Facilities that incorporate a solvent management plan into the Stormwater Pollution Prevention Plan may request that DWQ waive monitoring of total toxic organics. The solvent
management plan shall include a list of the total toxic organic compounds used and the other elements listed in the General Permit. For those facilities allowed such a waiver, the
discharger shall sign the following certification statement:

Stormwater Discharge Outfall Monitoring Report - Tier Two Response
Dear Sir/ Madam:
Please find attached two signed copies of the Stormwater Discharge Outfall (SDO) Monitoring Report for the LKQ
Self Service Raleigh facility located in Garner, North Carolina. This submittal covers the February 2017 monthly
monitoring period of the Tier Two response. The February 2017 monitoring period represents the third consecrative
monthly monitoring event in which sample results were below the benchmark values or within the benchmark range.
We will resume semi-annual monitoring in accordance with the general permit; therefore the next sampling event at

management plan shall include a list of the total toxic organic compounds used and the other elements listed in the General Permit. For those facilities allowed such a waiver, the

Enclosed is the Discharge Monitoring Report (DMR) for the 2002 monitoring period.
Please review the form to ensure the facility information is correct.
As a condition of your permit, you are required to fill out and submit a qualitative
monitoring report for each permitted outfall regardless of analytical reporting. This
form is just an observation of water quality. You are required to submit the
qualitative monitoring form twice a year for the duration of the permit. Please sign
and date both forms, fill out the qualitative monitoring report enclosed if you have not
already done so, make a copy of the forms for your files, and send them in the pre -
addressed certified mail envelope provided for tracking purposes.

In response to your renewal application for continued coverage under general permit NCG100000. the Division of
Water Quality (DWQ) is forwarding herewith the reissued stormwater general permit. This permit is reissued
pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement
between the state of north Carolina and the U.S. Environmental Protection Agency, dated December 6, 1983.
The following information is included with your permit package:
* A new Certificate of Coverage
* A copy of General Stormwater Permit NCG 100000
* A copy of the Analytical Monitoring Form (DMR)
* A copy of a Technical Bulletin for the general permit
Your coverage under this general permit is not transferable except after notice to DWQ. The Division may require
modification or revocation and reissuance of the Certificate of Coverage. This permit does not affect the legal
requirements to obtain other permits which may be required by DENR or relieve the permittee from responsibility
for compliance with any other applicable federal, state, or local law, rule, standard, ordinance, order, judgment, or
decree.

This certificate of coverage shall become effective September 1, 2002.
