Please find attached the Stormwater Discharge Outfall (SDO) Monitoring Report for the LKQ
East Carolina facility located in Lagrange, North Carolina. This submittal covers the ninth monthly
monitoring period (March 2019) of the Tier Two response. No Discharge occurred during the

Dear Mr. Smith:
Enclosed is your monthly Stormwater Discharge Outfall (SDO) Monitoring Report. No samples were
submitted for tab analysis by your facility for March. You are required by the permit to submit your SOO
Monitoring Report to the Division of Water Quality (DWQ). A cover letter to the DWQ for transmittal of the
SOO Monitoring Report is also attached here for your convenience. Please print the cover letter and two
copies of the SOO Monitoring Report, and sign and date inside the gray boxes oflhe cover letter. If no
discharge occurred at your facility within operating hours for this monitoring period, please check the "No
Discharge" box at the too of the table on page 1 on both copies of the SOO Monitorinq Reoorts to indicate
that No Discharge occurred. Please date the date submitted section at the top of the SOO Monitoring
Report. Please also sign and date the certification sections on the second page of both copies of the SOO
Monitoring Report (note: you will need to sign both the Solvent Management Plan certification on the top of
the second page, and permittee certification the bottom on the second page of the SOO Monitoring Report).
The original signed cover letter and SOO Monitoring Report should be sent via certified mail with a return
receipt or via Fed -Ex to the DWQ at the address shown on the cover letter. You should make copies of the
letter and form and file them in the monitoring section of your Storm Water Pollution Prevention Plan
(SPPP). The original signed cover letter and the two sinned copies of the SOO Monitoring Report should
be sent via certified mail with a return receipt or via Fed -Ex to the DWQ at the address shown on the cover

MONITORING REPORT
Facilities that incorporate a solvent management plan into the Stormwater Pollution Prevention Plan may request that DWQ waive monitoring of total toxic organics. The solvent
management plan shall include a list of the total toxic organic compounds used and the other elements listed in the General Permit. For those facilities allowed such a waiver, the
discharger shall sign the following certification statement:

"Based upon my inquiry of the person or persons directly responsible for managing compliance with the permit monitoring requirement for total toxic organics (TTO),
certify that to the best of my knowledge and belief, no dumping of concentrated toxic organics into the stormwater or areas which are exposed to rainfall or
stormwater runoff has occurred since filing the last discharge monitoring report. I further certify that this facility is implementing all the provisions of the solvent
management plan included in the stormwater Pollution Prevention Plan."
