3For purposes of this permit the definition of Total Toxic Organics is that list as stated in Appendix D, Table II of Chapter 40 Code of Federal Regulations (CFR) Part 122.
Facilities that incorporate a solvent management plan into the Stormwater Pollution Prevention Plan may request that DWQ waive monitoring of total toxic organics. The solvent
management plan shall include a list of the total toxic organic compounds used and the other elements listed in the General Permit. For those facilities allowed such a waiver, We

Please find attached the 151 Semi -Annual Stormwater Discharge Outfall (SDO) Monitoring Report for the LKQ
Self Service Greenville facility located in Greenville, North Carolina. Please note that samples were collected at the
facility in January 2015; however, due to a miscommunication with LKQ's previous consultant the January 2015
results were not released to us. Our current consultant, RPS GaiaTech, contacted your office in late July and was
advised that samples should be collected as soon as possible during the 2A semi-annual monitoring period for the
111 semi-annual monitoring period. As such, this submittal is intended to satisfy the monitoring requirements for the
first semiannual monitoring period for 2015. A second sample for 2015 will be collected in the fourth quarter and the
results will be submitted to the Division of Water Quality later in the year,

the permit to submit your SDO Monitoring Report to the Division of Water Quality (DWQ). A cover
letter to the DWQ for transmittal of the SDO Monitoring Report is also attached here for your
convenience. Please print the cover letter and the SDO Monitoring Report, and sign and date inside the
gray boxes of the cover letter. If no discharge occurred at your facility within operating hours for this
monitoring eriod lease write "No Dischar e" on the top line of the table on page I of the SDO
Monitorint=. Report form to indicate that No Discharge occurred, and sign and date the form in the
certification sections on the second ap he. Note that you must sign both the Solvent Management Plan
certification on the top of the second page, and the permitter certification on the bottom on the second
page. The original signed cover letter and SDO Monitoring Report should be sent via certified mail with a
return receipt or via Fed -Ex to the DWQ at the address shown on the cover letter. You should make
copies of the letter and form and file them in the monitoring section of your Storm Water Pollution
Prevention Plan (SPPP).
Please note that if discharge of storm water from your facility actually did occur during the referenced
Monitoring period and within operating hours, but you failed to collect a sample, you should not sign the
SDO Monitoring Report to indicate that no discharge occurred. Please contact me via email
