as the
permit required. We discussed the proper format for documenting the Qualitative
Monitoring. During the site visit, the Stormwater Discharge Outfall location was reviewed
and the sample location designated. Please use or follow closely the monitoring form
that was reviewed during the inspection.
4. Analytical Monitoring Reporting was last documented during the first half of 2012.
Failure to monitor is a permit violation and may result in a Civil Penalty Assessment. You
must sample as required by your permit. Please forward the Analytical Monitoring
Results to this office for the current monitoring period, as soon as your receive the
results.
Cl Auto Used Parts, LLC, is non -compliant with permit conditions of the NPDES
NCGl00210 permit. Please provide a written response to this letter, including a schedule
with dates for completion, within 30 days of its receipt. Please understand permit
conditions violations are subject to civil penalty assessments of up to $25,000 per
violation. It is requested that you address in writing your compliance efforts with
respect to all of the bold items noted above. It is the intentions of this office to conduct
a follow-up inspection, in order to determine if permit condition enforcement is
