A cover letter to the DWQ for transmittal of the SOO Monitoring Report is also attached here for your convenience. Please print the cover letter and two copies of the SOO Monitoring Report, and sign and date inside the gray boxes oflhe cover letter. If no discharge occurred at your facility within operating hours for this monitoring period, please check the "No Discharge" box at the too of the table on page 1 on both copies of the SOO Monitorinq Reoorts to indicate that No Discharge occurred. Please date the date submitted section at the top of the SOO Monitoring Report. Please also sign and date the certification sections on the second page of both copies of the SOO Monitoring Report (note: you will need to sign both the Solvent Management Plan certification on the top of the second page, and permittee certification the bottom on the second page of the SOO Monitoring Report). The solvent management plan shall include a list of the total toxic organic compounds used and the other elements listed in the General Permit. For those facilities allowed such a waiver, the discharger shall sign the following certification statement: Solvent Management Plan Certification Statement "Based upon my inquiry of the person or persons directly responsible for managing compliance with the permit monitoring requirement for total toxic organics (TTO), certify that to the best of my knowledge and belief, no dumping of concentrated toxic organics into the stormwater or areas which are exposed to rainfall or stormwater runoff has occurred since filing the last discharge monitoring report.
