Attached please find the Stormwater Discharge Monitoring Report (DMR) and analytical
report for the sampling conducted at the above -referenced site. Stormwater samples
were collected from Outfall 001 on November 18, 2015 and analyzed for total suspended
solids (TSS), total recoverable lead, oil and grease and ethylene glycol. As indicated on
the attached DMR, all parameters were reported below or within their respective

Attached please find the Stormwater Discharge Monitoring Report (DMR)1 and analytical
report for the sampling conducted at the above -referenced site. Stormwater samples
were collected from Outfali 001 on April 15, 2015 and analyzed for total suspended solids
(TSS), total recoverable lead, oil and grease and ethylene glycol. As indicated on the
attached DMR, all parameters were reported below their respective benchmark value.

MONITORING REPORT
Facilities that incorporate a solvent management plan into the Stormwater Pollution Prevention Plan may request that DWQ waive monitoring of total toxic organics. The solvent
management plan shall include a list of the total toxic organic compounds used and the other elements listed in the General Permit. For those facilities allowed such a waiver, the
discharger shall sign the following. certification statement:

MONITORING REPORT
Facilities that incorporate a solvent management plan into the Stormwater Pollution Prevention Plan may request that DWQ waive monitoring of total toxic organics. The solvent
management plan shall include a list of the total toxic organic compounds used and the other elements listed in the General Permit. For those facilities allowed such a waiver, the
discharger shall sign the following certification statement:

Attached please find the Stormwater Discharge Monitoring Report (DMR)1 and analytical
report for the sampling conducted at the above -referenced site. Stormwater samples
were collected from Outfall 001 on April 15, 2015 and analyzed for total suspended solids
(TSS), total recoverable lead, oil and grease and ethylene glycol. As indicated on the
attached DMR, all parameters were reported below their respective benchmark value.
Please note that pH measurements will be collected in the field during the next semi-
annual monitoring event (July -December 2015). If you have any questions or need

On behalf of McLamb's Auto Salvage, Inc., this letter and the attached Stormwater
Discharge Outfall Monitoring Report serve as notification that stormwater samples were
unable to be collected from the facility outfall during the previous semi-annual sampling
period (July -December 2018). During this timeframe, a measurable storm event did not
occur at the facility at a time when a stormwater sample could be safely collected (Le.
during daylight when the facility was occupied). Therefore, this submittal is intended to
serve as notification of "No Discharge" and to satisfy the requirements outlined in Part Il,
Section B of General Permit NCG100000 and NCAC T15A 0213.0506.
If weather conditions permit, McLamb's Auto Salvage personnel will endeavor to collect
representative stormwater samples during the current sampling period (January -June
2019). If you have any questions or need additional information, please contact me at

STORMWATER DISCHARGE OUTFALL (SDO)
MONITORING REPORT
Facilities that incorporate a solvent management plan into the Stormwater Pollution Prevention Plan may request that DWQ waive monitoring of total toxic organics. The solvent
management plan shall include a list of the total toxic organic compounds used and the other elements listed in the General Permit. For those facilities allowed such a waiver, the
discharger shall sign the following certification statement:

"Based upon my inquiry of the person or persons directly responsible for managing compliance with the permit monitoring requirement for total toxic organics (TTO), I
certify that to the best of my knowledge and belief, no dumping of concentrated toxic organics into the stormwater or areas which are exposed to rainfall or
stormwater runoff has occurred since fling the last discharge monitoring report. I further certify that this facility is implementing all the provisions of the solvent
management plan included in the Stormwater Pollution Prevention Plan."
